Resection of posterior fossa tumors in the semi-sitting position in children younger than 4 years of age.

PURPOSE: The semi-sitting position for resection of posterior fossa tumors is a matter of ongoing debate. Here we report about our experience with this approach in children younger than 4 years of age. METHODS: We retrospectively analyzed data of children younger than 4 years of age operated on in our institution in the semi-sitting position over a 15-year period. Patients were intraoperatively monitored for venous air embolism (VAE) by transthoracic Doppler (TTD) or transesophageal echocardiography (TEE). The severity of VAE was classified according to the Tübingen grading scale. Intraoperat

New Tools and Techniques to Prevent CSF Leak in Cranial and Spinal Surgery.

Cerebro-spinal fluid (CSF) leak in cranial and spinal surgery seriously threatens the clinical prognosis of surgical cases exposing the patients to higher risk of infection, prolonged hospital stay, and need of reoperation. Ultimately, this cascade of events may lead to life-threatening complications, as in cases of meningitis/encephalitis, spondylodiscitis, subdural hematomas, and severe pneumocephalus. Moreover, CSF leak causes a consistent rise in healthcare-related costs which are nearly doubled in CSF leak cases after craniotomy or in spinal surgery. The risk of CSF leak tends to be highe

Three-layer reconstruction with fascia lata and vascularized pericranium for anterior skull base defects.

BACKGROUND: We report an assessment of the efficiacy of a triple layer graft composed of fascia lata and vascularized pericranium for anterior skull base reconstruction. This technique is based on the concept that vascularized tissue over a free flap may promote vascularization and rapid wound healing. METHOD: A large fascial graft is prepared from the fascia lata and divided in two pieces and trimmed to a size larger than the bone and dural defect. Vascularized pericranium is harvested after bicoronal incision and elevating the bifrontal scalp flap down to the supraorbital rims. First is dur

Pneumosinus dilatans after prolonged cerebrospinal fluid shunting in young adults with cerebral hemiatrophy. A report of two cases and review of the literature.

Pneumosinus dilatans is a generalized or partial enlargement of the paranasal sinuses containing only air. Pneumosinus dilatans occurs as an idiopathic disorder as well as in association with other disorders, including cerebral hemiatrophy. We report two cases of patients with congenital cerebral hemiatrophy who developed juvenile pneumosinus dilatans of the frontal, ethmoidal, and sphenoidal sinus. The hydrocephalus of both patients was treated by prolonged cerebrospinal fluid shunting. The development of hyperpneumatization of the paranasal sinuses was proved by plain radiographs and CT. Pre

Single-Center Comparison of Chronic Subdural Hematoma Evacuation Outcomes Under Local Versus General Anesthesia.

BACKGROUND: Chronic subdural hematoma (CSDH) is a neurosurgical pathology of an aged populace. Pathogenetic risk factors include traumatic brain injury, prolonged use of antiplatelet drugs, hypertension, and some inflammatory processes. The incidence increases as patients age. Burr-hole evacuation is the most common approach in management of symptomatic cases. We compared evacuation of chronic subdural hematomas with general or local anesthesia (GA and LA, respectively) and evaluated the safety, economic benefits, effects of comorbidity, benefits, and shortcomings of both techniques. METHODS:

Analysis of a Novel Entry Point for Freehand Ventriculostomy Using Computerized Tomography Scans.

Background External ventricular drain (EVD) placement is one of the most common procedures in neurosurgery. Neurosurgeons generally prefer to access the ventricles via Kocher's point since it is the most common point of entry to this area; however, this point is used to describe different anatomic landmarks and is not well-defined. Objective The present study aims to describe and provide an anatomical assessment of a novel ventriculostomy access point developed by the authors using computerized tomography (CT) scans performed on 100 patients. Materials and methods Data were collected from 100

Updated Review of Traumatic Dislocation of the Mandibular Condyle Into the Middle Cranial Fossa.

PURPOSE: Traumatic dislocation of the mandibular condyle into the middle cranial fossa (DMCCF) is a rare event after maxillofacial trauma. Treatment may be performed with closed or open reduction (with or without craniotomy), and arthroplasty procedures might be necessary for long-standing cases. The aims of this study were to perform an integrative review of traumatic DMCCF cases reported in an electronic database and to report a case in which cerebrospinal fluid leakage occurred after open treatment. PATIENTS AND METHODS: The study was carried out in 2 phases. In the first part, an electron

Complete recovery and resolution of neurological symptoms following a penetrating gunshot in the left frontal lobe: a case report.

BACKGROUND: Traumatic brain injury is a major global health concern, with penetrating brain trauma representing an uncommon but highly lethal subset, especially in conflict-affected regions. Outcomes in penetrating brain trauma are strongly influenced by admission Glasgow Coma Scale (GCS), extent of structural injury, projectile characteristics, and imaging findings. Early expert assessment, rapid neuroimaging, and timely surgical intervention are critical for improving survival in selected patients although long‑term recovery often requires multidisciplinary rehabilitation. We report a rare c

Community-Acquired Pneumococcal Ventriculitis as a Complication of Otogenic Pneumococcal Meningitis: A Case Report.

Pyogenic ventriculitis is a rare and potentially fatal infection of the ventricular system. While most reported cases are nosocomial and catheter-associated, ventriculitis as a complication of community-acquired bacterial meningitis is uncommon, particularly in adults, and carries a poor prognosis. We report the case of an 82-year-old previously healthy and unvaccinated woman who presented with a one-day history of fever, confusion, and purulent left-sided otorrhea on a 2-week background of flu-like symptoms. Cranial computed tomography was consistent with otogenic meningitis complicated by pn

A comparison of prone versus sitting position for the surgical treatment of Chiari malformation type I in children.

OBJECTIVE: Posterior fossa decompression and duroplasty (PFD-D) for Chiari type I malformation (CM-I) is usually performed in the prone position. Nevertheless, few centres routinely perform it in sitting position. This study aims to compare, for the first time in the literature, the peri-operative surgical and anaesthesiologic outcomes of two similar cohorts of paediatric patients with CM-I treated with PFD-D in prone and sitting position. METHODS: Patients younger than 17 years old, who had undergone PFD-D for CM-I performed by a single surgeon between November 2013 and June 2024, were inclu

The Relationship between Risk Factors of Head Trauma with CT Scan Findings in Children with Minor Head Trauma Admitted to Hospital.

BACKGROUND: In emergency medicine for determining the intracranial injury (ICI) in children with head trauma, usually brain CT scan is performed. Since brain CT scan, especially in children, has some disadvantages, it is better to find a procedure which could help to choose only the children with real head trauma injury for brain CT scan. AIMS: The aim of this study is to find such procedure. This study was descriptive, analytic and non-interventional. METHODS: We reviewed the archived files of children with head trauma injuries referred to the emergency department of Imam Hossein Hospital w
