Differential effect of compression-ischaemia block on warm sensation and heat-induced pain.

The effect of compression-ischaemia nerve block on psychophysical thresholds for warm sensation and heat-induced pain was studied on 19 normal human volunteers. Although those two sensory submodalities should be predicted to block simultaneously, based on the fact that both are served by unmyelinated primary afferents, it was actually found that warm sensation was much more vulnerable to compression-ischaemia than heat-induced pain. This is interpreted as resulting from different summation requirements for each of the two sensory modalities; sensation of warmth depends on spatial summation to

Ciliary and retinal changes in myotonic dystrophy.

We examined 15 patients with myotonic dystrophy by cycloscopy, ophthalmoscopy, and fluorescein angiography. In six cases, we found depigmentation in the relatively short ciliary processes, which may account for ocular hypotony in this disorder. Eight of the patients had retinal lesions, three with butterfly-shaped dystrophy, two with reticular dystrophy, and three with peripheral yellow flecks. These lesions were biomicroscopically at the level of the retinal pigment epithelium or in the deep retinal layer and did not substantially alter visual functions. Psychophysical and electrophysiologic

Chromatic and luminosity processing in retinal disease.

Color vision loss can be an early sign of eye disease; in many retinal disorders the loss precedes any change in visual acuity. Noninvasive psychophysical methods allow factoring out of preretinal, receptoral, and postreceptoral (neural) components of the color vision change. A loss of chromatic but not achromatic sensitivity occurs for diabetics; the loss is selective for pathways subserving blue-sensitive photoreceptors. Both chromatic and achromatic pathways are altered in glaucoma and senile macular degeneration; the most marked change in central serous choroidopathy is loss of sensitivity

Comparison of susceptibility to motion sickness during rotation at 30 rpm in the earth-horizontal, 10 degrees head-up, and 10 degrees head-down positions.

Normal persons rotated about an Earth-horizontal axis vary in their susceptibility to motion sickness. The purpose of this experiment was to measure, intraindividual differences in susceptibility in 12 subjects when rotated 10 degrees head up and 10 degrees head down as well as in the horizontal position. Subjects assumed the test-position 60 min prior to rotation, thus providing an opportunity for translocation of body fluids. Physiological and psychophysical measurements were conducted throughout the experiment. There were no intraindividual differences in susceptibility to motion sickness i

Development and Clinical Application of CPAP-Specific Patient-Reported Outcome Measures in Obstructive Sleep Apnea.

BACKGROUND: Patient-reported outcome measures (PROMs) are relevant in sleep medicine, but CPAP-specific PROMs for obstructive sleep apnea (OSA) remain insufficiently characterized. We hypothesized that a PROM tailored to CPAP initiation would show acceptable psychometric performance, capture perceived benefit and treatment burden, and identify clinically relevant differences in subject experience to inform adherence support. METHODS: We conducted a prospective cohort study from April 2023 to December 2024 at the Pulmonology Service, with a home respiratory care provider. PROM development incl

Exploration of Olfaction and ChiPSO in Pediatric Cystic Fibrosis.

Background/Objectives: Olfactory dysfunction (OD) is a common symptom among people with cystic fibrosis (PwCF) and contributes to environmental safety concerns, nutritional challenges, and an overall diminished quality of life. OD is perceived to progress along the lifespan in PwCF, often due to worsening sinonasal disease. Among children with cystic fibrosis (CwCF), OD is poorly characterized as limited resources and tolerance contribute to challenges in psychophysical olfactory evaluation among pediatric populations. The Children's Personal Significance of Olfaction (ChiPSO) questionnaire wa

The feasibility of a complex intervention, a medical tattooing nurse-led clinic after breast cancer surgery: results from the ARCADE_M mixed-methods study.

Medical tattoos after breast cancer surgery are a therapeutic approach to regain psychophysical integrity. The tattooing of the Nipple-Areola Complex (NAC) in the ARCADE clinic involves three sessions, 30-40 days apart, and specialized nurses perform it upon medical indication. This study aims to assess the feasibility of a NAC tattooing clinic for women who have undergone surgical treatment for breast cancer in the Comprehensive Cancer Center of Azienda USL-IRCCS di Reggio Emilia, in Italy. This project is guided by the Medical Research Council framework for the development and evaluation of

Parietal function in good and poor readers.

BACKGROUND: While there are many psychophysical reports of impaired magnocellular pathway function in developmental dyslexia (DD), few have investigated parietal function, the major projection of this pathway, in good and poor readers closely matched for nonverbal intelligence. In view of new feedforward-feedback theories of visual processing, impaired magnocellular function raises the question of whether all visually-driven functions or only those associated with parietal cortex functions are equally impaired and if so, whether parietal performance is more closely related to general ability l

Speed of visual processing across the visual field.

Despite the phenomenological experience of vision being stable and uniform in both space and time, our visual system constructs this representation from markedly inhomogeneous spatial and temporal structures. While considerable progress has been made in understanding the inhomogeneities in spatial vision, much less attention has been given to the inhomogeneities in the temporal dynamics of vision. Specifically, little is known about how speed of processing varies across the visual field. In this non-systematic review, we focus primarily on psychophysical and neurophysiological studies of low-l

Pediatrics for Disability: A Comprehensive Approach to Children with Syndromic Psychomotor Delay.

Intellectual disability is the impairment of cognitive, linguistic, motor and social skills that occurs in the pediatric age and is also described by the term "mental retardation". Intellectual disability occurs in 3-28 % of the general population due to a genetic cause, including chromosome aberrations. Among people with intellectual disabilities, the cause of the disability was identified as a single gene disorder in up to 12 %, multifactorial disorders in up to 4 %, and genetic disorders in up to 8.5 %. Children affected by a malformation syndrome associated with mental retardation or intel

Pre-stimulus alpha predicts inattentional blindness.

Pre- and post-stimulus oscillatory activity between 8 and 12 hertz, referred to as the alpha-band, correlates with conscious visual awareness of stimuli across a variety of psychophysical tasks. Within an EEG-adapted inattentional blindness task, the current study sought to examine whether this relationship holds for conscious awareness of stimuli under conditions of inattentional blindness. Noticing rates of the task-irrelevant unexpected stimulus were correlated with a significant decrease in alpha power over bilateral parietal-occipital areas during the pre-stimulus interval, and a signific

Evaluation of olfactory dysfunction in neurodegenerative diseases.

It is known that the olfactory dysfunction is involved in various neurological diseases, such as Parkinson's disease, Alzheimer's disease, multiple sclerosis, Huntington's disease and motor neuron disease. In particular, the ability to identify and discriminate the odors, as well as the odor threshold, can be altered in these disorders. These changes often occur as early manifestation of the pathology and they are not always diagnosed on time. The aim of this review is to summarize the major neurological diseases which are preceded or accompanied by olfactory dysfunction. In addition, new inst

Who is looking at me? The cone of gaze widens in social phobia.

Gaze direction is an important cue that regulates social interactions and facilitates joint attention. Although humans are very accurate in determining gaze directions in general, they have a surprisingly liberal criterion for the presence of mutual gaze. Using an established psychophysical task that required observers to adjust the eyes of a virtual head to the margins of the area of mutual gaze, we examined whether the resulting cone of gaze is altered in people with social phobia. It turned out that during presence of a second virtual person, the gaze cone's width was specifically enlarged

Impaired acuity of the approximate number system underlies mathematical learning disability (dyscalculia).

Many children have significant mathematical learning disabilities (MLD, or dyscalculia) despite adequate schooling. The current study hypothesizes that MLD partly results from a deficiency in the Approximate Number System (ANS) that supports nonverbal numerical representations across species and throughout development. In this study of 71 ninth graders, it is shown that students with MLD have significantly poorer ANS precision than students in all other mathematics achievement groups (low, typically, and high achieving), as measured by psychophysical assessments of ANS acuity (w) and of the ma

Safety issues in functional capacity evaluation: findings from a trial of a new approach for evaluating clients with chronic back pain.

Although safety is recognized as a critical issue in functional capacity evaluations (FCEs), it has rarely been investigated. This paper reports on the findings of a study which examined safety aspects of a new approach to FCE. Fourteen rehabilitation clients with chronic back pain participated in the study. Aspects examined included the pre-FCE screening procedures, the monitoring of performance and safety during the FCE, and the end of FCE measures and follow-up procedures. Support was found for the screening procedures of the approach, particularly blood pressure measurement, and for the co

Multiple congenital symmetric skin dimples.

Skin dimples are seldom observed in sites other than the face. We report on a male premature child who was seen at the age of 2 months for the evaluation of cutaneous depressions symmetrically located on the shoulders, elbows and in the sacral region. Skin dimples have sometimes been considered a benign autosomal dominant trait. However, several authors have reported these cutaneous defects in a variety of conditions like congenital syndromes, infections, inborn errors of metabolism and mechanical trauma. In our case, the aetiology is unknown, even though maternal drug or infective exposure ca

To see but not to read; the magnocellular theory of dyslexia.

Developmental dyslexics often complain that small letters appear to blur and move around when they are trying to read. Anatomical, electrophysiological, psychophysical and brain-imaging studies have all contributed to elucidating the functional organization of these and other visual confusions. They emerge not from damage to a single visual relay but from abnormalities of the magnocellular component of the visual system, which is specialized for processing fast temporal information. The m-stream culminates in the posterior parietal cortex, which plays an important role in guiding visual attent

Selective magnocellular damage in melanoma-associated retinopathy: comparison with congenital stationary nightblindness.

Psychophysical methods for isolating and evaluating the function of specific neural pathways are used to characterize the visual losses in patients with melanoma-associated retinopathy (MAR). These are compared with those of congenital stationary nightblindness (CSNB), a condition which displays a similar grossly abnormal ERG and loss of rod function. In MAR patients achromatic contrast sensitivity was greatly reduced in the low spatial frequency range. Stimuli chosen to isolate the magnocellular pathway were seen badly, whereas stimuli signalled primarily by the midget of the parvocellular pa

[Diagnostic methods in the assessment of anosmia in neurologic diseases].

Several important neurological diseases can impair the sense of smell. In the past the substances generally used for testing were not well defined, and the testing procedures did not follow the rules of psychophysical test procedures. Therefore, in this paper standardized new methods of testing the olfactory system are presented. Based on published data, the University of Pennsylvania Smell Identification Test is the most suitable test for screening purposes. More detailed information can be obtained when using the Connecticut Chemosensory Clinical Research Center's Odor Identification Test th
