We reviewed the clinical and histological characteristics of the 44 cases of lupus erythematosus profundus (LEP) that have been encountered … EP to develop during the course of SLE (11 cases). The important histological findings were lobular panniculitis associated with mucin deposition (32 cases) and a tendency to be associated with damage to the basal cell layer. In addition, the direct immunofluorescence test was positive in both the basement membrane (90.5%) and blood vessels (85.7%) in a high p …

Mesenteric panniculitis is a rare disease characterized by chronic non-specific inflammation of mesenteric fat tissue. Several etiologic and/or associated factors have been reported in the literature so far. Although trauma or surgery is one of the potential etiologic factors for mesenteric panniculitis, to the best of our knowledge, no strong correlation has been shown in the literature until now.

An unusual histiocytic dermatitis associated with a lymphoplasmacytic and histiocytic panniculitis appears to represent a previously undescribed disease process. A 56-year-old woman presented with a 5 year history of a violaceous, maculo-papular rash primarily on her legs. It began approximately 8 months before therapy for thrombocytosis. The rash subsequently progressed to form confluent patches and plaques on her torso and arms. The clinical impression was of disseminated granuloma annulare, but multiple biopsies from different sites did not confirm this. Histologic examination revealed a di…

Lobular panniculitis, together with a polyarthritis may complicate pancreatic disease. Abdominal symptoms are frequently absent and mis-diagnosis of the joint-skin complex can lead to inappropriate treatment in a condition with an already high mortality. We report a case of the pancreatitis panniculitis polyarthritis (PPP) syndrome, complicated by metastatic fat necrosis, with bone marrow involvement and characteristic magnetic resonance imaging (MRI) and describe the clinical characteristics, therapy and outcome of patients affected by the syndrome.

Dermatomyositis (DM) is a rare treatable muscle disorder with a reported favorable outcome in most patients. A localized skin/muscle involvement in a DM patient raises questions of definition and causes such as lymphoma, or relapse. We describe here a young treated DM patient who presented a focal biopsy-proven destructive myositis and dermatitis restricted to the left thigh 15 months after the onset of a treated dermatomyositis. There was evidence of subcutaneous lobular panniculitis, somewhat resembling cytophagic histocytic panniculitis associated with a focal inflammatory myopathy with abu…

Pyogranulomatous panniculitis due to infection by Mycobacterium smegmatis was diagnosed in two cats in Finland, a country with a rather cold climate. The diagnosis was confirmed by sequencing of the 16S rRNA gene, which gave a perfect match with the M smegmatis strain ATCC 19420. Gene sequencing makes it possible to distinguish M smegmatis from closely related mycobacteria such as M goodii sp.nov. Diagnosing this entity seems to be a question of having a high index of suspicion. The appearance of the disease as well as sampling is described in detail. In our first case an initial erroneous dia…

Intraabdominal sclerosing panniculitis is a fibroinflammatory lesion of the intraabdominal fat tissue of unknown origin. The authors report 4 secondary cases, that were associated with other kind of intestinal pathology. The cases had different clinical manifestation (mesenterial sclerosis leading to bowel obstruction, lesion simulating transmural spread or peritoneal metastasis of colorectal carcinoma, and chance finding associated with ulcerative colitis). They review the literature and summarize the features of the reactive process characterized by a spindle cell proliferation, fibrosis (sc…

A 43-year-old Maori man presented with a 1 month history of malaise, weight loss, anorexia, arthralgia, recurrent fever and tender erythematous subcutaneous skin lesions. Histological examination of an incisional biopsy of a lesion revealed a lobular panniculitis with an inflammatory infiltrate of atypical lymphocytes and evidence of cytophagocytosis consistent with a diagnosis of subcutaneous T-cell lymphoma. The systemic symptoms and skin lesions resolved spontaneously within 3 weeks, only to recur 2 months later, requiring treatment with oral prednisolone. T-cell gene rearrangement studies …

OBJECTIVE: This study was undertaken to assess the prevalence of mesenteric panniculitis on CT and to describe its appearance and associated diseases.
SUBJECTS AND METHODS: A total of 7620 consecutive abdominal CT examinations were prospectively evaluated for features common to mesenteric panniculitis such as a well-delineated inhomogeneous hyperattenuated fatty mass at the mesenteric root, envelopment of mesenteric vessels, and no evidence of invasion of the adjacent small-bowel loops that may be displaced.
RESULTS: CT findings of mesenteric panniculitis were seen in 49 patients (0.6%). We …

We describe two patients with pancreatitis. One patient had acute pancreatitis of biliary origin and presented with small joint polyarthritis and panniculitis lesions. The other patient was originally hospitalised for dyspnoea with bilateral pleural effusion, and subsequently developed migratory polyarthritis. During his hospital stay he developed panniculitis lesions and a monoclonal IgG disorder of unknown significance. Very few patients with pancreatitis develop polyarthritis and panniculitis. The appearance of pseudocysts in the pleural and mediastinal cavity in the course of pancreatitis …

A 60-year-old woman with chronic renal failure, hyperphosphatemia, normocalcemia and hyperparathyroidism presented with skin and subcutaneous tissue necrosis over the upper lateral aspects of both thighs which became progressively worse with subsequent ulceration over the period of 1 week. A biopsy of the affected skin and subcutaneous tissues demonstrated discrete foci of calcification compatible with the diagnosis of calcifying panniculitis. While it was possible to control the patient's hyperphosphatemia using magnesium carbonate, local ulcer care, including daily whirlpool and repeated loc…

A 68-year-old woman presented with a symmetrical enlargement of the face and submandibular area associated with essential thrombocytosis. Biopsy of the lesions revealed a septal and lobular panniculitis disclosing a lymphohistiocytic and plasma cell proliferation with lymphophagocytosis by histiocytes, compatible with Rosai-Dorfman disease. The lesions regressed spontaneously without therapy over a period of 4 months. Diagnosis was confirmed by electron-microscopic examination and immunocytochemical presence of S-100 antigen within the histiocytic cells. A 2-year follow-up did not demonstrate …

Two patients with mesenteric panniculitis are presented. The first developed obstruction of the sigmoid colon requiring a decompressing proximal sigmoid end-colostomy; the second presented with an incomplete small bowel obstruction. A convincing, immediate, symptomatic response to steroids was noted in both patients. The first patients responded to such an extent that closure of the colostomy was possible some 10 months later. It would appear that active subacute mesenteric panniculitis, as evidenced by continuing fever, high erythrocyte sedimentation rate and predominance of inflammatory cell…

We present 2 cases of subcutaneous fat necrosis associated with pancreatic disease. In 1 of these cases the correct diagnosis led to the detection of a clinically asymptomatic chronic pancreatitis. The pathogenesis of this disease, not yet fully explained, and the differential diagnosis of other forms of panniculitis are reviewed.
