Robotic ureterolysis, retroperitoneal biopsy, and omental wrap for the treatment of ureteral obstruction due to idiopathic retroperitoneal fibrosis.

Retroperitoneal fibrosis (RPF) is a rare disorder of unclear etiology characterized by chronic inflammation of the retroperitoneum, which can involve any of the retroperitoneal structures, most notably the ureters, aorta, and vena cava. Historically, open biopsy, ureterolysis, and transpositioning or omental wrapping of the involved ureter(s) have been the preferred surgical treatments of RPF, with success rates greater than 90%. More recently, successful laparoscopic biopsy, ureterolysis, and ureteral omental wrapping and intraperi-tonealization have been described. We report the first case i…

Retroperitoneal fibrosis associated with immunoglobulin IgG4-related disease in the differential diagnosis in retroperitoneal tumors. Case report.

The patient was a 55-year-old woman with a 4-month history of pain in left flank that irradiated to the lumbar region. Computed tomography revealed a cystic tumor in the retroperitoneal region that compresses the ureter and left renal pelvis in contact with pancreatic body and tail. Surgery was performed and the definitive pathological report diagnosed nodular retroperitoneal fibrosis associated with IgG4; Ki67-positive in germinal centers (5%) and IgG4-positive (40 plasma cells in 3 fields of 40x) by immunohistochemistry. Retroperitoneal fibrosis is a rare disease, that develops gradually and…

Robot-assisted ureterolysis, retroperitoneal biopsy, and omental wrap: pilot series for the treatment of idiopathic retroperitoneal fibrosis.

BACKGROUND AND PURPOSE: Retroperitoneal fibrosis (RPF) is an uncommon disease of vague cause distinguished by a chronic inflammatory response. Traditionally, RPF with ureteral involvement has been managed with open ureterolysis and transposition, with excellent success rates. More recently, laparoscopic ureterolysis has been described. Here, we report our experience of managing idiopathic RPF with robot-assisted ureterolysis, retroperitoneal biopsy, and ureteral omental wrapping. PATIENTS AND METHODS: We performed robot-assisted ureterolysis, retroperitoneal biopsy, and ureteral omental wrapp…

Autoantibody reactome analysis reveals novel biomarkers for idiopathic retroperitoneal fibrosis.

OBJECTIVE: Idiopathic retroperitoneal fibrosis (iRPF) is a rare disease frequently misdiagnosed due to the lack of reliable biomarkers. This study aimed to identify novel autoantibodies to improve the diagnosis and differential diagnosis of iRPF. METHODS: A total of 33 patients with iRPF, 100 healthy controls, 11 with retroperitoneal sarcoma (RPS) and 74 with other autoimmune diseases were recruited from Renji Hospital from January 2018 to April 2025. Autoantibodies were screened using the Sengenics Immunome Protein Array. Their expression and particularly their IgG4 subclass were then valida…

Augmented reality for guiding biopsies in rare retroperitoneal fibrosis cases.

Retroperitoneal fibrosis (RPF) is a rare immune-mediated fibro-inflammatory disorder that often encases retroperitoneal structures, posing diagnostic and procedural challenges. Conventional image-guided biopsy techniques using CT or MRI may lead to missed lesions, sampling errors, and repeated procedures. Augmented reality (AR) platforms, such as Microsoft HoloLens-based systems, overlay 3D patient-specific anatomy in real time, enabling clinicians to visualize biopsy needle trajectories without dividing attention between monitors and the procedural field. Phantom studies have shown AR guidanc…

A Case of Idiopathic Retroperitoneal Fibrosis Triggering Recurrent Gastrointestinal Bleeding.

Idiopathic retroperitoneal fibrosis (IRF) is a rare condition characterized by fibrous tissue proliferation in the retroperitoneal space, commonly affecting the ureters and other abdominal structures. This case report describes a previously undocumented presentation of IRF in a 52-year-old female, who presented with recurrent gastrointestinal bleeding and severe anemia over six months. Diagnostic workup included endoscopy, colonoscopy, abdominal computed tomography (CT), and biopsy, revealing fibrous encasement of the mesenteric vessels leading to ischemic damage and gastrointestinal bleeding.…

Bilateral Ureteric Obstruction Due to Retroperitoneal Fibrosis: A Case Report.

Retroperitoneal fibrosis is a rare disease with a largely unknown aetiology, varying presentation, and is characterized by fibrous tissue formation in the retroperitoneal region. It causes entrapment and obstruction of retroperitoneal tubular structures, notably the ureters, and has been associated with autoimmune disorders. We report a 52-year-old male who was admitted to the emergency department with a seven-day history of lower abdominal pain, anorexia, and unintentional weight loss. Routine blood work revealed the patient to have acute kidney injury, and an unenhanced computed tomography s…

IgG4-Related Retroperitoneal Fibrosis: A Rare Association With Riedel's Thyroiditis.

Idiopathic retroperitoneal fibrosis is a rare fibro-inflammatory disease that can be associated with other IgG4-related diseases (IgG4-RDs). It is exceedingly uncommon to encounter this condition in a patient with Riedel's thyroiditis (RT), another disease in the IgG4-RD family. We present the case of a 53-year-old man with a history of RT who presented for severe localized lower abdominal and suprapubic pain due to obstructive uropathy from extensive retroperitoneal fibrosis. The biopsy of the mass demonstrated fibro-inflammatory tissue, and its immunohistochemistry was notable for IgG4-posit…

Renal Involvement in Retroperitoneal Fibrosis: Prevalence, Impact and Management Challenges.

Retroperitoneal fibrosis (RPF) is a rare disorder consisting of idiopathic and various secondary forms and characterized by chronic inflammatory infiltrates and marked fibrosis in the retroperitoneal space. In idiopathic RPF (IRPF), 35-60% of cases have been reported to be IgG4-related RPF, the retroperitoneal lesions of IgG4-related disease (IgG4-RD). IRPF can frequently lead to renal insufficiency mediated by urinary tract obstruction and hydronephrosis irrespective of being IgG4-related or not. Clinical pictures, laboratory and imaging findings, and location of the urinary tract obstruction…

Retroperitoneal Fibrosis After Spinal Surgery: A Report of 2 Cases.

CASE: Retroperitoneal fibrosis (RPF) is a rare chronic fibroinflammatory disorder with typically unknown etiology (i.e., idiopathic). However, several causes have been identified, including retroperitoneal injury or inflammation. In this study, we describe 2 patients who developed RPF complicated by hydroureteronephrosis after (combined) anterior lower interbody fusion (ALIF) and posterior spine fusion. We also reviewed 6 additional cases from the literature of suspected RPF development after spinal surgery. In these cumulative 8 cases, ALIF was the common denominator. CONCLUSION: RPF may dev…

IgG4-related retroperitoneal fibrosis: a rare cause of obstructive uropathy.

Immunoglobulin G4 (IgG4)-related disease was described in 2003 and is known to affect almost all organ systems. Rarely, this disease can manifest in the retroperitoneum, which can lead to sequalae such as obstructive uropathy. Here we describe a case of IgG4-related retroperitoneal fibrosis leading to obstructive uropathy. The patient was managed with bilateral Double J (DJ) stenting and oral steroids. Following remission of the disease, the DJ stents were removed and the patient remained asymptomatic over the follow-up period. A high index of suspicion is required for diagnosis of this rare d…

Retroperitoneal fibrosis masquerading as pelvi-ureteric junction obstruction - Laparoscopic management.

INTRODUCTION: Retroperitoneal fibrosis (RPF) is a rare fibro-inflammatory condition which is characterized by development of extensive fibrosis throughout the retroperitoneum. It is classically centred over the anterior surface of the fourth and fifth lumbar vertebrae. It results in entrapment and extrinsic compression of retroperitoneal structures. PRESENTATION OF THE CASE: We present the case of a 69 years old man who was reported to have right pelvi - ureteric junction obstruction on computed tomography, but turned out to have RPF. DISCUSSION: Retroperitoneal fibrosis commonly causes obst…

Retroperitoneal Fibrosis Diagnosed as IgG4-related Disease after 35 Years.

In 1982, we reported a case of retroperitoneal fibrosis (RPF) exhibiting various clinical manifestations. Our current understanding of immunoglobulin G4 (IgG4)-related disease led us to consider it as a possible diagnosis because all of the patient's clinical features could be explained by this disease entity. To confirm our hypothesis, were investigated the histopathological findings of resected specimens that had been stored for 35 years postoperatively. Typical pathological findings together with predominant IgG4+ plasma cell infiltration confirmed a potential diagnosis of IgG4-related RPF.…

Idiopathic retroperitoneal fibrosis: a clinicopathological study in 24 Spanish cases.

Idiopathic retroperitoneal fibrosis (IRPF) is a rare condition of unknown aetiology characterized by chronic non-specific inflammation of the retroperitoneum. The aim of this study is to describe, for the first time, the features of Spanish patients with IRPF. In this retrospective study, a clinical description was performed to examine the histopathological features, radiologic findings, laboratory data and treatment of a cohort of 24 IRPF Spanish patients who were admitted to Vall d'Hebron Hospital in Barcelona (Spain) between 1982 and 2009. Patients with secondary retroperitoneal fibrosis we…
