Regional Heterogeneity of Chronic Obstructive Pulmonary Disease Phenotypes: Pulmonary (3)He Magnetic Resonance Imaging and Computed Tomography.

Pulmonary ventilation may be visualized and measured using hyperpolarized (3)He magnetic resonance imaging (MRI) while emphysema and its distribution can be quantified using thoracic computed tomography (CT). Our objective was to phenotype ex-smokers with COPD based on the apical-to-basal distribution of ventilation abnormalities and emphysema to better understand how these phenotypes change regionally as COPD progresses. We evaluated 100 COPD ex-smokers who provided written informed consent and underwent spirometry, CT and (3)He MRI. (3)He MRI ventilation imaging was used to quantify the vent

Relationship between quantitative CT metrics and health status and BODE in chronic obstructive pulmonary disease.

BACKGROUND: The value of quantitative CT (QCT) to identify chronic obstructive pulmonary disease (COPD) phenotypes is increasingly appreciated. The authors hypothesised that QCT-defined emphysema and airway abnormalities relate to St George's Respiratory Questionnaire (SGRQ) and Body-Mass Index, Airflow Obstruction, Dyspnea and Exercise Capacity Index (BODE). METHODS: 1200 COPDGene subjects meeting Global Initiative for Chronic Obstructive Lung Disease (GOLD) criteria for COPD with QCT analysis were included. Total lung emphysema was measured using the density mask technique with a -950 Houns

Centrilobular Emphysema Is Associated with Pectoralis Muscle Reduction in Current Smokers without Airflow Limitation.

BACKGROUND: Physiological and prognostic associations of centrilobular emphysema (CLE) and paraseptal emphysema (PSE) in smokers with and without chronic obstructive pulmonary disease (COPD) have been increasingly recognized, but the associations with extrapulmonary abnormalities, such as muscle wasting, osteoporosis, and cardiovascular diseases, remain unestablished. OBJECTIVES: The aim of the study was to investigate whether CLE was associated with extrapulmonary abnormalities independent of concomitant PSE in smokers without airflow limitation. METHODS: This retrospective study consecutiv

Mesenchymal stromal cells-based therapy in a murine model of elastase-induced emphysema: Simvastatin as a potential adjuvant in cellular homing.

Chronic Obstructive Pulmonary Disease - COPD is characterized by the destruction of alveolar walls associated to a chronic inflammatory response of the airways. There is no clinical therapy for COPD. In this context, cell-based therapies represent a promising therapeutic approach for chronic lung disease. The goal of this work was to evaluate the effect of simvastatin on cell-based therapy in a mice emphysema model. Female FVB mice received intranasal instillation of elastase (three consecutive doses of 50 μL) in order to promote pulmonary emphysema. After 21 days of the first instillation, th…

Thoracoscopic Lobectomy in Infants and Children.

Indications for pulmonary lobectomy in infants and children include cystic pulmonary adenomatoid malformation, congenital lobar emphysema, chronic infection, and malignancy. These procedures can now all be done thoracoscopically avoiding the short- and long-term morbidity of an open thoracotomy. In this article we describe the technique of thoracoscopic lobectomy as well as the preoperative and postoperative care.

AARS1-Mediated RUNX3 K193 Lactylation Enhances the Activation and Cytotoxicity of CD8+T Cells in Smoking-Related Emphysema.

Emphysema is the primary pathological feature of chronic obstructive pulmonary disease (COPD). The accumulation of CD8+T cells induces epithelial cell damage, thereby impairing the pulmonary alveolar niche. In this study, we demonstrated that CD8+T cell activation was associated with COPD progression. Furthermore, our data indicated that AARS1-mediated lactylation was involved in cigarette smoke extract (CSE)-induced CD8+T cell activation. Mechanistically, cigarette smoke (CS) exposure upregulated AARS1 levels in CD8+T cells. Elevated AARS1 catalyzed the transfer of a lactyl group to RUNX3 at

The Goddard score for emphysema predicts the postoperative prognosis in pancreatic cancer.

PURPOSE: Chronic inflammation contributes to carcinogenesis and pulmonary emphysema is characterized by chronic pulmonary inflammation. We hypothesized that the severity of emphysema, quantitatively assessed using the Goddard score (GS), correlates with outcomes following surgical treatment for pancreatic cancer. METHODS: We retrospectively analyzed 191 patients who underwent surgery for pancreatic cancer between 2013 and 2022. GS was assessed using routine preoperative computed tomography. Univariate and multivariate analyses were performed to evaluate the impact of GS on oncological outcome…

Oxygen cost of breathing in patients with emphysema or chronic bronchitis in acute respiratory failure.

This study compared the oxygen cost of breathing (VO2 resp) in 19 patients with severe chronic obstructive pulmonary disease intubated for acute respiratory failure. Ten patients showed radiologic (X-ray and/or computed tomographic scan) evidence of emphysema. The remaining ones were considered as having chronic bronchitis. Measurements were made just before extubation. Despite similar expiratory airflow obstruction, patients with emphysema exhibited significantly higher VO2 resp than patients with chronic bronchitis (109 +/- 61 versus 42 +/- 26 ml/min/m2, respectively; p < 0.006). Moreover, e

Massive subcutaneous emphysema, pneumomediastinum, and pneumopericardium in children.

Massive subcutaneous emphysema (SE), pneumomediastinum (PM), and pneumopericardium (PP) are rare conditions in the pediatric population. Air leak syndrome is a constellation of disorders that include SE, PM, PP, and pulmonary interstitial emphysema. In children, SE, PM, and PP are associated with obstructive airway disease most often in the case of asthma. Management may be conservative or involve invasive procedures that require surgical intervention. Here, we describe a case of massive SE, PM, and PP in a 10-year-old child after placement of a peripherally inserted central line and review th

Sustained dapsone-induced remission of hypocomplementemic urticarial vasculitis--a case report.

Hypocomplementemic urticarial vasculitis (HUV) is often misdiagnosed. The response to drug therapy may be unsatisfactory, and immunosuppressive therapy may be associated with significant side effects. A 35-year-old patient whose condition was resistant to cyclophosphamide, corticosteroids, pentoxyphylline, cyproheptadine, sulindac, and colchicine was maintained in remission with dapsone, which may be the drug of choice for HUV. Emphysema has been reported to complicate HUV, but this is the first account of vasculitis-related emphysema with no confounding history of tobacco smoke exposure. The

Management of extensive surgical emphysema with subcutaneous drain: A case report.

INTRODUCTION: Subcutaneous emphysema (SE) is a frequent and often self-limiting complication of tube thoracostomy or other cardiothoracic procedures. On rare occasions, severe and extensive surgical emphysema marked by palpable cutaneous tension, dysphagia, dysphonia, palpebral closure or associated with pneumoperitoneum, airway compromise, "tension phenomenon" and respiratory failure require treatment. PRESENTATION OF CASE: A 67 year old lady presented with a large spontaneous pneumothorax on the background of end-stage chronic obstructive pulmonary disease (COPD) and newly diagnosed lung ca

Genes, oxidative stress, and the risk of chronic obstructive pulmonary disease.

BACKGROUND: The first-pass metabolism of foreign compounds in the lung is an important protective mechanism against oxidative stress. We investigated whether polymorphisms in the gene for microsomal epoxide hydrolase (mEPHX), an enzyme involved in this protective process, had any bearing on individual susceptibility to the development of chronic obstructive pulmonary disease (COPD) and emphysema. METHODS: We designed PCR-based genotyping assays to detect variant forms of mEPHX that confer slow and fast activity. We used these assays to screen 203 blood-donor controls and groups of patients wi

Congenital lobar emphysema: differential diagnosis and therapeutic approach.

BACKGROUND: Congenital lobar emphysema (CLE) is a rare anomaly of lung development that usually presents in the neonatal period with respirator distress and pulmonary lobar hyperinflation. It is commonly confused with pneumothorax. The aim of the present paper was to review the authors' experience in order to emphasize the importance of differential diagnosis with pneumothorax. METHODS: Children with CLE treatment at Department of Thoracic Surgery, Dicle University School of Medicine, Turkey, between January 1993 and June 2004, were reviewed. RESULTS: Ten children consisting of six boys and

Angiotensin-Converting Inhibitors and Angiotensin II Receptor Blockers and Longitudinal Change in Percent Emphysema on Computed Tomography. The Multi-Ethnic Study of Atherosclerosis Lung Study.

RATIONALE: Although emphysema on computed tomography (CT) is associated with increased morbidity and mortality in patients with and without spirometrically defined chronic obstructive pulmonary disease, no available medications target emphysema outside of alpha-1 antitrypsin deficiency. Transforming growth factor-β and endothelial dysfunction are implicated in emphysema pathogenesis, and angiotensin II receptor blockers (ARBs) inhibit transforming growth factor-β, improve endothelial function, and restore airspace architecture in murine models. Evidence in humans is, however, lacking. OBJECTI

Noninvasive and invasive pulmonary function in mouse models of obstructive and restrictive respiratory diseases.

Pulmonary function analysis is an important tool in the evaluation of mouse respiratory disease models, but much controversy still exists on … ed in well-established models of asthma (protein and chemical induced): a model of elastase-induced pulmonary emphysema, and a model of bleomycin-induced pulmonary fibrosis. In contrast to noninvasive tests, both invasive techniques were efficacious for the quantification of parenchymal disease via changes in functional residual capacity, total lung capacity, vital c …

Defining CT subtypes in chronic obstructive pulmonary disease: real world daily practice does not meet guidelines.

AIMS: To evaluate the quality and inter-rater reliability of CT-definable chronic obstructive pulmonary disease (COPD) subtype reporting in CT chest reports in a real-world setting, and assess concordance with Fleischner Society guidelines. METHODS: We undertook a retrospective review of 100 randomly selected CT chest scans containing the terms 'emphysema' or 'COPD'. Existing reports were evaluated for the description of emphysema phenotype, severity, and location, as well as the presence of associated findings, benchmarked against the Fleischner Society guidelines for CT reporting. The scans
