Cost-effectiveness and clinical outcomes comparison between noninvasive ventilation and high-flow nasal cannula use in patients with multiple rib fractures.

BACKGROUND: Patients with multiple rib fractures often require advanced respiratory support to prevent intubation and associated morbidity. Noninvasive ventilation (NIV) and high-flow nasal cannula (HFNC) are commonly used, but direct comparisons of clinical outcomes and cost-effectiveness remain limited. This study aimed to compare NIV versus HFNC using a large, nationwide US database. METHODS: This retrospective cohort study used the 2020-2021 National Inpatient Sample database to identify adult trauma patients (18-89 years) with two or more rib fractures who received either NIV or HFNC wit…

Detection, Classification, and Segmentation of Rib Fractures From CT Data Using Deep Learning Models: A Review of Literature and Pooled Analysis.

PURPOSE: Trauma-induced rib fractures are common injuries. The gold standard for diagnosing rib fractures is computed tomography (CT), but the sensitivity in the acute setting is low, and interpreting CT slices is labor-intensive. This has led to the development of new diagnostic approaches leveraging deep learning (DL) models. This systematic review and pooled analysis aimed to compare the performance of DL models in the detection, segmentation, and classification of rib fractures based on CT scans. MATERIALS AND METHODS: A literature search was performed using various databases for studies …

Respiratory Recovery in a Critically Ill Patient With Rib Fractures Using the Serratus Posterior Superior Intercostal Plane Block: A Case Report.

We present a case of a patient with multiple rib fractures who was managed in the intensive care unit using a serratus posterior superior intercostal plane (SPSIP) block. The SPSIP block is a relatively new interfascial plane block technique that provides broad thoracic dermatomal coverage for analgesia. In trauma patients with multiple rib fractures, pain control, preservation of respiratory function, prevention of pulmonary complications, and facilitation of early mobilization are important. This case suggests that the SPSIP block may represent a safe, effective, and opioid-sparing analgesic…

The application of electrical impedance tomography and surgical outcomes of thoracoscope-assisted surgical stabilization of rib fractures in severe chest trauma.

Serious blunt chest trauma usually induces hemothorax, pneumothorax, and rib fractures. More studies have claimed that early video-assisted thoracoscopic surgery with surgical stabilization of rib fractures (SSRF) results in a good prognosis in patients with major trauma. This study aimed to verify the outcomes in patients with chest trauma whether SSRF was performed. Consecutive patients who were treated in a medical center in Taiwan, for traumatic events between January 2015 and June 2020, were retrospectively reviewed. This study focused on patients with major trauma and thoracic injuries, …

No benefit from the addition of low-dose ketamine infusion to standard evidence-based care of patients with multiple rib fractures.

BACKGROUND: Multiple rib fractures from blunt thoracic trauma cause significant morbidity. Optimal current management includes multimodal analgesia, pulmonary hygiene, and early mobilization. Low-dose ketamine infusion (LDKI) has been proposed as an adjunctive analgesic in this setting. A prior study reported decreased pain scores with LDKI in patients with multiple rib fractures. We hypothesized that LDKI would decrease morphine milligram equivalents (MMEs) in patients with multiple rib fractures. METHODS: A prospective randomized placebo-controlled trial was performed in adult (18 years or …

Does surgical fixation improve pain and quality of life in patients with non-flail rib fractures? A best evidence topic review.

A best evidence topic in thoracic surgery was written according to a structured protocol. The question addressed was 'does surgical stabilization of rib fractures improve pain and quality of life in patients with non-flail rib fractures?'. Altogether >300 papers were found using the reported search, of which 6 represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers were tabulated. Whilst several non-randomized cohort studies demonstrate superior pain an…

Acute Diaphragmatic Injuries Associated with Traumatic Rib Fractures: Experiences of a Major Trauma Centre and the Importance of Intra-Pleural Assessment.

BACKGROUND: Diaphragmatic injuries following blunt or penetrating thoraco-abdominal trauma are rare, but can be life-threatening. Rib fractures are the most common associated injury in patients with a traumatic diaphragmatic injury (TDI). We hypothesized that the pattern of rib fracture injuries could dictate the likelihood of acute TDIs. METHODS: A retrospective study was carried out between April 2014 and October 2018 to analyze patients with TDIs and rib fractures at a major trauma center in London, United Kingdom. RESULTS: Over the study period, 1,560 patients had rib fractures, of whom …

A randomized clinical trial of single dose liposomal bupivacaine versus indwelling analgesic catheter in patients undergoing surgical stabilization of rib fractures.

INTRODUCTION: Locoregional analgesia (LRA) remains underused in patients with chest wall injuries. Surgical stabilization of rib fractures (SSRF) offers an opportunity to deliver surgeon-directed LRA under direct visualization at the site of surgical intervention. We hypothesized that a single-dose liposomal bupivacaine (LB) intercostal nerve block provides comparable analgesia to an indwelling, peripheral nerve plane analgesic catheter with continuous bupivacaine infusion (IC), each placed during SSRF. METHODS: Noninferiority, single-center, randomized clinical trial (2017-2020) was performe…

Prospective evaluation of epidural and intravenous administration of fentanyl for pain control and restoration of ventilatory function following multiple rib fractures.

To evaluate the efficacy of opiate administration regimens, 32 patients with multiple rib fractures were prospectively randomized to receive either continuous epidural (ED) or continuous intravenous (IV) infusions of fentanyl. Dosage was titrated to individual subjective pain relief. Ventilatory function tests (VFTs), arterial blood gases (ABGs), and visual analog scores were obtained before and after the institution of analgesia. Post-analgesia values were compared with pre-analgesia values using a two-tailed paired t-test looking for significant changes produced by the analgesic method. Both…

Breathe Easier: Evaluating Serratus Anterior Plane Block for Rib Fracture Analgesia-A Systematic Review and Meta-analysis.

OBJECTIVE: Rib fractures may cause severe pain that impairs ventilation, increases pulmonary complications, and lengthens hospitalization. Conventional analgesic techniques have several limitations, whereas the serratus anterior plane block (SAPB) offers a simpler, ultrasound-guided regional anesthesia alternative. This systematic review and meta-analysis examined whether SAPB improves pain control and reduces opioid use and hospitalization duration. MATERIAL AND METHODS: A systematic review and meta-analysis were performed in accordance with Preferred Reporting Items for Systematic Reviews a…

Performance of thoracic ultrasonography compared with chest radiography for the detection of rib fractures using computed tomography as a reference standard.

OBJECTIVE: Although there is growing evidence that ultrasonography is superior to X-ray for rib fractures' detection, X-ray is still indicated as the most appropriate method. This has partially been attributed to a lack of studies using an appropriate reference modality. We aimed to compare the diagnostic accuracy of ultrasonography and X-ray in the detection of rib fractures, considering CT as the reference standard. MATERIALS AND METHODS: Within a 2.5-year period, all consecutive patients with clinically suspected rib fracture(s) following blunt chest trauma and available posteroanterior/an…

Thoracic Epidural as a Rescue Analgesic in a Patient with a Continuous Erector Spinae Plane Block for Rib Fractures: A Case Report.

BACKGROUND Rib fractures are a common traumatic injury with significant morbidity and mortality resulting from respiratory compromise. Regional anesthetic techniques have demonstrated efficacy in reducing morbidity and mortality from rib fractures, but there is limited evidence comparing various techniques, and in complex trauma patients various factors may preclude the use of neuraxial and other techniques. CASE REPORT We report the case of a 72-year-old man who presented with left 4th-11th rib fractures. He was initially managed with a continuous erector spinae plane catheter, which resulted…
