Perioperative Complications and Anesthesia Practices in Managing Patients With Quadriplegia Undergoing Surgery: A Systematic Review.

Quadriplegia is associated with a multitude of health complications affecting numerous organ systems. Complications during the perioperative periods are not uncommon in this patient population due to abnormal responses to surgical stressors. Such complications include autonomic dysreflexia, cardiac ischemia, and respiratory compromise. Currently, there is no clear consensus on the ideal technique for perioperative anesthesia management in this population. In addition, the relationship between the perioperative complications and anesthesia practices have not been explored in-depth. Therefore, w

Challenges in the diagnosis and management of inflammatory bowel disease in resource-limited settings in Asia.

Inflammatory bowel disease (IBD) is increasing in prevalence in resource-limited settings in Asia. Although the prevalence of IBD is lower in these settings than in high-income countries, the high disease burden due to large population size is projected to overtake that of high-income countries in the near future. Unique challenges exist for diagnosing and managing IBD in Asia. On one hand, the inadequate disease awareness in physicians and the general population, the scarcity of diagnostic services, the infectious mimics of IBD (specifically intestinal tuberculosis), and the widespread use of

Congenital Syphilis Epidemiology, Prevention, and Management in the United States: A 2022 Update.

Congenital syphilis (CS) has dramatically increased in the United States (US) in the past decade despite the widespread availability of penicillin. Once considered an infection on the verge of elimination, CS has re-emerged as a familiar neonatal pathogen in US hospitals. This rise in cases has prompted the evaluation of potential causes and updates in prevention and management guidelines. Following a structured narrative approach, we reviewed CS data reports, peer-reviewed research articles, and updated management guidelines from state health departments over the past two decades. Our main se

Impact of temporomandibular disorders on oral health-related quality of life: A systematic review and meta-analysis.

BACKGROUND: The management of temporomandibular disorders (TMDs) requires a comprehensive approach that considers multiple factors, including the impact of oral health-related quality of life (OHRQoL). Through this investigation we aim to assess the impact of OHRQoL played in a TMD-afflicted individual. METHODS: Using keywords relevant to our research, such as "Oral health related quality of life," "Oral hygiene," "Temporomandibular joint" and "Temporomandibular disorders," a comprehensive search across multiple online databases was carried out, yielding a total of 632 studies at the prelimin

Positive psychological resources, self-management behavior, and glycemic outcomes in adolescents with type 1 diabetes and elevated diabetes distress.

OBJECTIVE: Positive psychological constructs (e.g., benefit finding, resilience, and diabetes strengths) have been associated with greater engagement in self-management behaviors among teens with type 1 diabetes (T1D). However, for teens with T1D experiencing diabetes distress, these potentially protective associations are less understood. To explore the potential protective functions of positive psychological constructs, we investigated their associations with subjective and objective diabetes self-management behaviors and health outcomes among adolescents with diabetes distress. METHODS: Th

Female genital mutilation: Experience in a West London clinic.

In 1997, a new clinic was established at the Central Middlesex Hospital to serve the needs of a mainly Somali population who had suffered genital mutilation in childhood. Between June 1997 and January 2005, 4,125 clinic attendances were recorded. A total of 215 reversals of circumcision were carried out (FGM 3), all on a day-care basis. In the majority of cases, an intact and undamaged clitoris was found under the scar tissue. The clinic staff were able to draw attention to cultural and religious issues which proved important in the medical management of these women. The experience of this cli

Rectal prolapse in the pediatric population-a narrative review of medical and surgical management.

BACKGROUND AND OBJECTIVE: Although more frequent in the adult population, rectal prolapse is a common anorectal condition that can occur in children and adolescents. While many cases spontaneously resolve without the need for intervention, the advent of newer minimally invasive procedures and operations have provided options for pediatric patients. Here, we review the pathophysiology, etiology, presentation, diagnosis and principles of management of rectal prolapse in the pediatric population as it has evolved over the past several decades. METHODS: The literature was queried from free databa

Post-intensive care syndrome. What clinicians and researchers must know.

The COVID-19 pandemic has highlighted intensive care as a cornerstone of modern medicine. In spite of global aging and the increase of comorbidities in the general population, a large proportion of patients survive their hospitalization in the Intensive Care Unit (ICU). Nevertheless, these positive results are challenged by the higher mortality rates than other non-critically ill populations after discharge. Moreover, there is growing evidence that ICU survivors display a high rate of mental health disorders (anxiety and depression symptoms, post-traumatic stress disorders), somatic impairment

Lived experiences, perspectives, and challenges related to medication management among older people living with HIV (PLWH) in Malaysia.

Human immunodeficiency virus (HIV) is a significant global public health concern. Antiretroviral therapy (ART) has transformed HIV into a chronic and manageable disease, leading to an increased lifespan for people living with HIV (PLWH). However, the growing population of older PLWH presents unique challenges owing to the complexities of managing multiple medications for HIV and age-related conditions. This study aimed to explore the lived experiences, perspectives, and challenges related to medication management among older PLWH in Malaysia. Semi-structured interviews were conducted with 18 o

Current issues in evaluation and management of osteoporosis in Thailand.

Osteoporosis is a major public health issue in Thailand, which increased morbidity, mortality, and health-care utilization. The objective of this review is to provide current perspectives on epidemiology, evaluation and management of osteoporosis in Thailand. According to epidemiologic data, the prevalence of osteoporosis and the incidence of hip fracture were comparable to the rest of the world. However, among Thai postmenopausal women, the prevalence of asymptomatic vertebral fracture was disproportionately high. In addition to established risk factors, conditions that may affect the risk of

Integrated prevention and management of non-communicable diseases, including musculoskeletal health: a systematic policy analysis among OECD countries.

INTRODUCTION: Development and implementation of appropriate health policy is essential to address the rising global burden of non-communicable diseases (NCDs). The aim of this study was to evaluate existing health policies for integrated prevention/management of NCDs among Member States of the Organisation for Economic Co-operation and Development (OECD). We sought to describe policies' aims and strategies to achieve those aims, and evaluate extent of integration of musculoskeletal conditions as a leading cause of global morbidity. METHODS: Policies submitted by OECD Member States in response

The Effect of Social Determinants of Health in Treating Chronic Limb-Threatening Ischemia.

Social determinants of health (SDOHs) are broadly defined as nonmedical factors that impact the outcomes of one's health. SDOHs have been increasingly recognized in the literature as profound and modifiable factors on the outcomes of vascular care in peripheral artery disease (PAD) and chronic limb-threatening ischemia (CLTI) despite surgical and technological advancements. In this paper, we briefly review the SDOH and its impact on the management and outcome of patients with CLTI. We highlight the importance of understanding how SDOH impacts our patient population so the vascular community ma

Navigating adulthood with maple syrup urine disease: patient and caregiver perspectives on healthcare transition and independent living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim

Prevalance and determinants of hypertension among adults in Somalia using Somalia demographic health survey data, SDHS 2020.

BACKGROUND: Hypertension, a major global health concern, continues to affect millions of adults worldwide, leading to significant morbidity and mortality. The burden of hypertension is particularly pronounced in low- and middle-income countries, where limited healthcare resources and infrastructure pose unique challenges for effective prevention and management. Despite the pressing need for accurate data on hypertension prevalence and determinants, there is a dearth of research focusing on the Somali population. In this study, we aim to fill this critical knowledge gap by analyzing the latest

An Ounce of Prevention: The Growing Need for Preventive Neurologists.

The global burden of neurologic disorders is rising, driven by aging populations and improved survival following acute neurologic events. As a result, more individuals are living with long-term disabilities from conditions such as stroke, dementia, and other neurodegenerative diseases. Despite significant advances in neurology, there remains an urgent need for a preventive approach to mitigate these trends. Growing evidence highlights the effectiveness of preventive strategies, including lifestyle modifications and risk factor management, in preserving brain health and reducing the risk of str

Cascade of care for hypertension among apparently healthy and unhealthy individuals of 25-64 years in the Czech Republic.

BACKGROUND: Despite accessible diagnostics and treatment of hypertension, a high proportion of patients worldwide remain unaware of their diagnosis, and even more remain untreated. Several studies suggest that absence of comorbidities is a predictive factor for unawareness and consequently non-treatment of hypertension. There are only a few studies that have assessed the hypertension prevalence and management among apparently healthy individuals. OBJECTIVE: The aim of the study was to assess and compare hypertension prevalence, awareness, treatment and control among apparently healthy individ
