Biomechanics of the rotator cuff: European perspective.

The rotator cuff has an important role in the stability and function of the glenohumeral joint. To understand the biomechanical proprieties of the rotator cuff, it is essential to understand the pathogenesis and effects of rotator cuff tears. The rotator cuff provides a stabilizing effect to the shoulder, because of compression of the humeral head against the glenoid cavity. The wide range of motion of the shoulder is allowed by the variety of rotational moments of the cuff muscles. Rotator cuff muscles action must be precisely coordinated to obtain the desired movement. Rotator cuff tendons a

Influence of psychomotor skills and innervation patterns on results of latissimus dorsi tendon transfer for irreparable rotator cuff tears.

This investigation was performed to analyze the influence of innervation and psychomotor skills on the outcome of latissimus dorsi transfer. Patients with the 10 best and 10 worst results after latissimus dorsi transfer for irreparable rotator cuff tears were selected. All patients meeting the inclusion criteria (n = 12) were subject to a psychomotor test battery (Motorische Leistungsserie) and electromyographic innervation assessment. There was no statistical difference between the 2 groups preoperatively in terms of the commonly tested factors known to influence the results of this procedure

Post-traumatic glenohumeral cartilage lesions: a systematic review.

BACKGROUND: Any cartilage damage to the glenohumeral joint should be avoided, as these damages may result in osteoarthritis of the shoulder. To understand the pathomechanism leading to shoulder cartilage damage, we conducted a systematic review on the subject of articular cartilage lesions caused by traumas where non impression fracture of the subchondral bone is present. METHODS: PubMed (MEDLINE), ScienceDirect (EMBASE, BIOBASE, BIOSIS Previews) and the COCHRANE database of systematic reviews were systematically scanned using a defined search strategy to identify relevant articles in this fi

[Current operative strategies for rotator cuff tears in German hospitals].

The surgical treatment of rotator cuff lesions is still a controversial subject. We therefore decided to perform a study designed to yield an overview of the treatment modalities currently applied in Germany. A nationwide questionnaire survey was sent to 470 orthopaedic and trauma departments to ask about their surgical treatment of rotator cuff lesions: frequency of operative treatment, methods used in diagnosis and types of operative treatment; particular attention was paid to the application of minimally invasive techniques and to postoperative rehabilitation. The response rate was 55%. In

The association of suprascapular neuropathy with massive rotator cuff tears: a preliminary report.

We studied a prospective, consecutive series of 8 patients presenting with massive rotator cuff tears (>5 cm. in maximum dimension), all associated with severe retraction and fatty infiltration of the supraspinatus muscle on magnetic resonance imaging studies. All 8 patients had suprascapular neuropathy shown by electromyography (EMG) findings of denervation in the supraspinatus and/or infraspinatus muscles. Clinically, all patients had severe limitation of active motion, with no patient able to elevate their affected arm actively >40 degrees . Four patients elected débridement and partial sur

Rotator cuff disease.

Rotator cuff disease is a complex condition. It includes a variety of pathologies, ranging from tendinosis to complete tear. The cause seems to be multifactorial and is most likely a combination of major trauma, microtrauma from daily wear and tear, and age related degeneration. When establishing a program for the patient with rotator cuff dysfunction, the rehabilitation specialist should consider biomechanical factors, the patient's functional goals, and issues beyond the shoulder that may affect the course of the disease.

[Shoulder arthroscopy in degenerative and inflammatory diseases].

A new concept of shoulder arthroscopy is presented based on experience with 800 shoulder arthroscopies. All open surgical procedure on the shoulder are preceded by arthroscopy. The procedure is performed with the patient lying on his or her side. The medium is CO2, and the advantages are less bleeding, a natural picture, no fluid edema. After diagnostic arthroscopy (with CO2), operative arthroscopy (with fluid) or open joint surgery follows. The surgery can easily be performed in the same position for good access with any approach. Preliminary diagnostic arthroscopy helps to plan the procedure

[Computed tomography of the rotator cuff--a comparison with other imaging technics].

We consider CT to be suitable for investigating the following lesions of the rotator cuff: 1) tendopathia calcarea. 2) Rotator cuff atrophy suitable for surgery. 3) Bone defects. 4) Inflammatory changes with effusions. For demonstrating rotator cuff defects that might require surgery, we recommend CT arthrography, if possible with sagittal sections. This is particularly the case for younger patients with a history of trauma and evidence of a complex shoulder injury and in cases with persistent symptoms after surgical intervention of ineffective conservative treatment. CT, however, is only indi

The effect of rotator cuff tears on surgical outcomes after type II superior labrum anterior posterior tears in patients younger than 50 years.

BACKGROUND: Rotator cuff injury in the setting of type II superior labrum anterior posterior lesions is a common finding. Although predictable surgical outcomes can be expected after type II superior labrum anterior posterior repair, the effect of rotator cuff tears on surgical outcome is unknown. HYPOTHESIS: Rotator cuff tears will not negatively affect surgical outcome of type II superior labrum anterior posterior repairs. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: The study group included 93 patients younger than 50 years who underwent arthroscopic type II superior labrum

Association Between Hormone Replacement Therapy and Incidence of Tendon Injuries and Surgical Repair in Perimenopausal Women: A Propensity Score-Matched National Cohort Study.

BACKGROUND: Tendinopathies are common in middle-aged women. While estrogen regulates tendon remodeling, the influence of systemic hormone replacement therapy (HRT) on the risk of tendon degeneration remains unclear. HYPOTHESIS/PURPOSE: The purpose of this study was to investigate the association between systemic HRT use and tendon injuries/repair in perimenopausal women. It was hypothesized that systemic HRT would be associated with an increased incidence of tendon injuries and repair. STUDY DESIGN: Cohort study; Level of evidence, 4. [Management of isolated subscapularis tendon tears].

Isolated subscapularis tendon tears are rare and often of traumatic origin. Despite specific clinical tests and exact radiological tools (ultrasound, magnetic resonance imaging), these tendon ruptures are often overlooked. For restoration of normal biomechanical joint function, immediate operative reconstruction is recommended. Tears of the upper subscapularis and partial articular-side tendon can be safely managed arthroscopically. The biceps tendon is often involved, and adequate treatment with tenotomy or tenodesis must be given to prevent postoperative pain. Complete subscapularis tendon a

Fibrous cartilage in the rotator cuff: A pathogenetic mechanism of tendon tear?

There is no consensus on the pathogenesis of rotator cuff tears. Fibrous cartilage has been hypothesized to develop in some tendons as a result of shear or compressive forces, resulting in a tissue less capable of resisting normal tensile load and more prone to tearing. To test the hypothesis that metaplastic fibrocartilage in the rotator cuff could be involved in the pathogenic mechanism of its tear, samples from 34 acute and chronic torn rotator cuffs were subjected to histologic and immunohistologic study for the presence and type of cartilage (hyaline, fibrous, or elastic) in the area of t

[Rotator cuff tears: diagnosis and therapy].

Since the life span of our society is increasing and the expectation of high functional demands is growing more and more older people take part in sports activities emphasises the necessity of early diagnosis of rotator cuff lesions and their correct treatment. The goal of any diagnostic means should be a proper treatment of rotator cuff lesions according to the detected pathology. New arthroscopic techniques to surgically reconstruct rotator cuff tears have brought the operative repair techniques another step further. In cases of large and complex cuff tears the surgeon can individually selec

[The traumatized shoulder in computerized and magnetic resonance tomography].

Diagnosis of traumatic changes of the shoulder can be achieved by several invasive and nonivasive radiological procedures allowing visualization of bony and/or soft tissue structures. With reference to 74 shoulder examinations CT, CT-arthrography, and MRI were reviewed and their value for further treatment assessed. Since joint effusions occurred in nearly all cases of glenoid and rotator cuff lesions, a cost-effective strategy for diagnosing traumatic changes of the shoulder is presented.
