Arterial stiffness is associated with small and large fiber neuropathy: The Maastricht Study.

AIM: To examine the association between arterial stiffness as determined by carotid-femoral pulse wave velocity (cfPWV) and neuropathy (retinal, corneal, and peripheral). METHODS: We used population-based cross-sectional data from The Maastricht Study of N  = 9188 participants (mean age 59.5 years, 50.2% women, 21% had type 2 diabetes) to study the associations between arterial stiffness and retinal nerve layer thickness, retinal sensitivity, corneal nerve measures, peripheral nerve conduction velocities, amplitudes, and vibration perception thresholds. We used linear regression analyses with

Foot drop in the setting of VA-ECMO: A possible alternative mechanism to ischemia.

Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is used widely as part of extracorporeal resuscitation (ECPR). Prolonged VA-ECMO may be associated with the development of foot drop, often secondary to limb ischemia. We report on three patients who developed foot drop following prolonged ECMO use, and we propose an alternative mechanism to ischemia. Three male patients, mean age 46.3 years, underwent peripheral VA-ECMO support, for an average of 22.6 days (17-25 days), two patients following ECPR and one post-cardiotomy shock. All patients underwent distal femoral artery perfusion c

Functional evaluation of a novel fibreglass-reinforced polyamide custom dynamic AFO for foot drop patients: A pilot study.

BACKGROUND: Ankle-foot orthoses (AFOs) are orthopaedic devices often prescribed to treat foot drop. For patients who are not satisfied with off-the-shelf solutions, custom AFOs personalized to the patient's lower limb anatomy are required. Dynamic AFOs provide stability while allowing for physiological ankle mobility in the stance phase of walking. RESEARCH QUESTION: Can a morphology-based dynamic custom AFO made of fiberglass-reinforced polyamide restore a quasi-normal gait pattern and improve comfort in patients with foot drop? METHODS: In this pilot study, the legs and feet of ten foot dr

Extensor Tendon Transfers for Treatment of Foot Drop in Charcot-Marie-Tooth Disease: A Biomechanical Evaluation.

BACKGROUND: In Charcot-Marie-Tooth (CMT) disease, selective weakness of the tibialis anterior muscle often leads to recruitment of the long toe extensors as secondary dorsiflexors, with subsequent clawing of the toes. Extensor hallucis longus (EHL) and extensor digitorum longus (EDL) tendon transfers offer the ability to augment ankle dorsiflexion and minimize claw toe deformity. The preferred site for tendon transfer remains unknown. Our goal was to quantify ankle dorsiflexion in the "intact" native tendon state, compared with tendon transfers to the metatarsal necks or the cuneiforms. We hyp

Correlation between muscle electrophysiology and strength after fibular nerve injury.

Muscle strength measurement is important when evaluating the degree of impairment in patients with nerve injury. However, accurate and objective evaluation may be difficult in patients with severe pain or those who intentionally try to avoid full exertion. We investigated the usefulness of the affected-to-unaffected side electrophysiological parameter ratios as a measure of objective ankle dorsiflexion (ADF) strength in patients with unilateral fibular nerve injury (FNI). ADF strength was measured in patients with FNI via handheld dynamometer and manual muscle test (MMT). Fibular nerve compoun

Common peroneal nerve palsy due to hematoma at the fibular neck.

Common peroneal nerve palsy is an infrequent pathology mostly related to endogenous or exogenous compression. The exogenous compression is frequently related to trauma: knee fractures or hematoma arisen after a direct blow. Fractures may cause a direct lesion of the nerve; hematoma causes a compression of the nerve at the fibular neck causing pain and functional loss. Lesions of the common peroneal nerve can also be related to total knee arthroplasty. The clinical evaluation is characterized by muscle weakness with or without sensory abnormality. The etiopathogeneses of the compression have to

Severe distal muscle involvement and mild sensory neuropathy in a boy with infantile onset Pompe disease treated with enzyme replacement therapy for 6 years.

Enzyme replacement therapy in infantile onset Pompe disease has led to a new phenotype with features not known in the pre-enzyme replacement therapy era. We investigated the origin of a rapidly emerging and severe weakness of the foot dorsiflexors in a 7-year-old boy after 6.5 years of enzyme replacement therapy. Electroneurography yielded normal findings except low compound muscle action potentials of the extensor digitorum brevis muscles after stimulation of the peroneal nerves. Electromyography of the tibial muscle demonstrated a myopathic pattern. Tibial muscle, sural nerve, and skin biops

To Block or Not to Block: Role of Ultrasonography in Guiding an Anesthetic Plan for a Patient With Charcot-Marie-Tooth Disease.

A 31-year-old man scheduled for a fifth metatarsal head resection secondary to osteomyelitis presented to the preoperative holding area for placement of an ultrasound-guided popliteal nerve block as part of a multimodal pain management plan. During the preoperative evaluation, a medical history of CharcotMarie-Tooth disease was noted. The patient had decreased range of motion and neuropathy in both lower extremities and required an assistive device when ambulating. Before placement of the block, a pre-procedure scan of the popliteal fossa revealed abnormal sonoanatomy of the distal sciatic ner

Peroneal muscular atrophy with pyramidal features.

Twenty-five cases of peroneal muscular atrophy with pyramidal features from 15 families are described. This disorder has been referred to as hereditary motor and sensory neuropathy (HMSN) type V by Dyck. Onset was usually in the first two decades of life with difficulty in walking. The clinical syndrome superficially resembled that of HMSN types I and II with distal wasting and weakness involving the legs more than the arms. The tendon reflexes in the upper limbs and at the knee tended to be normal or increased but the ankle jerks were often absent. The plantar responses were extensor in 22 pa

Rare Entrapment of the Deep Peroneal Nerve by a Ganglion Cyst at the Ankle.

Entrapment of the deep peroneal nerve (DPN) is an uncommon yet clinically significant cause of lateral ankle and dorsal foot pain. Among its rare etiologies, ganglion cysts are often overlooked during initial assessments. We present a case of a 56-year-old woman who reported a six-month history of progressive, electric shock-like pain on the dorsum of her left foot, accompanied by a firm mass on the anterolateral ankle. The pain became constant after three months. A physical exam showed a slightly mobile mass and a positive Tinel sign around the DPN. An MRI revealed a cystic lesion compressing

Neurological Complication After Laparoscopic Sleeve Gastrectomy: Foot Drop.

BACKGROUND: The relationship between rapid weight loss and peroneal nerve entrapment neuropathy (PNEN) was shown in various series following bariatric surgery. Thus, we aimed to determine the occurrence of PNEN in our patients who underwent laparoscopic sleeve gastrectomy (LSG) and to reveal the factors contributing to this complication. METHODS: We evaluated our series of 635 patients in terms of neurological symptoms following laparoscopic sleeve gastrectomy, retrospectively. We recorded the preoperative data, laboratory vitamin and nutrient levels, weight loss, electromyography (EMG) findi

Novel imaging techniques using 18 F-florbetapir PET/MRI can guide fascicular nerve biopsy in amyloid multiple mononeuropathy.

BACKGROUND: Multiple mononeuropathy is a rare presentation of primary (AL) amyloidosis and nerve biopsy is usually needed for diagnosis. Conventional imaging is useful to identify proximal nerve involvement but may be inadequate. We report a patient with multiple mononeuropathy whose presentation was suggestive of AL amyloid neuropathy and in whom repeated tissue biopsies were negative for amyloid (including two sensory nerves and one muscle). METHODS: The patient underwent magnetic resonance imaging (MRI) and whole body 18 F-florbetapir positron emission tomography (PET)/MRI. RESULTS: Whole

Muscle-nerve involvement in autosomal dominant progressive external ophthalmoplegia with hypogonadism.

Sixteen members of a family with a history of autosomal dominant progressive external ophthalmoplegia (adPEO) with hypogonadism were examined. The muscular involvement commenced cranially and descended in relation to increasing disease duration. The neuromuscular signs were PEO, dysarthria, dysphonia, limb muscle weakness with wasting, absence of Achilles tendon reflexes, and distal vibration sensory loss. The electromyogram (EMG) was myopathic in facial and proximal limb muscles. Neurogenic involvement was suspected in a few tibial anterior muscles. Neurography showed signs of axonal neuropat

Experimentally induced beriberi polyneuropathy in chickens.

Chickens fed with the same composition of diet as our low income beriberi polyneuropathic patients, developed clinical symptoms of thiamine deficiency in 22.3 +/- 6.3 days. There appeared to be a body store of thiamine which is utilized during a period of deficient intake. Haemoglobin content and serum albumin did not change appreciably during thiamine deficiency. The blood thiamine content was low and the thiamine pyrophosphate (TPP) effect increased to more than 25 percent during the development of the beriberi polyneuropathy, which resumed after one week on thiamine tetrahydrofurfuryl disul

[Posture-related injuries and patient insurance].

The Danish Patient Insurance Society (Patientforsikringsforeningen) was established by law in July 1992. The aim was to provide a just system of compensation for complications following medical treatment, that are not caused by culpous actions or omissions. The first 34 case records concerning position-related nerve injury were reviewed. The material has national coverage. The exact incidence of position related nerve injury in Denmark is unknown, but transitory neurological symptoms after otherwise uncomplicated surgical procedures are probably not infrequent complications. The material inclu
