Managing Postpartum Complications in the Emergency Department.

Postpartum patients may present to the emergency department with complaints ranging from minor issues, requiring only patient education and reassurance, to severe, life-threatening complications that require prompt diagnosis and multidisciplinary consultation and management. At times, vague presentations or overlapping conditions can make it difficult for the emergency clinician to recognize an emergent condition and initiate proper treatment. This issue reviews the major common emergencies that present in postpartum patients, by chief complaint, including hemorrhage, infection, pre-eclampsia,

[Peripartum cardiomyopathy: A review of the literature].

Peripartum cardiomyopathy is a rare and unpredictable pregnancy-related pathology. Idiopathic cardiomyopathy is characterized by a heart failure secondary to left ventricular systolic dysfunction appearing towards the end of pregnancy or in the months following delivery with a non-specific clinic presentation. Through reviewing previous research, our critical literature review wishes to bring a concise and objective summarize for a better understanding of physiopathology, evocative symptoms and knowing of factors influencing prognosis in order to standardize peripartum management. The treatmen

Stroke during pregnancy and puerperium among Japanese women: a single-center registry.

BACKGROUND: Stroke is one of the major causes of maternal death. This study aimed to analyze the maternal and fetal outcomes of stroke occurred during pregnancy and puerperium. METHODS: We conducted a retrospective analysis of patients admitted to our perinatology center between 1982 and 2012 with a diagnosis of acute cerebral stroke during pregnancy or within 6 weeks postpartum. RESULTS: Thirty-four patients were registered and all the patients had never been diagnosed as stroke nor detected cerebrovascular abnormalities before the current pregnancies. They were divided into 8 ischemic stro

Purtscher-like Retinopathy After Placental Abruption.

BACKGROUND: Pregnant women are susceptible to visual disturbances, particularly with hypertension. Vision generally recovers postpartum within hours to days. A rare condition known as Purtscher-like retinopathy presents abruptly postpartum with visual field defects and blindness. Recovery of vision is unpredictable, occurring over several weeks and months. CASE: A 38-year-old woman presented at 34 weeks' gestation with contractions and heavy bleeding. Placental abruption was confirmed after a repeat cesarean was performed. Hours after delivery, she complained of central vision loss despite nor

Henoch-Schönlein purpura nephritis occurring postpartum in a patient with anti-PL-7 anti-synthetase syndrome.

A 37-year-old pregnant woman developed purpura which was subsequently diagnosed as Henoch-Schönlein purpura (HSP). After childbirth, the patient developed proteinuria and hematuria. Further examination revealed that the HSP nephritis (HSPN) was associated with anti-threonyl-tRNA synthetase anti-synthetase syndrome. The onset of HSPN during pregnancy or after childbirth is rare. Moreover, to our knowledge, this is the first case to describe renal involvement in anti-synthetase syndrome.

Prevalence of bacteremia in dairy cattle with acute puerperal metritis.

BACKGROUND: Acute puerperal metritis (APM) affects 30% of postpartum dairy cattle. Bacteremia negatively impacts survival in cattle with coliform mastitis. However, the prevalence of bacteremia in dairy cattle with APM is unknown. HYPOTHESIS: Bacteremia is detectable in a large proportion of cattle with APM. ANIMALS: Seventeen dairy cows with APM and 17 healthy dairy cattle. METHODS: Prospective case-control study. Cases were identified by daily monitoring of cattle in the first 10 days after calving. Controls were matched to cases by parity and days in milk. Cows were examined at the time

Cerebral venous thrombosis complicating severe preeclampsia in the postpartum period: a diagnostic challenge.

A 26 year old primigravida with preeclampsia was diagnosed with a cerebral venous thrombosis 6 days following Cesarean section. The diagnosis was initially challenging due to the patient's history of migraines, the preeclampsia, multiple attempts at spinal anesthesia for Cesarean section, and a dural puncture while performing epidural blood patch.

Uterine prolapse in pregnancy: risk factors, complications and management.

Presentation of uterine prolapse is a rare event in a pregnant woman, which can be pre-existent or else manifest in the course of pregnancy. Complications resulting from prolapse of the uterus in pregnancy vary from minor cervical infection to spontaneous abortion, and include preterm labor and maternal and fetal mortality as well as acute urinary retention and urinary tract infection. Moreover, affected women may be at particular risk of dystocia during labor that could necessitate emergency intervention for delivery. Recommendations regarding the management of this infrequent but potentially

Prevention and treatment of postpartum hypertension.

BACKGROUND: Postpartum blood pressure (BP) is highest three to six days after birth when most women have been discharged home. A significant rise in BP may be dangerous (e.g., can lead to stroke), but there is little information about how to prevent or treat postpartum hypertension. OBJECTIVES: To assess the relative benefits and risks of interventions to: (1) prevent postpartum hypertension, by assessing whether 'routine' postpartum medical therapy is better than placebo/no treatment; and (2) treat postpartum hypertension, by assessing whether (i) one antihypertensive therapy is better than

Bilateral internal carotid artery dissection in the postpartum period.

BACKGROUND: Bilateral internal carotid artery dissection is a rare event associated with pregnancy, especially in a patient without any predisposing risk factors. CASE: A 34-year-old woman presented with postpartum unilateral weakness, headaches, and blurry vision 14 days after vaginal delivery. Radiologic imaging revealed bilateral cervical internal carotid artery dissections and cerebral infarctions. She was treated with anticoagulation and showed radiographic and clinical improvement. CONCLUSION: The pathophysiology of cervical artery dissection appears multifactorial, with evidence sugge

Low back pain post partum - a case report.

Sacral stress fracture is an unusual source of low back pain and can be easily confused with a number of other clinical conditions seen in physiotherapy clinics. The purpose of this case report is to describe the case of a patient presenting with low back pain post partum illustrating pertinent aspects of differential diagnosis and issues of management. A 31 year old female presented complaining of low back pain since the birth of her second child 11 months earlier. Subjective and objective examination led the Therapist to consider a diagnosis of sacral stress fracture. This diagnosis was conf

Postpartum headache due to spontaneous cervical artery dissection.

Postpartum headache is quite common and often related to potentially ominous cerebrovascular accidents. As illustrated in previously published reports, spontaneous cervical artery dissection is a rare but possible cause of headache in the postpartum. We provide 2 additional cases to the 19 described so far, including the first ever report of migraine with aura-like symptoms. Additionally, we summarize the literature and we speculate about the possible etiopathological mechanism underlying this condition.

[Viral encephalitis in puerperium period--case report].

Herpetic encephalitis (HSE) is one of the most severe infections of Central Nervous System (CNS). If not treated, it results in patient's death in 70% cases, although in properly treated cases the mortality rate is 30%. The most endangered group are patients with immunosuppression, including pregnant women and women in the peripartum period. We present a case of a young woman in puerperium period, who developed herpetic encephalitis. Despite proper treatment patient required long term hospitalization in ICU, where she was placed in pharmacological coma. Despite severe course of the disease no

Peripartum cardiomyopathy: a short review.

Peripartum cardiomyopathy is an unusual form of dilated cardiomyopathy, which manifests as acute heart failure in the last trimester of pregnancy or early postpartum period. Its aetiology is currently unknown. The presenting signs and symptoms are those of congestive heart failure and more specifically those of left ventricular failure. Its importance lies in the fact that it has a high mortality rate and strikes the patient in the prime of life. Peripartum cardiomyopathy has far reaching implications for the anaesthesiologist. The reason for this is that many of the signs and symptoms of norm

Reversible dilated cardiomyopathy associated with post-partum thyrotoxic storm.

The following case of a young mother presenting with post-partum pulmonary oedema, highlights a singular and life-threatening complication of a common condition (thyrotoxicosis). While thyrotoxic heart disease is well described, thyrotoxic storm in the post-partum period associated with dilated cardiomyopathy, congestive cardiac failure and ventricular fibrillation is exceptional.

[Late amniotic fluid embolism among the diferential diagnosis of acute respiratory failure in the postpartum: case report and review].

The amniotic fluid embolism is an uncommon condition with a high mortality. The cardinal symptoms are hypoxia, hypotension, altered mental status and disseminated intravascular coagulation. This syndrome occurs during delivery or in the immediate postpartum period, and its onset in the late postpartum is very unusual. We describe a case of a primigravida who, after an uneventful delivery, suffers an acute respiratory failure in the late postpartum period. Exclusion of other causes of acute respiratory failure occurring during delivery or in the postpartum period led to establish the diagnosis.
