Prosthetic joint infection with Pasturella multocida following cat scratch: a report of 2 cases.

Infection is a known cause of failure in a total joint arthroplasty. Secondary or delayed infections are caused by a wider variety of pathogens, including Gram-negative organisms. Pasturella multocida is a Gram-negative bacillus that forms part of the normal nasopharyngeal and gastrointestinal flora of cats and many other animals. Nontrauma-associated infections also have been reported, but these are more often confined to animal handlers. We report 2 patients who had cat scratch and who developed infection of their total hip arthroplasties with P multocida. Both patients were immunocompromise

Risk of endocarditis among patients with coagulase-negative Staphylococcus bacteremia.

Coagulase-negative staphylococci (CoNS) are currently considered typical microorganisms causing infective endocarditis (IE) in patients with prosthetic valves. The objective was to determine variables associated with IE in patients with CoNS bacteremia. We performed an analysis of the clinical characteristics of patients with CoNS bacteremia admitted to a university hospital in Madrid (Spain) from 2021 to December 2022 according to the occurrence of IE. This study is an evaluation of a bacteremia registry. During the study period, 106 patients with CoNS bacteremia were detected. In 85 patients

Preliminary Results of a Prospective Study on Methods of Cranial Reconstruction.

PURPOSE: Given its biological and anatomic features, autologous bone is the first choice for cranioplasty after bone decompression. When autologous bone is not available or must be replaced, surgeons can choose among various materials to create an alloplastic cranioplasty. The Italian Society for Neurosurgery promoted a prospective study conducted at 4 Italian neurosurgical units to compare different methods of cranioplasty and to assess the clinical results and incidence of complications. MATERIALS AND METHODS: Patients older than 14 years who underwent repositioning of autologous bone or 3-

Animal models of orthopaedic infections. A review of rabbit models used to induce long bone bacterial infections.

The development of infections is one of the main complications in orthopaedics, especially in the presence of implants for the osteosynthesis of compound fractures and joint prosthesis. Indeed, foreign materials and implants act as substrates for the adhesion and proliferation of bacterial strains able to produce biofilm, causing peri-implant osteomyelitis. The eradication of biofilm remains a great challenge for the host immune system, as well as for medical and surgical approaches, thus imposing the need for new prophylactic and/or therapeutic strategies in which animal models have an essent

Successful long-term antibiotic suppressive therapy in a case of prosthetic valve endocarditis and a case of extensive aortic and subclavian graft infection.

INTRODUCTION: The indication for surgical valve replacement in cases of infective endocarditis is well defined in current guidelines. However, some patients are not fit or willing to undergo major surgical procedures. Interestingly, to the best of our knowledge, there is scarce information in the literature on how to deal with such cases and what might be the outcome. CASE REPORT: We present two complicated cases of prosthetic infective endocarditis with definite indication for replacement of involved foreign material, who were treated successfully with long-term suppressive antibiotic therap

Is Elective Soft Tissue Hand Surgery Associated with Periprosthetic Joint Infection after Total Joint Arthroplasty?

BACKGROUND: Although current guidelines do not recommend the routine use of surgical antibiotic prophylaxis to reduce the risk of surgical site infection following clean, soft tissue hand surgery, antibiotics are nevertheless often used in patients with an existing joint prosthesis to prevent periprosthetic joint infection (PJI), despite little data to support this practice. QUESTIONS/PURPOSES: (1) Is clean, soft tissue hand surgery after THA or TKA associated with PJI risk? (2) Does surgical antibiotic prophylaxis before hand surgery decrease PJI risk in patients with recent THA or TKA? MET

[Allograft reconstruction in revision elbow arthroplasty].

BACKGROUND: There is limited knowledge on composite allograft reconstructions in revision elbow arthroplasty. A major reason for the restricted use of freshly frozen allografts is the divergent legal requirements for allograft procurement in European countries. RESULTS: The opportunities and limitations of this complex revision surgery of the elbow are outlined based on our own restricted experiences, as well as on current literature. Our experience is in accordance with that of other institutions. The results are heterogeneous and range from satisfying to poor. The main problems are the incr

[One-stage aseptic revision of loosened metatarsophalangeal prosthesis].

The high loosening rate of the Moje prosthesis in the treatment of hallux rigidus caused disappointing medium term results. Arthrodesis using an iliac crest bone graft is the standard salvage procedure. We present short-term results after one-stage revision of the metatarsophalangeal joint using the TOEFIT-PLUS™ prosthesis for implant revision. Clinical and radiological examinations were performed on three metatarsophalangeal joints after explantation of the Moje prosthesis and one-stage revision using the TOEFIT-PLUS™ prosthesis for implant revision. The AOFAS score and VAS were checked preop

[Successful treatment of aortic prosthetic valve endocarditis and aortic root abscess by Bentall procedure].

A 53-year-old woman had undergone aortic valve replacement in 1990. Three years later, aortic prosthetic valve endocarditis and aortic root abscess had been noted. Debridement of all apparently infected tissue created left ventricular-aortic discontinuity, but the orifice of the coronary arteries were intact. We decided to reconstruct the left ventricular outflow tract and aortic root by Bentall procedure. Composite graft was made with 26 mm gelseal tube graft and a 23 mm SJM prosthetic valve, and the coronary ostia were sutured into the side of the graft. The patient's recovery was uneventful

Prevention of voice prosthesis biofilms: current scenario and future trends in prolonging prosthesis lifetime.

Voice rehabilitation after a total laryngectomy is an important requisite for patients' rehabilitation. Oesophageal speech using tracheo-oesophageal-valved prostheses is now considered the state-of-art in postlaryngectomy voice rehabilitation. One of the major drawbacks of voice prostheses is their limited device lifetime. This is due to the deterioration of the silicone rubber material by different bacterial and yeast species, which are organised in the form of a biofilm resulting in internal leakage, increased airflow resistance, impeding speech, respiration and swallowing. The use of antimi

Characteristics and outcomes of patients with infective native aortic aneurysm or vascular graft and endograft infection.

BACKGROUND: Infections of the aorta and its branches can occur either in native tissue, known as infective native aortic aneurysms (INAAs), or following vascular surgery involving native or prosthetic grafts, referred to as vascular graft or endograft infections (VGEIs). OBJECTIVES: First, this study aimed to assess the all-cause on-year mortality of patients with INAAs and VGEIs. Second, this study described patients' characteristics and prognostic factors. METHODS: This observational study included patients hospitalised for INAA or VGEI, between 2014 and 2022, at Nancy University Hospital

Pacemaker endocarditis in patients with prosthetic valve replacements: case trilogy and literature review.

Infective endocarditis is not uncommon in patients with both a permanent pacemaker system and a prosthetic valve. No guidelines exist to aid management. The recommendations for pacemaker infective endocarditis alone are limited and contradictory. We present a case trilogy and literature review that highlights these shortcomings and the challenges facing physicians. Complete extraction of the infected pacemaker system is essential. The timing of extraction, duration of antibiotic therapy, and timing of reimplantation are all controversial. The presence of a concomitant prosthetic valve exacerba

In situ replacement of infected vascular prostheses with rifampin-soaked vascular grafts: early results.

Based on the excellent results of experimental studies with antibiotic-bonded vascular prostheses for prevention of graft infection, gelatin-sealed grafts soaked with rifampin were implanted in situ in five patients with vascular infection. All patients were at risk for limb loss or death and could not be treated by standard techniques such as graft excision and extra-anatomic bypass. In one patient an infected aortic stump aneurysm with involvement of both renal and visceral arteries was found. He was treated by implantation of a bifurcation rifampin-soaked graft between the subdiaphragmal ao

Cardiac valve prostheses at autopsy.

We describe the pathologic findings at autopsy in 96 persons who had undergone prosthetic heart valve implantation. The most common indication for implantation was rheumatic heart disease (58%), followed by infective endocarditis (22%). Cardiac failure led to death in two thirds of the cases, most often from infective endocarditis (18%) and congestive heart failure (13%). Failure of the prosthetic valve caused death in eight cases. There were 13 deaths intraoperatively or in the immediate postoperative period. Patients with mechanical prostheses survived longer than did those with bioprosthese

Infection of Transcatheter Valvular Devices.

PURPOSE OF REVIEW: This review explores the epidemiology, clinical traits, and diagnosis of Transcatheter Aortic Valve Replacement-Associated Infective Endocarditis (TAVR-IE) and mitral transcatheter edge-to-edge repair infective endocarditis (TEER-IE), focusing on a multimodal imaging approach. It addresses the rising prevalence of TAVR and TEER, emphasizing the need to understand long-term complications and clinical consequences, which poses significant challenges despite advancements in valve technology. RECENT FINDINGS: Studies report a variable incidence of TAVR-IE and TEER-IE influenced

Retained Functional Antibiotic Hip Spacers Have High Rates of Stem Loosening, Subsidence, and Reoperation.

BACKGROUND: Functional antibiotic hip spacers for treatment of periprosthetic joint infection may be retained in patients deemed too sick for reimplantation, or who elect to forego additional surgery. Our aim was to characterize reoperations, modes of failure, and function outcomes in patients who have retained hip spacers. METHODS: We performed a retrospective review of 43 hips that underwent placement of an articulating hip spacer between January 1, 2014 and November 30, 2021. There were 28 hips that completed 2-stage exchange (TS group) and were reimplanted at mean 4 months (range, 2 to 10

Salvage of Prosthetic Vascular Graft Infection in the Distal Thigh Using Free Latissimus Dorsi Musculocutaneous Flap: A Case Report.

BACKGROUND: Prosthetic vascular graft infection (PVGI) in the distal thigh is a rare wound; thus, little is known about which muscle flaps are the most useful and cause less lower extremity morbidity in such cases. Moreover, very few reliable muscle flaps are available around the distal thigh. CASE REPORT: We report the case of a 72-year-old woman suffering from a distal thigh wound with PVGI. The graft was successfully preserved after coverage with a free latissimus dorsi musculocutaneous (LDM) flap. CONCLUSIONS: The free LDM flap procedure is more challenging than local muscle flaps; howev
