Feasibility and Implementation of a Digital Health Intervention Electronic Patient-Reported Outcomes-Based Platform for Telemonitoring Patients With Breast Cancer Undergoing Chemotherapy.

PURPOSE: Breast cancer (BC) is a leading cause of morbidity and mortality among women. Symptoms and treatment-related side effects often go undetected during routine follow-ups. Digital health interventions offer promising tools for real-time monitoring and personalized care. We aimed to implement and evaluate the feasibility of a mobile health electronic Patient-Reported Outcomes (ePRO)-based platform for telemonitoring patients with BC undergoing (neo)adjuvant chemotherapy. METHODS: This prospective observational study enrolled patients with BC (TNM stages I to III) initiating chemotherapy

Palliative care & chronic liver disease: barriers to care, health disparities & the role of health literacy.

Cirrhosis continues to excel as one of the leading causes of death worldwide, characterized by an often tumultuous and unpredictable disease course with progressive decline accompanied by severe symptom burden, impacted quality of life (QOL), high healthcare expenditures, and caregiver burn out. This patient population often has concurrent concerns related to their underlying disease such as substance use disorders, mental health, or socioeconomic factors that may complicate their management. Palliative care (PC) and a multidisciplinary team approach can make substantial impacts through additi

Effect of shunt surgery on idiopathic normal pressure hydrocephalus with incomplete Gerstmann syndrome: A CARE-compliant case report.

RATIONALE: Idiopathic normal pressure hydrocephalus (iNPH) occasionally co-exists with neurodegenerative disease, but its concurrence with Gerstmann syndrome (GS) has not been reported, leaving the reversibility of GS-like deficits after cerebrospinal fluid diversion unknown. PATIENT CONCERNS: A 77-year-old woman experienced a 1-year progressive decline in memory, object naming, and spatial orientation, eventually requiring institutional care. DIAGNOSES: Neurological examination revealed severe cognitive impairment (mini-mental state examination [MMSE] 4/30) with acalculia, agraphia, finger

Pancreatoblastoma: an unusual diagnosis in an adult patient.

The authors describe a 31-year-old man admitted due to progressive weight loss, diarrhea and massive hepatomegaly. Laboratory data showed anemia (haemoglobin 11.7 g/dl), abnormal liver tests (total bilirubin 1.4 g/dl, aspartate aminotransferase 70 U/l, alanine aminotransferase 37 U/l and alkaline phosphatase 520 U/l). Abdominal ultrasound (US) displayed a large heterogeneous liver with a segment IV 25 mm nodule. Magnetic resonance revealed a 4 cm pancreatic tail mass and several liver nodules consistent with metastasis. The patient underwent an endoscopic ultrasound (EUS) with fine needle aspi

Top Ten Tips Palliative Care Clinicians Should Know About Caring for People Receiving Critical Care for Cardiovascular Disease.

Cardiovascular disease (CVD) remains the leading cause of death worldwide and is a major driver of critical illness, resulting in admission to cardiac intensive care units (CICUs). Patients with acute, life-threatening CVD often experience episodes of acute decompensation characterized by high symptom burden that may portend progressive loss of function, significantly impair quality of life (QOL), and involve high-intensity, technologically advanced therapies to sustain life. Despite these advances, up to 30% of patients die during CICU hospitalizations, and those who survive may subsequently

When Surgical Intervention Becomes a Risk: Severe Drug-Induced Gingival Overgrowth and Clinical Decision-Making in Alpers-Huttenlocher Syndrome.

INTRODUCTION: Alpers-Huttenlocher syndrome (AHS) is an ultra-rare, autosomal recessive mitochondrial encephalopathy caused by pathogenic POLG variants, characterized by refractory epilepsy, hepatic dysfunction, and progressive neurodegeneration. Drug-induced gingival overgrowth (DIGO) is a recognized complication of anticonvulsant and immunosuppressive therapy, particularly in medically complex patients. This report describes severe DIGO in a child with genetically confirmed AHS and explores the implications for multidisciplinary dental management. METHODS: A CARE-compliant case analysis was

Necrotizing fasciitis of the head and neck: 34 cases of a single institution experience.

We analyzed 34 patients with necrotizing fasciitis (NF) of the head and neck. According to the clinical presentation, we classified NF into the cervical and cranial type. Infection can spread along the superficial lamina or along the pretracheal lamina of the deep cervical fascia. In the first type, skin necrosis is the main sequelae, while in the second case, descending infection and mediastinitis are the main complications. Of the 34 patients, in 17 the infection progressed in the mediastinum. Due to aggressive surgical therapy and postoperative intensive care, the mortality rate remained lo

Predicting Postoperative Length of Stay in Patients Undergoing Laparoscopic Right Hemicolectomy for Colon Cancer: A Machine Learning Approach Using SICE (Società Italiana di Chirurgia Endoscopica) CoDIG Data.

The evolution of laparoscopic right hemicolectomy, particularly with complete mesocolic excision (CME) and central vascular ligation (CVL), represents a significant advancement in colon cancer surgery. The CoDIG 1 and CoDIG 2 studies highlighted Italy's progressive approach, providing useful findings for optimizing patient outcomes and procedural efficiency. Within this context, accurately predicting postoperative length of stay (LoS) is crucial for improving resource allocation and patient care, yet its determination through machine learning techniques (MLTs) remains underexplored. This study

Cervical Spinal Cord Injury During Prone Position Ventilation in the COVID-19 Pandemic.

The prone positioning of patients experiencing acute respiratory distress syndrome (ARDS) due to coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has been proven effective in optimizing oxygenation and lung function. However, such patients may be at risk of developing complications due to the prolonged prone position in intensive critical care. A 45-year-old COVID-19 female, not known with cervical spine disease, presented with progressive severe COVID-19-related hypoxemia that required intensive care unit admission for pulmonary care

Continuity of Care and Risks of Hospitalization and Mortality in COPD: A Nationwide Cohort Study.

PURPOSE: Chronic obstructive pulmonary disease (COPD) is characterized by unpredictable patterns of exacerbations, making continuity of care (CoC) a critical component in disease management. CoC influences COPD exacerbation-related hospitalization and mortality. Therefore, we assessed the impact of CoC levels on subsequent hospital admissions and all-cause mortality among individuals with COPD. PATIENTS AND METHODS: This retrospective nationwide cohort study utilized National Health Insurance Service-Senior cohort data (2002-2019) and included 29,316 patients newly diagnosed with COPD. The Cost of Crohn's Disease: Varied Health Care Expenditure Patterns Across Distinct Disease Trajectories.

BACKGROUND: Patients with Crohn's disease (CD) encompass a heterogeneous disease spectrum, with variable health care utilization and expenditure patterns. Lémann Index (LI) is a metric that quantifies cumulative bowel damage and has shown utility in delineating distinct disease phenotypes. We aimed to characterize the financial burden from all medical care in CD cohort in relation to the variations in LI-based disease phenotypes. METHODS: CD patients with 5-year (y) follow-up from a prospective registry were included. LI was calculated from first (LI1) and last (LI2) clinical encounters. Chan

[Charles Bonnet syndrome. A 45-case series].

INTRODUCTION: Charles Bonnet Syndrome (CBS) is a condition characterised by the presence of visual hallucinations, mainly complex, in patients with significant vision loss and without cognitive impairment. The rise in CBS cases is due to an increased life expectancy and to the development of age-related pathologies such as age-related macular degeneration (AMD). PATIENTS AND METHODS: We herein analyse the main characteristics present in 45 patients diagnosed with CBS at the Neuro-ophthalmology Unit in Hospital Clinico San Carlos. The patients were referred from the macular pathology, glaucoma
