Predictors of cardiac arrhythmic events in non coronary artery disease patients.

Arrhythmic sudden cardiac death (SCD) represents a major worldwide public health problem accounting for 15-20% of deaths. Risk stratification to identify patients at risk of SCD is crucial in order to implement preventive measures in the general population. Several biomarkers have been tested exploring different pathophysiological mechanisms of cardiac conditions. Conflicting results have been described limiting so far their use in clinical practice. The use of new biomarkers such as microRNAs and sex hormones and the emerging role of genetic on risk prediction of SCD is a current research top

Regulation of LH secretion by RFRP-3 - From the hypothalamus to the pituitary.

RFamide-related peptides (RFRPs) have long been identified as inhibitors of the hypothalamus-pituitary-gonad axis in mammals. However, less progress has been made in the detailed roles of RFRPs in the control of LH secretion. Recent studies have suggested that RFRP-3 neurons in the hypothalamus can regulate the secretion of LH at different levels, including kisspeptin neurons, GnRH neurons, and the pituitary. Additionally, conflicting results regarding the effects of RFRP-3 on these levels exist. In this review, we collect the latest evidence related to the effects of RFRP-3 neurons in regulat

Gender in the consulting room.

Gender influences psychoanalysis and psychotherapy in a variety of ways. This article discusses these with reference to the woman therapist and analyst. Choice of therapist is influenced by realistic, transferential, and stereotyped ideas such as (1), wishes for a role model, (2) unconscious fantasies for a better mother, and (3) ideas that women are more nuturent. Family and work conflicts, pregnancy, and lifestyle issues are special issues for the woman professional. Transference and coutertransference manifestations concern variations in erotic and eroticized transference, maternal and pate

Multideterminism in asthmatic disease.

A discussion of "The Role of Primal Scene and Masochism in Asthma," by Cecilia Karol, M.D. Dr. Karol has described a relationship between primal scene experience, sadomasochism, and asthma. While her clinical material strongly supports the relationship that she is attempting to prove, other researchers have found different conflicts that appear equally convincing in relationship to asthma. The following discussion attempts to correlate these differing perspectives, along with data from the fields of allergy, immunology, and animal research, in an attempt to provide a more inclusive explanation

Myofascial pain and fibromyalgia: two different but overlapping disorders.

There is good evidence supporting that people with fibromyalgia syndrome (FMS) exhibit central sensitization. The role of peripheral nociception is under debate in FMS. It seems that widespread pain experienced in FMS is considered multiple regional pains; therefore, several authors proposed that muscles play a relevant role in FMS. Trigger points (TrPs) have long been a contentious issue in relation to FMS. Preliminary evidence reported that the overall spontaneous pain is reproduced by referred pain from active TrPs, suggesting that FMS pain is largely composed of pain arising, at least part

Towards an understanding of the role of the 'magnocellular advantage' in fluent reading.

Reading disability is a relatively common developmental disorder, the aetiology of which is clouded by conflicting theoretical approaches and the heterogeneity of the subtypes found. Recent advances in understanding of the visual system have revived interest in the role of visual processing in the persisting inability to read fluently that characterises dyslexia. A new integrated model of visual processing based on primate single cell and human electrophysiology may provide such a framework, implicating the magnocellular pathway's role in activating and driving attentional mechanisms in higher

[Function of nonspecific brain systems in mild war cranio-cerebral trauma].

129 participants of military conflicts were examined; 72 of them had a mild combat craniocerebral trauma (CCT) (8.46 +/- 0.72 years before the observation on the average); 57 hadn't any trauma; 15 healthy individuals formed a control group. The examination included an analysis of the indices of autonomic background, cardiointervalograms, EEG, EMG, visual and acoustic evoked potentials as well as peculiarities of personality according to the modified MMPI test. The results confirmed a role of chronic combat stress in the development of the dysfunction of non-specific systems of the brain in the

Nitric oxide and inflammatory bowel diseases.

Nitric oxide (NO) production, as detectable by indirect and direct methods, as well as the expression of inducible nitric oxide synthase (iNOS) in the intestinal mucosa appear to be enhanced in active ulcerative colitis and, when in excess, to play a proinflammatory role in the disease. Despite some conflicting data, there is evidence that NO production is also increased in Crohn's disease. Many inflammatory features of inflammatory bowel disease are in keeping with the physiological properties of NO, and toxic megacolon, a complication of chronic colitis characterized by acute colonic dilatat

The role of botulinum toxin in multimodal treatment of spasticity in ambulatory children with spastic Cerebral Palsy: extensive evaluation of a cost-effectiveness trial.

BACKGROUND: A cost-effectiveness trial (the Space Bop study) on the added value of botulinum toxin injections (BoNT-A) in the leg muscles, as part of a multimodal intervention for ambulatory children with spastic cerebral palsy in the context of a single distinct cycle of care was performed recently by our group. For a broad set of effect outcomes, we found that BoNT-A had no added value if children received comprehensive rehabilitation. However, this counterintuitive finding was met with scepticism. OBJECTIVE: Since several noteworthy facts and experiences were recorded during the course of

Conflict, collusion or collaboration in consultations about medically unexplained symptoms: the need for a curriculum of medical explanation.

OBJECTIVE: To identify the basis of the communication problems that characterise consultations about medically unexplained symptoms (MUS) and to identify implications for clinical education. METHOD: Recent research into the details of clinical communication about MUS was reviewed narratively and critically, and broader research literature was scrutinised from the perspective of a practitioner who wishes to provide patients with explanations for such symptoms. RESULTS: Consultations about MUS often involve contest between patients' authority, resting on their knowledge of their symptoms, and

Conservation Practitioners' Perspectives on Management Responses to Poaching in South African Protected Areas.

Biodiversity across Africa faces escalating threats from agricultural expansion, urbanisation, resource overexploitation, and pollution, particularly at the margins of protected areas (PAs). These land‑use changes intensify interactions between people and wildlife by compressing habitats, increasing competition for space and resources, and disrupting ecological processes, thereby exacerbating human-wildlife conflict and threatening conservation outcomes. In this context, conservation practitioners play an important role in managing these conflicts by implementing conservation management interv

Improving pathway prediction accuracy of constraints-based metabolic network models by treating enzymes as microcompartments.

Metabolic network models have become increasingly precise and accurate as the most widespread and practical digital representations of living cells. The prediction functions were significantly expanded by integrating cellular resources and abiotic constraints in recent years. However, if unreasonable modeling methods were adopted due to a lack of consideration of biological knowledge, the conflicts between stoichiometric and other constraints, such as thermodynamic feasibility and enzyme resource availability, would lead to distorted predictions. In this work, we investigated a prediction anom

Simulations of monomeric amyloid β-peptide (1-40) with varying solution conditions and oxidation state of Met35: implications for aggregation.

The amyloid β-peptide (Aβ) is a 40-42 residue peptide that is the principal toxic species in Alzheimer's disease (AD). The oxidation of methionine-35 (Met35) to the sulfoxide form (Met35(ox)) has been identified as potential modulator of Aβ aggregation. The role Met35(ox) plays in Aβ neurotoxicity differs among experimental studies, which may be due to inconsistent solution conditions (pH, buffer, temperature). We applied atomistic molecular dynamics (MD) simulations as a means to probe the dynamics of the monomeric 40-residue alloform of Aβ (Aβ40) containing Met35 or Met35(ox) in an effort to

Ambulatory blood pressure and heart rate in paramedics: effects of cynical hostility and defensiveness.

Ambulatory blood pressure and heart rate responses were obtained in 33 male paramedics during a 24-hour work shift to examine the effects of episodes of occupational stress on cardiovascular reactivity and subjective reports of stress. The aim of this study was to determine how individual differences in cynical hostility and defensiveness interacted with situational demands to affect cardiovascular responses in a natural setting. Defensiveness was found to interact significantly with cynical hostility in predicting subjects' heart rate responses in different work contexts. Specifically, in a h

Initial management of haemorrhagic war casualties: tactical priorities and innovative approaches in modern and future warfare.

BACKGROUND: Haemorrhage remains the leading cause of preventable death in modern armed conflict, affecting both combatants and civilians. Recent conflicts-particularly the ongoing conflict in Ukraine- have highlighted the increasing complexity of battlefield injuries, characterised by hybrid warfare, disrupted evacuation chains, and delayed access to definitive surgical care. These realities challenge traditional trauma paradigms, such as the "Golden Hour" and demand adaptation of haemorrhage control and resuscitation strategies to austere environment. CONTENT: This narrative review synthesiz

The RNA-RNA interactome between a phage and its satellite virus reveals a small RNA that differentially regulates gene expression across both genomes.

Satellite viruses are present across all domains of life, defined as subviral parasites that require infection by another virus for satellite progeny production. Phage satellites exhibit various regulatory mechanisms to manipulate phage gene expression to the benefit of the satellite, redirecting resources from the phage to the satellite, and often inhibiting phage progeny production. While small RNAs (sRNAs) are well documented as regulators of prokaryotic gene expression, they have not been shown to play a regulatory role in satellite-phage conflicts. Vibrio cholerae encodes the phage induci
