Fluid resuscitation after an otherwise fatal hemorrhage: I. Crystalloid solutions.

One half of deaths among trauma victims occur within 1 hour of injury and are due to rapid hemorrhage or CNS trauma. We developed a rapid hemorrhage model in unanesthetized swine to simulate human exsanguination. We compared the ability of four crystalloid solutions to prevent death after an otherwise fatal hemorrhage: normal saline (NS), Ringer's lactate (RL), Plasmalyte-A (PA), and Plasmalyte-R (PR). Five days before hemorrhage swine received an aortic sideport and a central venous treatment catheter. Aortic blood (54 ml/kg) was removed in 15 minutes from 116 swine. The percentages of shed b

Maternal and fetal effects of prophylactic hydration with crystalloids or colloids before epidural anesthesia.

Prophylactic intravenous hydration decreases the incidence and severity of hypotension due to obstetric epidural anesthesia. This study assesses whether infusion of normal serum albumin (NSA) offers any advantages over Ringer's lactate (RL) solution. Sixty patients scheduled for elective cesarean sections were divided into three equal groups. Group 1 received 1200 ml of RL; group 2, 700 ml of RL plus 500 ml of 5% NSA; and group 3, 1000 ml of RL plus 200 ml of 25% NSA. In group 1, the concentrations of serum albumin (SA) decreased from 3.5 +/- 0.3 (mean +/- SD) to 3 +/- 0.2 g/dL (P less than 0.

Effects of cations and pH on apical membrane potential of in vitro Necturus antrum.

The surface epithelial cells of the gastric mucosa transport Na+ from lumen to serosa. The first step in this process is the entry of Na+ through an amiloride-sensitive Na+ channel at the apical membrane. The exact function(s) of this Na+ transport are unknown, but it has been suggested that it might help the stomach to withstand an acid load. The present study was undertaken to examine the effects of low luminal pH and the alkali cations Li+, K+, Rb+, and Cs+ on the amiloride-sensitive Na+ permeability of the apical membrane of Necturus antral cells by measuring the changes in apical membrane

Acid-base effects on electrolyte transport in CA II-deficient mouse colon.

To determine the role of carbonic anhydrase (CA) in colonic electrolyte transport, we studied Car-2(0) mice, mutants deficient in cytosolic CA II. Ion fluxes were measured under short-circuit conditions in an Ussing chamber. CA was analyzed by assay and Western blots. In Car-2(0) mouse colonic mucosa, total CA activity was reduced 80% and cytosolic CA I and membrane-bound CA IV activities were not increased. Western blots confirmed the absence of CA II in Car-2(0) mice. Normal mouse distal colon exhibited net Na(+) and Cl(-) absorption, a serosa-positive PD, and was specifically sensitive to p

Gastric emptying in humans: influence of different regimens of parenteral nutrition.

Intravenous administration of nutrients can suppress oral food intake. Inhibition of gastric emptying (GE) is a potential explanation for this process. Inhibition of GE during parenteral nutrition (PN) and attenuation of this by parenteral nutrition enriched with branched-chain amino acids (BCAAs) was examined in nine healthy males maintained on standard liquid diets for 6 d before each of three GE studies. GE was measured by scintigraphy after ingestion of a liquid test meal, at weekly intervals, after a 6-h infusion of Ringer lactate solution (RL), peripheral PN, or PN with half the amino ac

Hypertonic resuscitation: physiologic mechanisms and recommendations for trauma care.

Hypertonic saline solutions, with or without added colloid, have received extensive evaluation as volume expanders in both animal studies and clinical trials. Most studies have used 7.5% NaCl/6% dextran 70 (HSD). HSD's primary mechanism of action is rapid osmotic mobilization of cellular water into the blood volume. The measured volume expansion efficiency of HSD is equal to 10 times that of lactated Ringer's solution. Part of HSD's effectiveness is because of the hyperosmotic vasodilation of both systemic and pulmonary vessels. Increased cardiac effectiveness occurs because of the combination…
