Effectiveness of external rotation immobilization in highly active young men with traumatic primary anterior shoulder dislocation or subluxation.

We treated 15 highly active young men (16 shoulders) with traumatic primary anterior shoulder dislocation or subluxation using 3-week external rotation immobilization. Fourteen patients (14 shoulders) were members of the Self-Defense Force and the other patient (2 shoulders) was a high school student who played club-level rugby. Average patient age at the time of the primary injury was 21.3 years (range, 17-26 years). Magnetic resonance imaging (MRI) was performed on 14 of 16 shoulders after the 3-week external rotation immobilization and showed that the anteroinferior labrum was reduced on th…

Incidence and etiology of lumbar spondylolysis: review of the literature.

BACKGROUND: Lumbar spondylolysis is a defect of the pars interarticularis known to occur as a stress fracture. Its incidence varies consider … eport that the incidence in higher in male subjects than in female subjects. We found that Japanese rugby and judo players were prone to suffer lumbar spondylolysis, at an incidence of about 20%. However, the incidence for Japanese professional soccer and baseball players was much higher, at 30%, which was more than five times the incidence in the gener …

Management guidelines for participation in collision activities with congenital, developmental, or post-injury lesions involving the cervical spine.

Conditions involving the cervical spine in the athlete requiring management decisions are numerous. The purpose of this paper is to present appropriate guidelines for return to collision activities in those with congenital, developmental, or post-injury lesions. Collision activities include boxing, football, ice hockey, lacrosse, rugby and wrestling. Information compiled from over 1200 cervical spine lesions documented by the National Football Head and Neck Injury Registry, an extensive literature review, as well as an understanding of injury mechanisms have resulted in reasonable management g…

[Traumatic middle cerebral artery occlusion].

A 16-year-old boy developed motor aphasia and right hemiplegia after head trauma caused during Rugby football club activity. About 2 hours after trauma, these neurological deficits improved dramatically. CT scan on admission revealed no abnormal finding. But, on the next day, CT scan revealed an irregular low density area at the left basal ganglia. Cerebral angiography showed a slight narrowing at the left carotid siphon, severe stenosis at M1 portion of the left middle cerebral artery with vasospasm of its distal branches and occlusion of the left anterior temporal artery. The extracranial po…

High tackle headache: implications of referee agreement for tackle height law change.

OBJECTIVES: Rugby Union has a relatively high risk of injury. Early evidence suggests a benefit of lowering tackle height to reduce head and neck injuries, although concerns persist among stakeholders regarding implementation challenges. This study aimed to understand whether referees can reach the same conclusion regarding tackle height in a controlled environment (ie, video) and whether priming influenced these decisions. METHODS: Forty-eight active referees completed a questionnaire based on high-tackle decision-making guidelines after watching tackles. Participants were randomly assigned …

Mild traumatic brain injury (mTBI or concussion) is receiving increased attention due to the incidence in contact sports and limitations wit … cient (MFCC) features from audio recordings of speech that were collected from athletes engaging in rugby union who were diagnosed with an mTBI or not. These features were trained on our novel particle swarm optimised (PSO) bidirectional long short-term memory attention (Bi-LSTM-A) deep learning model. Two-year follow-up after operative treatment of an osseous Bankart lesion with a flap-detached cartilage lesion of the glenoid: A case report.

Glenoid articular cartilage lesion is a rare complication following traumatic anterior dislocation of the shoulder. We report the case of a 14-year-old male rugby player with traumatic anterior shoulder instability, an extensively flapped lesion on the glenoid articular cartilage, and an osseous Bankart lesion. Arthroscopic findings revealed that the glenoid cartilage was flap-detached, extending from the anteroinferior to the center. Repair of the osseous Bankart lesion using suture anchors and resection of the unstable peripheral part of the cartilage was performed arthroscopically. The main…

Blowing the Whistle on Concussion Knowledge and Education in Youth Sport Referees.

PURPOSE: To gain an understanding of referee concussion knowledge, what sporting organizations were doing to prepare their referees to recognize concussions in youth sport, and what role referees could play in overall concussion safety. An International Exploration of the Effect of Media Portrayals of Postconcussion Management on Concussion Identification in the General Public.

BACKGROUND: Concussion portrayal in media broadcasts of sporting events may contribute to lack of public understanding regarding concussion. METHODS: In total, 828 participants from Australia, New Zealand, and the United Kingdom completed a questionnaire assessing concussion knowledge. Participants were randomly assigned to either receive sports return-to-play (RTP) guideline information (RTP group) or not (no-RTP group). Participants viewed 12 short clips from televised rugby games and indicated whether they believed the player in each clip had sustained a concussion. Participants were then …

Validating Tackle Mechanics in American Football: Improving Safety and Performance.

Research has helped to understand the risks of injuries of tackling in American football and rugby; however, approaches to teaching and analysis are not well-documented. Shoulder-led tackling has been proposed as a safer approach to tackling even though data on the effectiveness for safety and defensive performance is limited. Additionally, some have argued that safety and effectiveness are incompatible. The purpose of the study was to validate a specific sequence of tackling actions as a tool for teaching safer and more effective tackling skills. Results suggested tackle scores help predict p…

Concussion in Sports.

Concussion, a peculiar type of mild traumatic brain injury (mTBI), is an injury frequently encountered in various contact and noncontact sports, such as boxing, martial arts, American football, rugby, soccer, ice hockey, horse riding, and alpine skiing. Concussion occurs anytime external forces of specific intensities provoke acceleration-deceleration of the brain, and it is characterized by the rapid onset of short-lived impairment of neurologic functions, spontaneously resolving within weeks, persisting for longer times only in a small percentage of cases. A wide range of molecular alteratio…
