[Communication with children: practical hints and tools for the anesthesiology routine].

BACKGROUND: Pediatric patients represent a special challenge both for the management of anesthesia and for communication, especially the anxious and screaming child. Children have specific features of fears, cognition, comprehension and skills depending on the stage of development. In addition, behavior and anxiety are strongly shaped by the parents who have to be incorporated. AIM: This article presents the special features of children as well as practical strategies and aids for dealing with children in a perioperative setting. MATERIAL AND METHODS: In children suggestibility and susceptib

Explainable Machine Learning Analysis of Perioperative Factors Associated with Clinically Significant Emergence Agitation After Pediatric Ophthalmic Surgery.

Background and Objectives: Emergence agitation (EA) is a common neurobehavioral disturbance during recovery from sevoflurane anesthesia in pediatric patients, particularly after ophthalmic surgery. Clinically deployable and rigorously validated risk stratification approaches remain limited. We aimed to develop and internally validate an explainable machine learning model to estimate individualized EA risk after pediatric ophthalmic surgery. Materials and Methods: This retrospective cohort study included 1029 children aged 3-7 years who underwent ophthalmic surgery under sevoflurane anesthesia

Melatonin or its analogs as premedication to prevent emergence agitation in children: a systematic review and meta-analysis.

BACKGROUND: Emergence agitation (EA) is a prevalent complication in children following general anesthesia. Several studies have assessed the relationship between melatonin or its analogs and the incidence of pediatric EA, yielding conflicting results. This meta-analysis aims to assess the effects of premedication with melatonin or its analogs on preventing EA in children after general anesthesia. METHODS: PubMed, EMBASE, the Cochrane Library, ProQuest Dissertations & Theses Global, Web of Science, CNKI, Wanfang Data, clinicaltrials.gov, and WHO International Clinical Trials Registry Platform

Percutaneous release for trigger thumb in children under local anesthesia.

Trigger thumb surgery can be performed through open surgery or percutaneous release. Open surgery often requires hospitalization, an operating room, a surgical incision, and postoperative wound care; however, percutaneous release does not require hospitalization or surgery, and is relatively easier and faster. We aimed to assess the results of percutaneous A-1 pulley release using local anesthesia without hospitalization for the treatment of pediatric trigger thumb. In this retrospective study, we included patients operated on between March 2013 and August 2020 with the diagnosis of trigger th

Treatment and outcomes of fractures in children: a registry-based cohort study of 10,144 fractures.

BACKGROUND AND PURPOSE: There is no universal consensus on evaluating the quality of pediatric fracture care. While U.S. News ranks hospitals based on surgical timing and procedural benchmarks, additional metrics such as complication rates, reoperations, and patient-reported outcomes could provide a more comprehensive assessment. We aimed to evaluate pediatric fracture care outcomes at a Level I trauma center, emphasizing standardization and transparency in benchmarking. METHODS: This retrospective study reviewed all fractures diagnosed at HUS New Children's Hospital, Helsinki from 2018 to 20

Anesthesia in a patient with mucopolysaccharidosis type VI (Maroteaux-Lamy syndrome).

We report a case of anesthesia during surgery to enlarge the foramen magnum in a pediatric patient with an extremely rare form of mucopolysaccharidosis type VI (Maroteaux-Lamy syndrome). Airway control was unexpectedly easy, and intraoperative anesthetic management with total intravenous anesthesia went smoothly. However, the disease is progressive, with no guarantee that future anesthetic management of this patient will remain easy. If repeated surgery is required, thorough testing should be conducted over time to assess both airway and systemic complications. Nevertheless, we found that safe

Flexible fiberoptic bronchoscopy through the laryngeal mask airway in a small, premature neonate.

The laryngeal mask airway (LMA) was introduced as a supraglottic device in anesthesia for routine use in the normal adult and pediatric population. Because the distal end of properly placed LMA faces the laryngeal inlet, this device can be used as a guide to flexible fiberoptic bronchoscopy (FFB) performance. In this clinical case, we present a small premature neonate with severe congenital biphasic stridor. FFB was performed successfully through the LMA while maintaining a patent airway during general anesthesia and permitting spontaneous respiration, as well as allowing assisted ventilation

Drug-Induced Sleep Endoscopy in Pediatric Obstructive Sleep Apnea: Clinical Applications, Classification Systems, and Anesthetic Considerations.

Background/Objectives: Drug-induced sleep endoscopy (DISE) enables dynamic, real-time visualization of the level, severity, and configuration of upper airway collapse during pharmacologically induced sleep, providing anatomical information that polysomnography cannot. It is increasingly used to guide individualized surgical planning in obstructive sleep apnea (OSA), particularly in children with persistent OSA after adenotonsillectomy or at elevated risk of surgical failure. However, the absence of a universally accepted pediatric-specific classification system and standardized protocol contin

Effects of Virtual Reality Distraction on Pain, Stress, and Heart Rate During Pediatric Ingrown Toenail Surgery: A Randomized Controlled Trial.

OBJECTIVE: Immersive virtual reality (VR) shows efficacy in reducing pediatric procedural pain and stress, yet studies remain heterogeneous and conflate stress with anxiety while neglecting physiological measures such as heart rate (HR). The role of goal-directed attention remains underexplored, as no studies have controlled for VR-interactivity to isolate its contribution under clinically relevant conditions. Thus, this RCT investigates the effects of goal-directed VR vs. free-interaction VR vs. standard-of-care (SoC) distraction (controls) on self-reported pain, stress, and HR during ingrown

An update on the mechanisms and risk factors for anesthesia-related cardiac arrest in children: a narrative review.

The relation between surgery and anesthesia safety in children and a country's Human Development Index (HDI) value has been described previously. The aim of this narrative review was to provide an update on the mechanisms and risk factors of Anesthesia-Related Cardiac Arrest (ARCA) in pediatric surgical patients in countries with different HDI values and over time (pre-2001 vs. 2001‒2024). Electronic databases were searched up to March 2024 for studies reporting ARCA events in children. HDI values range from 0 to 1 (very-high-HDI countries: ≥ 0.800, high-HDI countries: 0.700‒0.799, medium-HDI

Comparison of the efficacy of pentazocine and tramadol for reducing emergence agitation in children after general anesthesia: a retrospective cohort study.

BACKGROUND: Emergence agitation (EA) is a common postoperative complication in pediatric anesthesia that can lead to serious safety events and increased healthcare burden. Both pentazocine and tramadol are frequently used for postoperative analgesia in children, but their comparative effectiveness in preventing EA remains unknown. This study aimed to compare the efficacy of a single intravenous dose of tramadol versus pentazocine for reducing EA in children undergoing tonsillectomy and adenoidectomy. METHODS: This single-center retrospective cohort study included 240 children aged 2-12 years …
