A socio-ecological model of the management of leprosy reactions in Indonesia and India using the experiences of affected individuals, family members and healthcare providers.

BACKGROUND: Leprosy reactions (LR) are immune-mediated complications of leprosy that may be associated with severe and irreversible nerve damage. Non-medical aspects, such as financing, service provision, and healthcare resources in the management of LR are generally overlooked as studies tend to concentrate on clinical features and treatment. Barriers to accessing care and services are a major cause of suboptimal care for people with leprosy. This study aims to explore the barriers to and facilitators of high-quality care and management of LR in two leprosy-endemic countries with different he

Mortality outcomes and economic burden of antimicrobial-resistant bacterial bloodstream infections in a Nigerian tertiary hospital.

BACKGROUND: Antimicrobial resistance (AMR) poses a global public health and economic challenge, significantly contributing to poor outcomes of bacteria bloodstream infections (BSI). There is a paucity of local data on outcomes of AMR BSIs in Nigeria. This prospective cohort study determined outcomes and healthcare costs of BSIs stratified by resistance status. METHOD: Data were prospectively collected from enrolled participants (neonates, children and adults) with susceptible (cohort 1) and resistant (cohort 2) BSIs caused by Enterobacterales, Pseudomonas spp, Acinetobacter spp., Staphylococc

The economic burden of lung cancer in low- and lower-middle-income countries: a systematic review.

BACKGROUND: Lung Cancer is one of the four most frequent malignant neoplasms in the world, and first in mortality. In 2020, lung cancer resulted in roughly 1.8 million deaths globally. Lung cancer is a major public health problem in low and lower-middle income countries, where high rates of tobacco smoking, environmental pollution, and limited access to healthcare resources contribute to a significant burden of the disease. The objective of our review was to compare costs among available countries and identify major cost drivers. METHODS: A systematic database search was performed according t

Genital inflammation screening for predicting sexually transmitted infections and bacterial vaginosis: an updated cost analysis from the GIFT study in Madagascar, South Africa, and Zimbabwe.

BACKGROUND: Sexually transmitted infections (STIs) and bacterial vaginosis (BV) are significant public health issues, particularly in sub-Saharan Africa, and are associated with genital inflammation and increased HIV acquisition risk. A substantial proportion of these infections are asymptomatic, limiting the effectiveness of syndromic management. The Genital InFlammation Test (GIFT), a novel rapid point-of-care (POC) test, was developed to detect elevated inflammatory biomarkers associated with genital inflammation. The first-in-field prototype of the GIFT device was evaluated in a multicente

The impact of frailty and fear of falling on health-related quality of life in older adults with osteoporosis: a cross-sectional study.

BACKGROUND: As population aging intensifies, osteoporosis has emerged as a critical public health concern in China, substantially impacting the health and well-being of older adults. Osteoporosis not only diminishes health-related quality of life in late life but also contributes to increased healthcare expenditures and societal burdens. In older adults with osteoporosis, frailty and fear of falling are prevalent and interrelated conditions that further impair health-related quality of life. Addressing frailty and fear of falling in this population may significantly improve health-related qual

The Cost and Financial Burden of Thyroid Cancer on Patients in the US: A Review and Directions for Future Research.

IMPORTANCE: In the US, thyroid cancer has been identified as a cancer type with a high degree of associated financial burden on patients, and survivors of thyroid cancer report higher rates of bankruptcy than those of other cancer types. However, the available literature on the financial burden of thyroid cancer has not yet been described. OBSERVATIONS: Estimates of the out-of-pocket costs of initial thyroid cancer diagnosis and treatment range widely ($1425-$17 000) and are influenced by age, surgical treatment type, and health insurance coverage. The rates of patient-reported financial burd

Antibody development for preventing the human respiratory syncytial virus pathology.

Human respiratory syncytial virus (hRSV) is the most important etiological agent causing hospitalizations associated with respiratory diseases in children under 5 years of age as well as the elderly, newborns and premature children are the most affected populations. This viral infection can be associated with various symptoms, such as fever, coughing, wheezing, and even pneumonia and bronchiolitis. Due to its severe symptoms, the need for mechanical ventilation is not uncommon in clinical practice. Additionally, alterations in the central nervous system -such as seizures, encephalopathy and en

Gathering momentum for the way ahead: fifth report of the Lancet Standing Commission on Liver Disease in the UK.

This report presents further evidence on the escalating alcohol consumption in the UK and the burden of liver disease associated with this major risk factor, as well as the effects on hospital and primary care. We reiterate the need for fiscal regulation by the UK Government if overall alcohol consumption is to be reduced sufficiently to improve health outcomes. We also draw attention to the effects of drastic cuts in public services for alcohol treatment, the repeated failures of voluntary agreements with the drinks industry, and the influence of the industry through its lobbying activities.

Public health and chronic low chlordecone exposures in Guadeloupe; Part 2: Health impacts, and benefits of prevention.

BACKGROUND: Inhabitants of Guadeloupe are chronically exposed to low doses of chlordecone via local food due to its past use in banana plantations. The corresponding health impacts have not been quantified. We develop a quantitative method and present the results in two articles: 1. Hazard identification, exposure-response functions, and exposure, 2. Health impacts, and benefits of a program to reduce the exposure of the population. Here is the second article. METHODS: The exposure-response functions derived in Part 1 (for liver and prostate cancer, renal dysfunction and cognitive development

2011 Alzheimer's disease facts and figures.

Alzheimer's disease (AD) is the sixth leading cause of all deaths in the United States and is the fifth leading cause of death in Americans aged ≥65 years. Although other major causes of death have been on the decrease, deaths because of AD have been rising dramatically. Between 2000 and 2008 (preliminary data), heart disease deaths decreased by 13%, stroke deaths by 20%, and prostate cancer-related deaths by 8%, whereas deaths because of AD increased by 66%. An estimated 5.4 million Americans have AD; approximately 200,000 people aged <65 years with AD comprise the younger-onset AD population

Assessing access barriers to tuberculosis care with the tool to Estimate Patients' Costs: pilot results from two districts in Kenya.

BACKGROUND: The poor face geographical, socio-cultural and health system barriers in accessing tuberculosis care. These may cause delays to timely diagnosis and treatment resulting in more advanced disease and continued transmission of TB. By addressing barriers and reasons for delay, costs incurred by TB patients can be effectively reduced. A Tool to Estimate Patients' Costs has been developed. It can assist TB control programs in assessing such barriers. This study presents the Tool and results of its pilot in Kenya. METHODS: The Tool was adapted to the local setting, translated into Kiswah

Economic analysis of the global polio eradication initiative.

The global polio eradication initiative (GPEI), which started in 1988, represents the single largest, internationally coordinated public health project to date. Completion remains within reach, with type 2 wild polioviruses apparently eradicated since 1999 and fewer than 2000 annual paralytic poliomyelitis cases of wild types 1 and 3 reported since then. This economic analysis of the GPEI reflects the status of the program as of February 2010, including full consideration of post-eradication policies. For the GPEI intervention, we consider the actual pre-eradication experience to date followed

Sickness absence poses a threat to the Swedish Welfare State: a cross-sectional study of sickness absence and self-reported illness.

BACKGROUND: The increasing cost of public social sickness insurance poses a serious economic threat to the Swedish welfare state. In recent years, expenditures for social insurance in general, as well as social sickness insurance in particular, have risen steeply in Sweden. This cross-sectional study analyzed the association between sickness absence (SA) and self-reported reduced working capacity due to a longstanding illness (>3 months), as well between SA and a number of other health problems. METHODS: Self-reported data on longstanding illness and resultant reduced working capacity, socioe

Engaging with community-based public and private mid-level providers for promoting the use of modern contraceptive methods in rural Pakistan: results from two innovative birth spacing interventions.

BACKGROUND: Family planning (FP) interventions aimed at reducing population growth have negligible during the last two decades in Pakistan. Innovative FP interventions that help reduce the growing population burden are the need of the hour. Marie Stopes Society--Pakistan implemented an operational research project--'Evidence for Innovating to Save Lives', to explore effective and viable intervention models that can promote healthy timing and spacing of pregnancy in rural and under-served communities of Sindh, Punjab and Khyber Pakhtunkhwa provinces of Pakistan.
