How Percutaneous Balloon Compression Is Performed Today: Technical Adaptations for Trigeminal Neuralgia in the Absence of Commercial Kits.

Percutaneous balloon compression is an established treatment of medically refractory trigeminal neuralgia and remains a commonly performed percutaneous procedure for patients requiring immediate pain relief. Historically, the procedure was standardized through dedicated commercial instrument kits designed for trigeminal balloon rhizotomy, facilitating technical reproducibility across centers. However, the withdrawal of the Food and Drug Administration-approved Mullan percutaneous trigeminal ganglion microcompression set from the medical supplies market in 2016 created a supply gap that require…

Identifying pediatric neurosurgery training priorities in Paraguay: a novel quadrant analysis.

BACKGROUND: The landscape of pediatric neurosurgical training in Paraguay is largely unknown, partly due to the absence of pediatric fellows … itoneal ventricular shunts, endoscopic lavage, vertebral body tethering (WBT), and selective dorsal rhizotomy (SDR). Finally, in terms of technology, the highest priorities were intraoperative electrocorticography (ECOG), laser ablation, and focused ultrasound (FUS). All respondents were open to more dedicated pediatric exposure as part of their residency t …

Outcomes After Repeat-Percutaneous Balloon Compression for Recurrent Trigeminal Facial Pain.

BACKGROUND AND OBJECTIVES: Surgical techniques for treating neuralgic facial pain, including percutaneous balloon compression (PBC), glycerol rhizotomy, stereotactic radiosurgery, and radiofrequency thermocoagulation, provide significant, albeit temporary, pain relief, often necessitating repeat procedures. Although repeated radiofrequency thermocoagulation can cause masseter weakness, reduced corneal reflex, and deafferentation pain, repeated PBC data are lacking. The aim of this study was to investigate the safety and efficacy of repeated PBC. METHODS: Data were collected retrospectively fr…

Tension pneumocephalus resulting from iatrogenic subarachnoid-pleural fistulae: report of three cases.

BACKGROUND: Symptomatic pneumocephalus may result from a cerebrospinal fluid leak communicating with extradural air. However, it is a rare event after thoracic surgical procedures, and its management and physiology are not widely recognized. METHODS: During the past 2 years, we have identified 3 patients who developed pneumocephalus after thoracotomy for tumor resection. Only 1 patient had a discernible spinal fluid leak identified intraoperatively. Two patients experienced delayed spinal fluid drainage from their chest tubes and subsequently developed profound lethargy, confusion, and focal

Microsurgical DREZotomy for pain due to spinal cord and/or cauda equina injuries: long-term results in a series of 44 patients.

According to the literature estimations, 10-25% of patients with spinal cord and cauda equina injuries eventually develop refractory pain. Due to the fact that most classical neurosurgical methods are considered of little or no efficacy in controlling this type of pain, the authors had recourse to microsurgery in the dorsal root entry zone (DREZ). This article reports on the long-term results of the microsurgical approach to the dorsal root entry zone (DREZotomy) in a series of 44 patients suffering from unbearable neuropathic pain secondary to spine injury. The follow-up ranged from 1 to 20 y

Diffusivity Metrics Three Months After Upfront Gamma Knife Radiosurgery for Trigeminal Neuralgia May Be Correlated with Pain Relief.

BACKGROUND: Gamma Knife radiosurgery (GKRS) is a safe and effective treatment option for trigeminal neuralgia (TN). However, there is no objective, noninvasive tool to identify nonresponders or late responders to GKRS and to facilitate longitudinal patient management. We hypothesized that diffusivity metrics obtained 3 months after GKRS may correlate with response to treatment. METHODS: Sixteen patients with TN treated with GKRS underwent preprocedural and 3-month postprocedural 3-T magnetic resonance imaging of the brain. Diffusion tensor metrics of axial diffusivity, radial diffusivity, and

One Hundred Eleven Percutaneous Balloon Compressions for Trigeminal Neuralgia in a Cohort of 66 Patients with Multiple Sclerosis.

BACKGROUND: Trigeminal neuralgia associated with multiple sclerosis (MS-TN) is comparatively rare and larger series of percutaneous balloon compression (PBC) in such cases are few in the literature. OBJECTIVE: To evaluate the results after PBC for MS-TN with regards to therapeutic effect, side effects, and complications. METHODS: One hundred eleven procedures with PBC performed in 66 cases of MS-TN were analyzed. Therapeutic effect was measured as postoperative time to pain recurrence without medication. All complications were compiled and the sensory function was evaluated in a subgroup of

Familial Idiopathic Cranial Neuropathy in a Chinese Family.

Cranial neuropathy is usually idiopathic and familial cases are uncommon. We describe a family with 5 members with cranial neuropathy over 3 generations. All affected patients were women, indicating an X-linked dominant or an autosomal dominant mode of inheritance. Our cases and a review of the literature suggest that familial idiopathic cranial neuropathy is a rare condition which may be related to autosomal dominant vascular disorders (e.g. vascular tortuosity, sclerosis, elongation or extension), small posterior cranial fossas, anatomical variations of the posterior circulation, hypersensit

Effects of the MR-DTI Characteristics of the Trigeminal Ganglion Target on Radiofrequency Treatment in Patients with Trigeminal Neuralgia: A Retrospective Observational Clinical Study.

BACKGROUND: In the percutaneous treatment of trigeminal neuralgia (TN), the difficulty in accessing the foramen ovale (FO) has been widely recognized. However, the most efficient percutaneous treatment target is the so-called trigeminal ganglion target (TGT). We propose that the TGT in a puncture can be identified by magnetic resonance diffusion tensor imaging (MR-DTI). OBJECTIVES: To observe the effect of the characteristics of the TGT as detected by MR-DTI on percutaneous stereotactic radiofrequency rhizotomy (PSR) in TN patients. METHODS: In our observational study, we preoperatively perf…

Comparison of neuropathic pain and neuronal apoptosis following nerve root or spinal nerve compression.

Altered dorsal root ganglion (DRG) function is associated with neuropathic pain following spinal nerve injury. However, compression of the cauda equina and dorsal rhizotomy proximal to the DRG do not induce significant pain, whereas in the spinal nerve and peripheral nerve, injury distal to the DRG does induce neuropathic pain. Caspase signaling induces apoptosis, and caspase inhibitors prevent pain-related behavior. The degree of DRG neuronal apoptosis is thought to play a role in pain behavior. We suggest that differences in pain behavior according to the injury sites within the DRG may be r…

Upregulation of a silent sodium channel after peripheral, but not central, nerve injury in DRG neurons.

After transection of the central axonal projections (dorsal rhizotomy) of these small DRG neurons, in contrast, the repriming kinetics of TTX-sensitive sodium currents remain similar to those of control (uninjured) neurons. What is the best treatment of drug-resistant trigeminal neuralgia in patients affected by multiple sclerosis? A literature analysis of surgical procedures.

OBJECTIVE: Drug-resistant trigeminal neuralgia (TN) can complicate the clinical course of patients affected by multiple sclerosis (MS). Various surgical procedures have been reported for the treatment of this condition, but there is no agreement on the best management of these patients. To our knowledge, there is no critical literature analysis focusing on this particular topic. The aim of this study was to evaluate the clinical outcome of different surgical procedures utilized for drug-resistant TN in MS patients. METHODS: We reviewed the literature about the studies reporting on surgical tr
