Does transrectal ultrasound guided eight-core prostate biopsy improve cancer detection rates in patients with prostate-specific antigen levels of 4.1-10 ng/mL?

BACKGROUND: To investigate retrospectively whether the eight-core biopsy method improves the prostate cancer detection rate when compared with the standard sextant biopsy method in patients with prostate specific antigen (PSA) levels of 4.1-10 ng/mL. MATERIAL AND METHODS: Of 437 patients whose PSA levels ranged from 4.1 to 10 ng/mL, 237 underwent a transrectal ultrasound guided sextant biopsy (sextant group), and 200 underwent an eight-core biopsy (eight-core group). Eight core samples were obtained from each of the far lateral regions in addition to the standard sextant biopsy cores. None of

Serum proteomic profiling can discriminate prostate cancer from benign prostates in men with total prostate specific antigen levels between 2.5 and 15.0 ng/ml.

PURPOSE: Artificial intelligence based pattern recognition algorithms have been developed and successfully used to analyze complex serum proteomic data streams generated by surface enhanced, laser desorption ionization time-of-flight mass spectroscopy. In the current study we used a high performance, hybrid quadrupole time-of-flight mass spectrometer to generate discriminatory serum proteomic profiles to determine if this technology could be used to determine the need for prostate biopsy in men with elevated prostate specific antigen (PSA). MATERIALS AND METHODS: Serum samples were collected

Development of gene therapy using prostate-specific membrane antigen promoter/enhancer with Cre Recombinase/LoxP system for prostate cancer cells under androgen ablation condition.

To enhance the efficacy of suicide gene therapy for prostate cancer under androgen deprivation, we designed a promoter system that consists of the prostate-specific membrane antigen (PSMA) promoter / enhancer (PEPM) and Cre-loxP DNA recombination system. We constructed two kinds of plasmids. One plasmid contains a Cre recombinase (Cre) under the control of PEPM and the other expresses CMV-lox-luciferase / herpes simplex virus thymidine kinase (TK). In PSMA-positive LNCaP cells, the promoter activity of the PEPM-Cre plus CMV-lox-luciferase demonstrated 800-fold greater activity compared with th

Prostate-Specific Membrane Antigen (PSMA) Uptake in the Scrotum and Epididymis on PET-CT: When is it Pathological?

Prostate cancer is the most common solid organ tumor in men and has been reported to metastasize to unusual sites such as the epididymis. The clinical standard for detecting recurrent disease is through positive emission tomography/computed tomography with the radiotracer 18F-DCFPyL binding prostate-specific membrane antigen (PSMA) expressed by cancerous cells. Although PSMA can also be expressed physiologically, metastases are more likely to be intensely PSMA expressing and in a typical distribution depending on the extent of disease burden in the individual patient. A MEDLINE search revealed

Serum prostate-specific antigen concentrations are not useful for monitoring the treatment of hirsutism with oral contraceptive pills.

Prostate-specific antigen (PSA) is produced in several female tissues, under the regulation of steroid hormones. Serum PSA levels are increased in women with hirsutism, and they correlate with serum androgen levels. Therefore, as a marker of androgen excess, measurement of serum PSA may play a role in monitoring the treatment of hirsutism with contraceptive pills. Sixteen hirsute patients were included in the study. Clinical and biochemical variables, including serum PSA (using an ultrasensitive chemiluminescent immunoassay), total testosterone, sex hormone-binding globulin, androstenedione, d…

Combining Serum PVT1 Exon 4A and Exon 9 with Serum Prostate-Specific Antigen Shows Potential for Improving Identification and Risk Stratification of Prostate Cancer.

PURPOSE: Prostate-specific antigen (PSA), a common screening tool for prostate cancer (PCa), has a high false-positive rate and is largely suboptimal. PSA is also weakly validated in men of African ancestry (moAA), who disproportionately bear the disease burden, necessitating investigation into its widespread use. Previously, we found that plasmacytoma variant translocation 1 (PVT1) exons 4A, 4B, and 9 are overexpressed in PCa tissues. Here, we report on the first serum-based evaluation of these exons to determine their utility in improving detection and risk stratification of PCa. PATIENTS A…

Retrospective analysis of micronutrient status and prostate health: linking serum zinc, selenium, and vitamin D levels to prostate-specific antigen and urinary tract function.

BACKGROUND: Micronutrients such as zinc, selenium, and vitamin D play a crucial role in prostate physiology. Deficiencies in these nutrients may be associated with prostate-specific antigen (PSA) levels and with impaired urinary tract parameters, including the International Prostate Symptom Score (IPSS), maximum urinary flow rate (Qmax), and post-void residual (PVR) volume. This retrospective study evaluated the association between serum micronutrient status and prostate health indicators in adult men. METHODS: Medical records of 400 men aged ≥45 years who underwent prostate evaluation betwee…

Radiomics for Differentiating Prostate-Specific Membrane Antigen (PSMA)-Positive and PSMA-Negative Lymph Nodes in Prostate Cancer: An Exploratory 68Ga-PSMA PET-CT Study.

Introduction Accurate lymph node assessment in prostate cancer remains clinically relevant for staging and treatment planning. Although 68Ga-prostate-specific membrane antigen (PSMA) PET-CT has improved nodal evaluation, quantitative imaging biomarkers may provide additional information beyond visual assessment. This exploratory study aimed to characterize CT-derived radiomic features extracted from PSMA-positive and PSMA-negative lymph nodes using 68Ga-PSMA PET/CT positivity as the imaging reference standard. Materials and methods This retrospective, cross-sectional study included 41 patients

Prostate-Specific Antigen Density to Select Magnetic Resonance Imaging-Targeted vs Combined Biopsy for Prostate Cancer Diagnosis: A Secondary Analysis of the Trio Study.

PURPOSE: The addition of systematic prostate biopsy enhances the detection of clinically significant prostate cancer compared with MRI-targeted biopsy alone. However, there is growing interest in using only MRI-targeted biopsy. We sought to evaluate PSA density (PSAD) as an adjunctive predictor of clinically significant prostate cancer detection in men undergoing combined biopsy and as a potential metric to stratify which patients may reasonably avoid systematic biopsies in favor of MRI-targeted biopsy only. MATERIALS AND METHODS: Men with elevated PSA and/or abnormal digital rectal examinati

Prostate-specific Membrane Antigen Positron Emission Tomography Before Reaching the Phoenix Criteria for Biochemical Recurrence of Prostate Cancer After Radiotherapy: Earlier Detection of Recurrences.

BACKGROUND AND OBJECTIVE: Biochemical recurrence (BCR) of prostate cancer (PCa) after curative radiotherapy (RT) is defined according to the Phoenix criteria, which is a prostate-specific antigen (PSA) rise of ≥2.0 ng/ml above the PSA nadir. Prostate-specific membrane antigen (PSMA)-based positron emission tomography/computed tomography (PET/CT) can identify PCa recurrences at very low PSA values. Our aim was to investigate the detection rate and extent of PCa recurrences using PSMA PET/CT after curative RT among patients with a PSA rise of ≥2.0 ng/ml above the nadir (Phoenix positive, Ph+) an…

Early Detection of Secondary Bladder Urothelial Carcinoma and Disseminated Bone Metastases with Normal Prostate-Specific Antigen Level after Pelvic Salvage Radiotherapy in Prostate Cancer.

This report describes the case of a 65-year-old man who presented with gross hematuria and a history of pelvic salvage radiotherapy for prostate cancer. Cystoscopy and transurethral resection of the bladder revealed urothelial carcinoma. Subsequently, disseminated bone metastases were detected with normal prostate-specific antigen (PSA) levels, and palliative radiotherapy and systemic chemotherapy were administered. Because gross hematuria can appear in both acute/chronic cystitis and bladder cancer in patients who have undergone pelvic radiotherapy for prostate cancer, close follow-up along w…

Prevalence and characteristics of prostate cancer among participants of a community-based screening in Nigeria using serum prostate specific antigen and digital rectal examination.

INTRODUCTION: Prostate cancer (CaP) is the most commonly diagnosed cancer among Nigerian men but CaP screening is not a common practice. The true burden of the disease in Nigeria is not known. The study was aimed at studying the community burden of CaP in Lagos. METHODS: During a community-based prostate cancer awareness program in 13 local government areas of Lagos, men aged >40 years had serum total PSA (tPSA) test and digital rectal examination (DRE). Those with abnormal DRE or tPSA >95th percentile of the cohort or both were selected for prostate biopsy (TRPB). RESULTS: 4172 men were scr

Preoperative serum prostate-specific antigen (PSA) below 10 microg/l predicts neither the presence of prostate cancer nor the rate of postoperative PSA failure.

Recent information on the relationship of serum prostate-specific antigen (PSA) to prostate cancer and new reports on death rates in men warrant a reassessment of how we diagnose and treat prostate cancer. We now know for the first time that the annual death rate from prostate cancer in men > or =65 years of age is only 226 per 100 000 men. At least 40 000 of 100 000 men over age 65 (40%) have invasive prostate cancer as judged by examination of prostates in 3- to 4-mm step-sections. Thus, only 1 of every 177 men 65 years of age or older (226 in 40 000) with invasive prostate cancer dies annua…

Development and validation of prostate-specific membrane antigen-adjusted Prostate Imaging Reporting and Data System: A new approach to enhancing the accuracy of diagnosing treatment-naïve primary prostate cancer.

OBJECTIVE: The diagnostic efficacy of hybrid positron emission tomography (PET)/MRI in detecting primary prostate cancer (PCa) is limited due to the lack of a quantitative integrated analysis. A new Prostate Imaging Reporting and Data System (PI-RADS) incorporating prostate-specific membrane antigen (PSMA) PET for PET/MRI has been introduced to improve primary detection of PCa. METHODS: One hundred and twenty consecutive patients with high clinical suspicion of PCa were enrolled in this prospective study to assess the diagnostic performance of PSMA-adjusted PI-RADS (PAPI-RADS) using 68Ga-PSMA
