Diagnostic accuracy of different imaging modalities following computed tomography (CT) scanning for assessing the resectability with curative intent in pancreatic and periampullary cancer.

BACKGROUND: Periampullary cancer includes cancer of the head and neck of the pancreas, cancer of the distal end of the bile duct, cancer of the ampulla of Vater, and cancer of the second part of the duodenum. Surgical resection is the only established potentially curative treatment for pancreatic and periampullary cancer. A considerable proportion of patients undergo unnecessary laparotomy because of underestimation of the extent of the cancer on computed tomography (CT) scanning. Other imaging methods such as magnetic resonance imaging (MRI), positron emission tomography (PET), PET-CT, and en

[Pancreatic hormonal function and proteolytic activity in peptic ulcer].

Blood serum insulin, glucagon, pepsinogen, trypsin was studied by radioimmunological methods in 95 patients with ulcer disease. Fasting values and values 1 and 2 hours after a standard breakfast (1212 kcal) were evaluated. It was established that all patients showed a statistically valid increase of the basal level of glucagon while patients with gastric ulcer showed an increase of the basal insulin level. Use of a test breakfast showed reserve and compensatory capacities of the hormonal pancreatic function. Patients with gastric and duodenal ulcer revealed an increase of the pepsinogen level

Case report: acute pancreatitis induced by Clozapine.

Two percent of acute pancreatitis are drug induced. In the present paper, we reported the case of a 39 year-old patient with chronic-hallucinatory schizophrenia who developed symptomatic pancreatitis during the clozapine dose titration performed to reach the therapeutic range. Diagnosis of pancreatitis was suggested by clinical examination and abnormal laboratory values of pancreatic enzymes and confirmed by C-T scan and ultrasonography. The causal incrimination of clozapine in this case seems likely as all other possible causes of pancreatitis were excluded, as AP developed shortly after the

Non-occlusive mesenteric ischemia and its associated intestinal gangrene in acute pancreatitis.

BACKGROUND/AIMS: Non-occlusive mesenteric ischemia (NOMI) has been defined as diffuse intestinal ischemia that often results in intestinal gangrene in the presence of a patent arterial trunk. The prevalence and nature of NOMI in acute pancreatitis was investigated. METHODS: A total of 120 consecutive patients with acute pancreatitis managed in the Department of Surgery II, Kumamoto University Medical School, from April 1992 through December 2002, were investigated retrospectively. Among them, 60 patients had the severe form. RESULTS: The overall mortality of acute pancreatitis patients was 8

[How much diagnostics in patients with pancreatic cancer?].

The possibilities which actually exist in order to obtain an earlier diagnosis in pancreatic carcinoma, a better differentiation of mass-forming pancreatitis and of other pancreatic tumors and an improved staging for reducing unnecessary operations are presented and evaluated. Possible relevant hints of the history and indicators for diagnostic procedures in view of a pancreatic carcinoma are upper abdominal complaints especially in relation to a new developed diabetes mellitus and/or unexplained elevations of pancreatic enzymes. The endosonography in combination with a target biopsy is of imp

Reduction in insulitis following administration of IFN-gamma and TNF-alpha in the NOD mouse.

In insulin dependent diabetes mellitis (IDDM) beta cell destruction is associated with infiltration of the pancreatic islets by T lymphocytes and macrophages. Cytokine products from the infiltrating immunocytes not only have powerful immunoregulatory actions but also are capable of impairing islet cell functions and have thus been postulated to assume a central role in mediating anti-beta cell immunity and beta cell destruction. In an effort to explore further the role of cytokines in the pathogenesis of IDDM, we examined clinical, metabolic and pathological features of NOD/Wehi mice injected

Altered bone metabolism and bone density in patients with chronic pancreatitis and pancreatic exocrine insufficiency.

CONTEXT: Due to maldigestion, pancreatic exocrine insufficiency (PEI) in chronic pancreatitis may lead to deficiencies in fat-soluble vitamins, including vitamin D. This may, in turn, can cause disturbances in bone metabolism and reduce bone mineral density. OBJECTIVE: To conduct a prospective study of maldigestion, bone metabolism, and bone mineral density in a group of patients with chronic pancreatitis. METHODS: A total of 50 male patients with proven chronic pancreatitis (36/50 alcohol; 42/50 smokers) were studied. Pancreatic exocrine function was assessed using the fecal elastase-1 test

Comments on "duct-related complications of pediatric post-traumatic pancreatitis": Expanding the discussion.

• Clarification of CP diagnosis: Criteria like imaging and functional tests are lacking. • Severity and ascitic fluid: Scoring systems, SAAG, and cultures could guide better care. • Questionable spironolactone use: Pancreatic ascites needs ductal repair, not diuretics. • Vancomycin monotherapy lacks justification without microbiological evidence. • Pediatric guidelines: NSAIDs are not routinely recommended; focus on supportive care. • Imaging for ERCP: Definitive evidence is missing; MRCP could complement diagnostics.

Early-phase insulin secretion during mixed-meal tolerance testing predicts β-cell function and secretory capacity in cystic fibrosis.

UNLABELLED: Insulin secretion within 30 minutes of nutrient ingestion is reduced in people with cystic fibrosis (PwCF) and pancreatic insufficiency and declines with worsening glucose tolerance. The glucose potentiated arginine (GPA) test is validated for quantifying β-cell secretory capacity as an estimate of functional β-cell mass but requires technical expertise and is burdensome. This study sought to compare insulin secretion during mixed-meal tolerance testing (MMTT) to GPA-derived parameters in PwCF. METHODS: Secondary data analysis of CF-focused prospective studies was performed in PwC

Maternal diabetes on fetal endocrine pancreas development in rats.

To evaluate if an unfavorable maternal environment induced by hyperglycemia, hypoxia, and oxidative stress status impairs the morphological development of pancreatic islets in the fetuses on days 18 and 21 of pregnancy. Wistar rats were injected with streptozotocin for diabetes induction. At adulthood (3 months), all animals underwent an oral glucose tolerance test (OGTT) for glucose assessment as an inclusion criterion. Following, the animals were mated. On day 18 and 21 of pregnancy, the mothers were killed for blood biochemical data and, fetal pancreas was collected for immunohistochemical

Repercussions of low fructose-drinking water in male rats.

Fructose consumption has increased worldwide, and it has been associated with the development of metabolic diseases such as insulin resistance (IR) and steatosis. The aim was to evaluate if lower fructose concentrations may cause pancreatic structural abnormalities, leading to a glucose intolerance without steatosis in male rats. Young male rats orally received 7% fructose solution for 12 weeks. Body weight, food, water, and energy intake were measured. An oral glucose tolerance test (OGTT) was performed. After final experimental period, all rats were anaesthetized and killed. Blood samples we

Role of the kinin B1 receptor in insulin homeostasis and pancreatic islet function.

Kinins are potent vasoactive peptides generated in blood and tissues by the kallikrein serine proteases. Two distinct kinin receptors have been described, one constitutive (subtype B2) and one inducible (subtype B1), and many physiological functions have been attributed to these receptors, including glucose homeostasis and control of vascular permeability. In this study we show that mice lacking the kinin B1 receptor (B1-/- mice) have lower fasting plasma glucose concentrations but exhibit higher glycemia after feeding when compared to wild-type mice. B1-/- mice also present pancreas abnormali

Influence of catch-up growth on islet function and possible mechanisms in rats.

OBJECTIVE: The purpose of this study was to examine how catch-up growth modulated islet function and what the detailed mechanisms were, especially at various stages and in different forms, of catch-up growth. METHODS: We examined the modulation of islet function during catch-up growth by employing an oral glucose tolerance test and gained some insight into the possible mechanisms involved by measuring general physiologic parameters, pancreatic morphometry, insulin content, and the state of chronic oxidative stress. Correlation analyses were used to assess the correlation of insulin/glucose in
