Porphyria cutanea tarda, a metabolic disorder of heme biosynthesis, is characterized by cutaneous hyperpigmentation, facial hypertrichosis, dark urine, and a distinctive pattern of excess porphyrin production. Hepatic uroporphyrinogen decarboxylase activity is markedly reduced in patients with this disorder. Although porphyria cutanea tarda may be familial, it is more often sporadic in occurrence, and has been associated with excess alcohol ingestion, estrogen administration, iron overload, and several environmental hepatotoxins. It has also been associated on occasion with malignancy. We repo

The term "porphyria cutanea tarda" originally described the dermatological manifestations of various chronic porphyrias. Its usage now is usually restricted to disorders associated with a deficiency of uroporphyrinogen decarboxylase (UROD), for which the term "UROD-deficiency" may be more appropriate. Four etiologic agents have been implicated in this condition: alcohol, oral estrogens, halogenated aromatic hydrocarbons, and iron. An inherited deficiency of UROD is also recognized, with increased susceptibility to these agents. Certain halogenated aromatic hydrocarbons can cause UROD-deficienc…

In contrast to Porphyria cutanea tarda (PCT), which is characterized by constant hepatic involvement, the liver seems to be unaltered in Porphyria variegata (PV). However, the authors have found microscopic alterations in the liver of 2 patients with PV (mild inflammation and fibrosis of the portal tracts and the presence of iron deposits in one of the cases). Although these changes could be attributed to other causes, the hypothesis of possible liver involvement in PV is not excluded.

Photobiological tests were carried out on a 32-year-old man who suffered from porphyria cutanea tarda (PCT). The patient developed an immediate type of skin reaction with erythema and whealing following monochromatic irradiation at 400 nm, but did not have any abnormal immediate skin reaction after exposure to natural sunlight. Pre- or simultaneous irradiation with visible light, wavelength greater than 650 nm, suppressed the development of urticaria induced by 400 nm monochromatic radiation. On the basis of these findings and our previous observation of an inhibitory spectrum in two cases of

Serum ferritin was measured in 9 patients with overt porphyria cutanea tarda. In 6, the level was higher than normal. We confirm that moderate, but not gross iron overload is a feature of such patients.

The authors report 5 cases of porphyria cutanea tarda (PCT) occurring in carriers of the beta-thalassemia trait. This hematologic condition may be responsible for the development of liver damage resulting in siderosis, higher susceptibility to hepatitis B virus infection, and the earlier appearance of clinical features of PCT. Consequently, the authors propose chelation therapy with long-term subcutaneous desferioxamine infusions.
