Resistant hypertension, secondary hypertension, and hypertensive crises: diagnostic evaluation and treatment.

Hypertension is a very common modifiable risk factor for cardiovascular morbidity and mortality. Patients with hypertension represent a diverse group. In addition to those with primary hypertension, there are patients whose hypertension is attributable to secondary causes, those with resistant hypertension, and patients who present with a hypertensive crisis. Secondary causes of hypertension account for less than 10% of cases of elevated blood pressure (BP), and screening for these causes is warranted if clinically indicated. Patients with resistant hypertension, whose BP remains uncontrolled

Update in pharmacologic treatment of hypertension.

Initial pharmacologic therapy for hypertension is low-dose thiazide diuretics, beta-blockers, and ACE inhibitors. Increasing data have confirmed that ACE inhibitors have specific benefit in patients with diabetes, atherosclerosis, left ventricular dysfunction, and renal insufficiency. CCBs are alternative agents for ISH in the elderly and appear to decrease stroke with perhaps less protection against progression of renal insufficiency and proteinuria, CAD mortality and new onset heart failure versus other initial agents, especially ACE inhibitors. ARBs are well tolerated and effective blood pr

Evaluating the usage of droperidol in acutely agitated persons with brain injury.

The objective of this study was to compare the effectiveness and safety of intramuscular droperidol to other intramuscularly administered agents used in the management of acutely agitated patients. Twenty-seven inpatients with a history of brain injury were prospectively monitored over a period of 2 months. Data collected for each episode of agitation include: dose, number of doses, time to achieve an adequate response or calming effect, post-episodic functioning, treatment-emergent side effects, and other patient demographics. A retrospective medical records review was also performed on the s

Advances in specific therapy for multiple sclerosis.

During the past year, two new agents have been demonstrated to be of value in multiple sclerosis. The first agent, a form of recombinant interferon beta (Betaseron; Berlex Labs, Cedar Knolls, NJ), is effective in reducing relapses and also prevent accumulation of brain lesions over time, but has no demonstrated effect on the accumulation of disability over time. The second agent, methylprednisolone (Solumedrol; The Upjohn Co., Kalamozoo, MI), administered in megadose pulses over several days and followed by an oral prednisone taper, reduces the conversion rate of patients with optic neuritis t

Small Interfering RNA in Cardiovascular Disease: A New Frontier in Precision Medicine.

Despite advances in traditional lipid-lowering, antithrombotic, and antihypertensive therapies, cardiovascular disease remains the leading cause of mortality worldwide. Small interfering RNA (siRNA) therapeutics harness the endogenous RNA interference pathway to degrade target messenger RNA, thereby reducing synthesis of disease-associated proteins. Because gene silencing occurs intracellularly and persists beyond circulating drug exposure, siRNA agents result in sustained effects with infrequent dosing. Hepatocyte-directed agents targeting PCSK9, LPA, ANGPTL3, APOC3, angiotensinogen, and tran

Smilaxsieboldiol A, a new stilbenoid from the whole plants of Smilax sieboldii Miq. (Smilacaceae).

As part of an ongoing search for new anti-adipogenic agents from medicinal plants, a new stilbenoid (6), together with eight known compounds (1-5 and 7-9), was isolated from the whole plants of Smilax sieboldii Miq. (Smilacaceae). The structure of smilaxsieboldiol A (6) was elucidated using spectroscopic and spectrometric techniques, including 1D and 2D-NMR (COSY, HSQC, and HMBC) and HRESIMS. All isolated compounds were evaluated for metabolic disorder-related activities using an adipocyte differentiation (adipogenesis) inhibition model in 3T3-L1 cells at a concentration of 100 μM. Among them,

A Rationally Engineered MAGL-Activatable Fluorogenic Probe Enables Efficient Discovery of Anti-Inflammatory and Hepatoprotective Agents.

Targeted inhibition of monoacylglycerol lipase (MAGL) represents a promising therapeutic strategy for inflammatory diseases and drug-induced hepatotoxicity. However, the lack of robust screening assays hindered the efficient discovery of potent MAGL inhibitors. Herein, a high-performance MAGL-activatable fluorogenic probe, CN-2, was developed through three-round structural optimization of luminescent scaffold, N-site substituent, and recognition moiety. CN-2 displayed an optimal combination of signal enhancement, detection sensitivity, and favorable kinetic behaviors. CN-2-based high-throughpu

Selective Use of Nonselective Beta-Blockers: A Tailored Antiarrhythmic Approach.

Beta-blockers (BBs) are a cornerstone of antiarrhythmic therapy, yet assuming a uniform "class effect" oversimplifies their diverse pharmacological properties and might compromise efficacy. This review synthesizes evidence comparing the antiarrhythmic effects of different BBs in various clinical settings. While cardioselective agents offer improved tolerability, they often fail to blunt the broad adrenergic surge driving specific arrhythmias. Nonselective BBs (eg, nadolol, propranolol) provide superior efficacy in inherited channelopathies, such as catecholaminergic polymorphic ventricular tac

A facile combined therapy of chemotherapeutic agent and microRNA for hepatocellular carcinoma using non-cationic nanogel.

High drug resistance remains a challenge for chemotherapy against hepatocellular carcinoma (HCC). Combining chemotherapeutic agents with microRNA (miRNA), which simultaneously regulates multiple pathways, offers a promising approach to improve therapeutic efficacy against HCC. Although cationic amphiphilic copolymers have been used to co-deliver these agents, their effectiveness is often limited by low co-encapsulation efficiency and inherent cationic toxicity. In this study, we developed a facile approach to co-deliver doxorubicin (DOX) and miRNA-26a (miR-26a) using a non-cationic nanogel. Th

S100A8 induces cyclophosphamide-induced alopecia via NCF2/NOX2-mediated ferroptosis.

Chemotherapy-induced alopecia (CIA), commonly associated with agents such as cyclophosphamide (CYP), is a prevalent and distressing side effect of numerous chemotherapeutic treatments, significantly impacting patients' quality of life. S100A8, a calcium-binding protein involved in inflammatory responses and oxidative stress regulation, plays a pivotal role in cellular homeostasis. In this study, we investigated the involvement of S100A8 in ferroptosis within a CYP-induced CIA mouse model. We found that CYP treatment upregulated S100A8, NCF2, and NOX2 while reducing GPX4 levels in hair follicle

Clinical Pharmacokinetics of Inhaled Drugs for Pulmonary Hypertension.

Pulmonary hypertension (PH) is a serious condition characterized by elevated blood pressure in the pulmonary arteries. Current treatment approaches mainly focus on using vasodilator agents to reduce pulmonary blood pressure and improve blood flow. Inhalation treatments offer targeted delivery to the lungs, improving efficacy and reducing systemic side effects. Understanding the pharmacokinetics (PK) of the molecules used in the inhalation treatment is crucial for dose optimization and drug product development. This review examines the clinical PK characteristics of key inhaled PH drugs (approv

Antibacterial Activities of Prenylated Flavonoids from Sophora flavences against Xanthomonas oryzae pv oryzae.

The increasing resistance of phytopathogenic bacteria to chemical pesticides has underscored the urgent need for novel bactericidal agents. Sophora flavescens produces a wide range of secondary metabolites with a wide range of biological activities. Here, eight prenylated flavonoids were evaluated for the activity against phytopathogenic bacteria. Among the compounds tested, sophoraflavanone G (SFG) demonstrated broad-spectrum activity, showing a MIC50 of 1.56 μg/mL against Xanthomonas oryzae pv oryzae (Xoo). Subsequently, the mechanism of SFG on Xoo was initially explored. SFG inhibited biofi

GLP-1-based therapies for obesity: Impact on comorbidities or obesity-related diseases.

Agents targeting the glucagon-like peptide-1 receptor (GLP-1R) are effective in managing metabolic conditions associated with obesity, such as obstructive sleep apnea (OSA), metabolic dysfunction-associated steatotic liver disease (MASLD), and chronic kidney disease (CKD). In OSA, studies with first generation GLP-1R agonists (ArGLP-1) and co-agonists (GLP-1/GIP) have demonstrated significant improvements in the apnea-hypopnea index and weight reduction. In MASLD, GLP-1RAs and co-agonists (GLP-1/GIP or GLP-1/glucagon) have shown efficacy in reducing hepatic fat, improving fibrosis, and resolvi

Selection of epigenetically privileged HIV-1 proviruses during treatment with panobinostat and interferon-α2a.

CD4+ T cells with latent HIV-1 infection persist despite treatment with antiretroviral agents and represent the main barrier to a cure of HIV-1 infection. Pharmacological disruption of viral latency may expose HIV-1-infected cells to host immune activity, but the clinical efficacy of latency-reversing agents for reducing HIV-1 persistence remains to be proven. Here, we show in a randomized-controlled human clinical trial that the histone deacetylase inhibitor panobinostat, when administered in combination with pegylated interferon-α2a, induces a structural transformation of the HIV-1 reservoir

Role of Flavonoids in Management of Various Biological Targets in Alzheimer's Disease: Evidence from Preclinical to Clinical Studies.

More than 10 million people worldwide have Alzheimer's disease (AD), a degenerative neurological illness and the most prevalent form of dementia. AD's progression in memory loss, cognitive deterioration, and behavioral changes are all symptoms. Amyloid-beta 42 (Aβ42), the hyperphosphorylated forms of microtubule-associated tau protein, and other cellular and systemic alterations are all factors that contribute to cognitive decline in AD. Rather than delivering a possible cure, present therapy strategies focus on reducing disease symptoms. It has long been suggested that various naturally occur
