The relationship between the vestibulo-ocular reflex (VOR) and gaze fixation was investigated in normal adults, normal children, patients with cerebellar lesion and patients with labyrinthine dysfunction. Large VOR gains were found in patients with cerebellar lesion and normal children, both presenting poor suppression by gaze. Pathological decline in the VOR gain with labyrinthine lesion impaired spatial gaze fixation; however, it did not affect fixation-induced suppression. Spasm of the near reflex is characterized by transient attacks of convergence, accommodation, and miosis. It is usually observed in young individuals and considered functional. We studied seven patients with spasm of the near reflex who had associated neurologic disorders or head trauma. Two patients had posterior fossa abnormalities (cerebellar tumor, Arnold-Chiari malformation), two patients had pituitary tumors, one patient had a vestibulopathy, and two patients had a history of antecedent head trauma.

Reversed or hyperactive stapedial reflexes are found in a wide variety of patients complaining of audiosensitivity, defined as a need to turn down the volume on audio equipment. This was checked in an independent sample of children with learning problems. One or both of these reflex abnormalities were over three times commoner in those reporting audiosensitivity. Persons with diminished or absent vestibulo-ocular reflexes (VORs) are thought to have difficulty with ocular stabilization during sufficiently rapid head movements. This suggests the hypothesis that labyrinthine defective individuals might have impaired visual abilities when in a moving motor vehicle. Blink reflexes were analysed in 28 cases with cerebello-pontine angle tumors (22 cases of acoustic neurinoma and 6 cases of meningioma). In patients with acoustic neurinoma, delay or absence of R1 was found in all but two cases (91%) and abnormality of R2 was in 11 (50%). Circumstantial evidence is given in support of the hypothesis that vagal lung receptors are involved in local axon reflexes. All the components of such mechanisms are present, and teleologically it would make sense for them to exist. The effectiveness of the "diving reflex" in treating paroxysmal supraventricular tachycardia (PSVT) in 10 infants aged under 6 months is demonstrated. No rhythm disturbances were encountered during the maneuver, even in patients receiving digitalis. The recommended water temperature is 5 degrees C and the total time of facial immersion, between 6 and 7 seconds. The diving reflex might be considered the first therapeutic approach in infants under 6 months old with PSVT.

Immersing the patient's face in cold water to induce the "diving reflex" is a quick and simple method of treating paroxysmal supraventricular tachycardia. Patients can be trained to perform the "diving reflex" at home. There are many advantages to this method of treatment when compared to many of the other therapies. Polyuria up to 3 L is often noted during the first 90 minutes of a paroxysmal tachycardia. The diuresis is thought to be due to atrial distention with resultant inhibition of antidiuretic hormone secretion.
