Healthcare access and utilization by patients infected with human immunodeficiency virus: does gender matter?

OBJECTIVE: To examine access to and utilization of primary healthcare services with respect to gender. Greater family and child-rearing responsibilities are possible barriers to healthcare access and utilization for women with HIV infection. METHODS: This study was part of a prospective, randomized, controlled trial evaluating primary care for HIV-infected patients at Duke University Medical Center (DUMC), a tertiary care medical center. Subjects were 214 HIV-infected, uninsured or publicly insured participants. Ambulatory care visits, emergency room utilization, hospitalization rates, length

Differences in the acceptability of non-abstinence goals by type of drug among American substance abuse clinicians.

To assess whether acceptability of non-abstinence outcome goals varied depending on the specific drug a client consumes (alcohol, cannabis, amphetamine, heroin, cocaine, MDMA/ecstasy, polydrug), severity of diagnosis (DSM-IV Abuse vs. Dependence), and finality of outcome goal (intermediate vs. final), we recruited 432 clinicians to complete a web-based questionnaire. More respondents rated non-abstinence acceptable as an intermediate goal for clients diagnosed with alcohol abuse (44%) or cannabis abuse (43%) than for clients diagnosed as abusing the other listed drugs (23 to 31%). Similarly, l

Comparison of major adverse event rates after elective endovascular aneurysm repair in New England using a novel measure of complication severity.

OBJECTIVE: Major adverse event (MAE) rates are used as an outcome measure after surgical procedures. Although MAE rates summarize the occurrences of adverse events, they do not reflect differences in severity of these events. We propose that a measure of complication severity could provide a more accurate assessment about the quality of care. We aimed to analyze and to describe the regional variation in elective endovascular aneurysm repair (EVAR) MAE rates across centers in the Vascular Study Group of New England and to create an index for describing complication severity. METHODS: Patients

Measuring uptake of the 2024 national standards for survivorship care: Insights from National Cancer Institute funded pilot projects.

PURPOSE: Measuring alignment with clinical program standards is essential for strengthening cancer survivorship care yet remains challenging due to the range and duration of services involved. The 2024 National Standards for Survivorship Care provide aspirational recommendations across three domains-policy, processes, and evaluation/assessment-for health systems developing survivorship care. Following their release, the National Cancer Institute (NCI) funded 18 supplements to understand alignment of existing services and programs with the standards and identify implementation barriers and faci

Does hospital performance on process measures directly measure high quality care or is it a marker of unmeasured care?

OBJECTIVE: Quality measures may be associated with improved outcomes for two reasons. First, measured activities may directly improve care. Second, success on these measures may be a marker for other unmeasured aspects of high quality care. Our objective is to test the contribution of both possible effects. DATA SOURCES: 2004 Medicare data on hospital performance from Hospital Compare and risk-adjusted mortality rates from Medicare Part A claims. STUDY DESIGN: We studied 3,657 acute care U.S. hospitals and compared observed differences in condition-specific hospital mortality rates based on

The impact of a community mental health initiative on outcomes for offenders with a serious mental disorder.

BACKGROUND: The Community Mental Health Initiative (CMHI) is mandated to assist offenders with serious mental disorders in their transition from institutions to the community, but this incorporates different styles of service. An important unanswered question is whether these are equivalent. AIMS/HYPOTHESES: Our aim was to compare outcomes for different intervention styles within the CMHI, a programme for serious offenders in prison who also have at least one major mental disorder. Our specific research questions were as follows: do outcomes differ according to whether offenders with mental h

Modifying the process of treatment to meet the threat of AIDS.

Treatment for drug users is unlikely to receive sufficient resources to expand to the level of treatment need. Moreover, there is indication that a substantial minority of injection drug users have never initiated needed treatment in spite of long histories of injecting drugs. Given the potential for that population to contract and spread AIDS, it is important that we explore street-based treatment alternatives derived from findings regarding the efficacy of outreach/intervention programs. Those programs were found both to reduce drug-taking behaviors of injecting drug users and to prepare tho

Managing obesity in children: a clinical practice guideline.

BACKGROUND: Obesity is a complex, chronic, stigmatized disease whereby abnormal or excess body fat may impair health or increase the risk of medical complications, and can reduce quality of life and shorten lifespan in children and families. We developed this guideline to provide evidence-based recommendations on options for managing pediatric obesity that support shared decision-making among children living with obesity, their families, and their health care providers. METHODS: We followed the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. We used the Gui

Global assessment of atherosclerotic cardiovascular disease quality of care index in adults aged ≥60 years.

AIMS: Atherosclerotic cardiovascular disease (ASCVD) remains the primary global cause of mortality and disability. Older adults (≥60 years) represent a vulnerable population with elevated cardiovascular risk, establishing ASCVD quality of care (QoC) as a critical global health priority. This study aimed to investigate temporal trends in the global ASCVD QoC for adults aged ≥60 years. METHODS AND RESULTS: This analysis utilized Global Burden of Disease 2021 data across 204 countries and territories. The age-standardized quality of care index (QCI) assessed ASCVD management, with concentration

Understanding variability in hospital-specific costs of coronary artery bypass grafting represents an opportunity for standardizing care and improving resource use.

BACKGROUND: This study was undertaken to examine interhospital variability in inpatient costs of coronary artery bypass grafting (CABG). METHODS: The Nationwide Inpatient Sample was used to identify isolated CABGs performed between 2005 and 2008 in the United States. Charges for inpatient care were supplied by the data set, and hospital charge-to-cost ratios were used to derive inpatient costs for each patient and aggregated at the hospital level. Mixed-effect linear regression models were created to evaluate variability in costs between hospitals adjusting for 34 patient, operative, complica

[Time trends of process and impact indicators in Italian breast screening programmes (2000-2010)].

Since its establishment in 1990, one of the main tasks of the Italian group for breast cancer screening (GISMa) is the systematic data collection on the activity of the organised breast cancer screening programmes implemented in Italy. Data are collected in an aggregated way and gathered through a standardised form to calculate process and impact parameters. Data analysis from 2000-2010 shows that crude attendance rate reached the acceptable 50% standard, presenting a higher level of participation in Northern and Central Italy compared to Southern Italy/Islands, where attendance rates are stil

Why we fail: mechanisms and co-factors of unsuccessful thrombectomy in acute ischemic stroke.

PURPOSE: Mechanical thrombectomy (MT) is an effective treatment for patients suffering from acute ischemic stroke. However, recanalization fails in about 16.5% of interventions. We report our experience with unsuccessful MT and analyze technical reasons plus patient-related parameters for failure. METHODS: Five hundred ninety-six patients with acute ischemic stroke in the anterior circulation and intention to perform MT with an aspiration catheter and/or stent retriever were analyzed. Failure was defined as 0, 1, or 2a on the mTICI scale. Patients with failing MT were analyzed for interventio

Profiling Hospital Performance Based on Mortality After Transcatheter Aortic Valve Replacement in Ontario, Canada.

BACKGROUND: Public reporting of hospital-level outcomes is increasingly common as a means to target quality improvement strategies to ensure the delivery of optimal care. Despite the rapid dissemination of transcatheter aortic valve replacement (TAVR), there is a paucity of reliable case-mix adjustment models for hospital profiling in TAVR. Our objective was to develop and evaluate different models for calculating risk-standardized all-cause mortality rates (RSMRs) post-TAVR. METHODS AND RESULTS: In this population-based study in Ontario, Canada, we identified all patients who underwent a TAV

A process for developing community consensus regarding the diagnosis and management of attention-deficit/hyperactivity disorder.

There remain large discrepancies between pediatricians' practice patterns and the American Academy of Pediatrics (AAP) guidelines for the assessment and treatment of children with attention-deficit/hyperactivity disorder (ADHD). Several studies raise additional concerns about access to ADHD treatment for girls, blacks, and poorer individuals. Barriers may occur at multiple levels, including identification and referral by school personnel, parents' help-seeking behavior, diagnosis by the medical provider, treatment decisions, and acceptance of treatment. Such findings confirm the importance of
