A diagnostic approach to popliteal fossa masses.

There is a myriad of potential mass lesions that occur in the popliteal fossa, which present as palpable masses or are found incidentally on imaging. With a thorough knowledge and understanding of the appearances and locations of these different entities, one can narrow the differential diagnoses in the majority of cases. This will eliminate unnecessary additional investigations and enable a more rapid management. We present a review of frequently encountered and less common entities using an anatomical sieve, with the aim of providing a diagnostic approach to popliteal fossa masses.

[Rheumatology emergencies in general practice].

In rheumatology there may occur emergencies especially in the field of inflammable diseases, the sudden occlusion of the central retinal artery in temporal arteritis as an example. Such incidents are rare. The general practitioner is more often confronted with not necessarily threatening, but very painful and function obstructing acute cases. In this paper four typical problems are represented which can be seen in everyday practice, sometimes misleading to wrong actions and therefore needing to be recognized in time and treated correctly: acute low back pain, periarthropathy of the shoulder jo

Radiologic case study. Pigmented villonodular synovitis.

Pigmented villonodular synovitis is a benign, locally aggressive disorder characterized by a unique clinical radiographic, and histopathologic presentation. While it is considered to be a rare disorder, a delay in diagnosis and treatment can potentially result in severe disruption of joint function due to subchondral invasion. Once the diagnosis is confirmed, treatment should consist of complete synovectomy. Recurrence is common, but malignant transformation is rare. The etiology of PVNS is still unknown, and perhaps its future discovery will assist in the definitive treatment of this disorder

Modification of beta 2-microglobulin with D-glucose or 3-deoxyglucosone inhibits A beta 2M amyloid fibril extension in vitro.

beta 2-microglobulin (beta 2M) is a major constituent of amyloid fibrils (fA beta 2M) deposited in patients with A beta 2M amyloidosis. Recently, advanced glycation end products (AGE) of beta 2M and fA beta 2M have been suggested to play an important role in the pathogenesis of A beta 2M amyloidosis. We first characterized the states of AGE modification of fA beta 2M. Western blot analysis with a monoclonal anti-AGE antibody showed that purified fA beta 2M was naturally modified with AGE. Immunohistochemical studies of amyloid-deposited tissue have revealed a patchy distribution of the AGE-mod

Arthroscopic management of degenerative meniscus tears in patients with degenerative arthritis.

Frequently, meniscal pathology accompanies degenerative changes affecting the articular surface of the knee. The attritional changes in the meniscus lead to fragmentation of the meniscus and a variety of tears, usually of the posterior horn of the medial meniscus. A prospective study of the results of arthroscopic partial meniscectomy in the presence of at least Outerbridge Grade III chondromalacia of the accompanying joint surface was performed between 1980 and 1984. Eighty-seven knees in 84 patients were studied. The mean age was 62 (29-84) years. The right knee was involved in 44 patients,

Ultrasonographic evaluation of pes anserinus tendino-bursitis in patients with type 2 diabetes mellitus.

OBJECTIVE: To assess musculoskeletal ultrasonographic (US) findings in patients with type 2 diabetes mellitus (DM) with and without pes anserinus (PA) tendinitis or bursitis syndrome; and to determine possible etiologic factors such as systemic diabetic microvascular disease complications in these patients. METHODS: The knee joints were examined with an ultrasound real-time scanner using a 10 MHz electronic linear transducer in 48 patients with type 2 DM and 25 controls. The presence of systemic diabetic microvascular disease complications was evaluated. RESULTS: On examination 23 (23.9%) kn

Application of three-dimensional rendering in joint-related ganglion cysts.

The origin of para-articular cysts is poorly understood and controversial. The relatively common, simple (extraneural) cysts are presumed to be derived from joints, although joint connections are not always established. Rarer complex cysts are thought by many to form de novo within nerves (intraneural ganglion cysts) or within vessels (adventitial cysts) (degenerative theory). We believe that these simple and complex ganglion cysts are joint-related (articular theory). Joint connections are often not readily appreciated with routine imaging or at surgery. Not identifying and/or treating joint

Lateral meniscal body extrusion is associated with MRI-defined knee structural damage progression over 4 years: Data from the osteoarthritis initiative.

PURPOSE: To investigate the association of lateral meniscal body extrusion (LMBE) with OA-related knee structural damage progression over 4 years and to obtain thresholds of LMBE in predicting knee structural damage progression. METHOD: We studied 196 subjects (67 male) with a mean age of 59.5 ± 7.9 (SD) years (range45-78 years) that were randomly selected from the Osteoarthritis Initiative. Radiological assessment was performed using the Osteoarthritis Research Society International grading system. Baseline LMBE was quantified on coronal sections of intermediate-weighted MRI sequences obtain

Case Report: Postoperative pulmonary embolism causing obstructive shock and circulatory collapse after gynecologic surgery: successful rescue with VA-ECMO.

Very-high-risk pulmonary embolism (PE) occurring within hours after major surgery presents a therapeutic dilemma because urgent reperfusion may be required at precisely the time when bleeding risk is greatest. We describe a 45-year-old woman who developed catastrophic bilateral PE approximately two hours after elective gynecologic surgery consisting of myomectomy and ovarian endometriotic cyst excision. Shortly after surgery, she experienced abrupt respiratory failure, profound hypotension, and rapidly progressive circulatory collapse. On arrival at a tertiary intensive care unit, arterial blo

All-Inside Meniscal Repair: A Historical View.

BACKGROUND: All-inside meniscal repair first became popularized in the early 2000s. Since that time, there has been a wide variety of all-inside implants on the market with rapid changes and developments in recent years. INDICATIONS: Small, peripheral, longitudinal tears are best suited for all-inside repair, but this technique may even be used for large bucket handle tears especially when hybridized with an inside-out repair. TECHNIQUE DESCRIPTION: A percutaneous release of the medial collateral ligament (MCL) with an 18-g spinal needle is frequently performed when working in the medial com

Complementary Effects of Surgery and Pexidartinib in the Management of Patients with Complex Diffuse-Tenosynovial Giant Cell Tumor.

Tenosynovial giant cell tumor (TGCT) is a rare neoplasm of the joint synovium that has a wide clinical spectrum including pain and stiffness in the affected joint, joint swelling, periarticular erosions, and cartilage loss, which can severely impact quality of life. The mainstay treatment for TGCT has been surgery involving partial or total synovectomy using arthroscopic or open techniques. However, surgical resection alone is associated with high recurrence rates, particularly in diffuse-TGCT (D-TGCT) cases. The 3 cases presented here summarize a combination approach (surgery+pexidartinib [ty

Ultrasonographic assessment of Baker's cysts after intra-articular corticosteroid injection in knee osteoarthritis.

PURPOSE: To assess sonographic changes in Baker's cysts (BCs) of patients with knee osteoarthritis after a single intra-articular corticosteroid injection. METHODS: Thirty patients with knee osteoarthritis complicated with a symptomatic BC received a single intra-articular injection of 40 mg triamcinolone acetonide. Knee pain, swelling, and range of motion were evaluated. BC area and thickness of the cyst wall were measured with sonography before and 4 weeks after local treatment. RESULTS: A significant improvement in knee pain, swelling, and range of motion after corticosteroid injection wa

[Recurrence of Baker's cysts with regard to operation procedure and intraarticular pathology].

During the period from January 1990 to December 1998 65 patients were operated upon for a Baker's cyst. 41 patients (43 operations) were subjected to a clinical and sonographical follow-up examination and interrogation. In 11 cases, the cysts were merely removed, while an open synovectomy was carried out four times (rheumatics) in addition to the removal of the cysts, and arthroscopy with exstirpation was performed in 28 cases. The total recurrence rate was 14 % (6/43). Any significant differences between the groups were not found (p = 0.291). The overall complication rate was 18.6 %. In case

Prevalence of deep-vein thrombosis of the leg in patients with acute exacerbation of chronic obstructive pulmonary disease.

Acute exacerbation of chronic obstructive pulmonary disease (COPD) is associated with dyspnea and, consequently, reduced mobility. Immobility is a recognized risk factor of deep-vein thrombosis (DVT), but few data exist regarding the prevalence of DVT in patients with acute exacerbation of COPD. Real-time B-mode ultrasonography (US) is a noninvasive screening method for the diagnosis of DVT. We therefore used US to investigate the prevalence of DVT in patients with an acute exacerbation of COPD. In a prospective cohort study, 196 patients with COPD were studied [110 males, 86 females, age: 66.

Cystic adventitial degeneration of the popliteal artery: report on 3 cases and review of the literature.

BACKGROUND: Cystic adventitial disease (CAD) is a rare, nonatherosclerotic vascular condition predominantly seen in middle-aged men with no cardiovascular risk factors. Three cases have been diagnosed and treated in our institution during the past 8 years. The purpose of this report is to provide an updated literature review of this condition with the addition of 3 new cases. METHODS: Information about 3 new cases is presented along with data obtained from articles published between 1979 and 2012 from PubMed and Embase databases. Two hundred thirty-eight articles were found, and 98 were inclu

Spontaneous resolution of cystic adventitial disease: a word of caution.

Spontaneous resolution of cystic adventitial disease has been occasionally reported in the literature. It is unclear, however, whether this resolution is permanent. In this case report, we describe recurrence of a popliteal artery cystic adventitial disease after spontaneous resolution, which was successfully treated with surgery. The underlying mechanism is proposed. Without definitive treatment, the patients with spontaneous resolution of cystic adventitial disease may need long-term follow-up, given the risk of recurrence.
