Paediatric Patella Tendon Rupture: A Systematic Review.

The patella tendon is an important part of the extensor mechanism, and ruptures can lead to significant impairment. The condition is rare in children and adolescents, and as a result, the paediatric literature is limited. Injury patterns may differ from those seen in adults. This systematic review therefore aims to evaluate the presentation, diagnosis, management and outcomes of patella tendon ruptures in children and adolescents. A literature search was performed using PubMed, Medline and Embase to identify studies based on the Preferred Reporting Items for Systematic Reviews and Meta-analyse…

Endovascular Management of Aorto-iliac Bifurcation Rupture.

Iatrogenic rupture at the aorto-iliac bifurcation is a rare but important life-threatening complication of endovascular intervention with limited guidance in the literature. This case report includes two cases of intra-procedural aorto-iliac bifurcation rupture managed emergently with temporary balloon occlusion and covered kissing stent placement. Both cases highlight the source of the complication and viable initial strategies for achieving temporary hemostasis while emphasizing the need for rapid recognition, appropriate equipment, and close collaboration with vascular surgery.

Open Approach of Ascending Aorta Septic Rupture.

BACKGROUND: Septic pseudoaneurysms of the native ascending aorta are rare and challenging to treat, especially in elderly, comorbid patients with septic conditions and unfavorable anatomy for endovascular repair. CASE SUMMARY: A 78-year-old man with prior tricuspid valve replacement for endocarditis presented with methicillin-sensitive Staphylococcus aureus bacteremia. Imaging showed a large ascending aorta pseudoaneurysm, likely at the previous cardioplegia site. He underwent successful surgical repair via direct bovine pericardial patch closure under normothermic cardiopulmonary bypass. Rec

Rupture of Acute Triple-Barreled Aortic Dissection.

We experienced a case of rupture of acute triple-barreled aortic dissection (AD), a rare form of dissection characterized by the presence of two distinct false lumens in addition to the true lumen, involving a dissecting aneurysm of the descending thoracic aorta (DTA). A 71-year-old woman presented with increasing back pain. She had a history of ascending aortic replacement for type-A acute AD, and there was a residual AD from the aortic arch to the bilateral common iliac arteries. An increasing diameter of the DTA had been observed on follow-up CT scans. At the emergency department, her radia…

Spontaneous rupture of renal calyx in pregnancy.

A primigravida in her 20s presented to the emergency department at 30 weeks gestation with acute onset, sharp right lower abdominal pain with associated nausea and emesis. Symptoms were refractory to intravenous antiemetics and multimodal pain regimen. Laboratory work-up was unremarkable. Abdominal ultrasound showed mild right hydronephrosis with perinephric oedema and complex fluid collection. Follow-up MRI revealed a large fluid collection in the right retroperitoneum, consistent with a right renal forniceal rupture. A right double-J ureteral stent was placed with resolution of symptoms. The…

[Intraoperative left ventricular rupture in cardiac surgery].

Intraoperative left ventricular rupture is a rare but extremely dangerous complication in cardiac surgery. This event can lead to massive bleeding and/or cardiac tamponade, as well as critical hemodynamic disturbances. In some cases, mortality can reach 100% that emphasizes the need for timely correct diagnosis and effective treatment of this complication.

Primary cardiac angiosarcoma with right atrial rupture.

A 47-year-old man presented with a recurrent refractory bloody pericardial effusion. Based on the preoperative imaging and pericardiocentesis results with negative cytology, we concluded that blood from a coronary fistula caused the repeated cardiac tamponade. However, pathological findings of the right atrial tissue confirmed primary cardiac angiosarcoma. In this report, we present a unique case as it involves a preoperatively or intraoperatively undetected tumour and a primary cardiac angiosarcoma that induced a fatal outcome, emphasizing the importance of early therapeutic intervention.

Isolated posterior capsule rupture post blunt trauma.

A young child presented with redness and blurred vision in his right eye following a history of blunt trauma with a ball. On examination, he was found to have an isolated posterior capsule rupture (PCR) with cataract formation. A phacoaspiration surgery was performed, and a foldable three-piece intraocular lens (IOL) was implanted in the sulcus with posterior optic capture. The child was reviewed periodically up to 6 months postoperatively and had a vision of 20/25 with a well-centred IOL. Isolated PCR due to blunt trauma is a rare finding. Several factors determine the management of these cas…

Ascending aortic rupture by blunt abdominal trauma.

Traumatic ascending aortic rupture has been firmly associated with blunt thoracic trauma. Adding to the current understanding, we report a case of a patient without signs of external thoracic collision, who suffered a contained aortic rupture just distally of the sinotubular junction, due to severe blunt abdominal trauma. The increased intra-abdominal pressure ruptured the left hemidiaphragm and, presumably, led to the abrupt angulation and rupture of the proximal ascending aorta, sparing the rest of the aorta and the heart. The patient underwent an uneventful urgent aortic repair with a tubul…

Chronic papillary muscle rupture, a case report.

BACKGROUND: Papillary muscle rupture is a rare and potentially fatal complication of acute myocardial ischemia, which typically results in acute severe mitral regurgitation and cardiogenic shock. Due to the urgent need for medical and surgical intervention and high mortality, chronic presentation of mitral regurgitation from papillary muscle rupture is unusual and, to our knowledge, with no reported incidence. CASE PRESENTATION: A 70-year-old female presented with several months of increased fatigability and dyspnea on exertion. Examination and diagnostic echocardiographic studies confirmed s…

Balloon rupture during transcatheter aortic valve replacement.

A distinctive complication with balloon-expandable (BE) THV platforms such as the Edwards Sapien (Edwards Lifescience) is the possibility of balloon rupture during THV deployment. Balloon rupture is a rare occurrence that can result in stroke due to fragment embolism, incomplete THV expansion, and/or vascular injury upon retrieval of the balloon. Careful evaluation of preoperative computed tomography is essential to identify high-risk cases. While annular and left ventricular outflow tract (LVOT) calcification are widely acknowledged as common risks for balloon injury, it's essential to note t…

Rupture of an Aneurysmal Pulmonary Sequestration Artery.

Pulmonary sequestration (PS) is a rare lung malformation seldomly accompanied by aneurysmal deformation of its arterial vasculature. This is a first report of aneurysmal PS presenting with acute aneurysmal rupture. The ruptured aneurysm was treated uneventfully by emergency endovascular coiling, and thoracoscopic hemothorax drainage.

Subclavian Artery Aneurysm Rupture: A Case Report.

Subclavian artery aneurysm is an extremely rare condition with potentially life-threatening complications, including rupture and embolization. This condition is generally the result of atherosclerosis, medial degeneration, trauma, or infection. We report the case of an 83-year-old man who developed hemoptysis due to the rupture of a saccular aneurysm at the origin of the left subclavian artery. This case highlights the importance of early recognition and emergency management of subclavian artery aneurysms, which, despite their rarity, carry significant morbidity and mortality risks.

Current treatment concepts for Achilles tendon rupture.

Achilles tendon rupture is a common and primary cause of lower limb tendon injury suffered during sports-related activities. The causes of Achilles tendon rupture include the calf muscle and tendon overuse, poor tendon quality, and various medical conditions. Historically, acute Achilles tendon rupture was treated conservatively. However, historical techniques are now associated with an increased risk of rerupture. To address this problem, open repair has been proposed. Open repair is associated with a reduced risk of rerupture; however, it is also closely associated with wound complications, …

Traumatic rupture of congenital pelviureteric junction obstruction.

The kidney is frequently injured in blunt abdominal trauma, accounting for 10 % of such cases and being the most commonly affected genitourinary organ in all traumas. Pelviureteric junction obstruction (PUJO) is typically a congenital and asymptomatic anatomical dilatation to the renal pelvis. This article reports on a 21-year-old male who ruptured a congenital PUJO following blunt abdominal trauma. He was managed with a retrograde ureteric stent and a staged pyeloplasty. This case highlights a rare case of urogenital injury due to blunt trauma and explores the impact of congenital anomalies c…

Laparoscopic repair of a traumatic diaphragmatic rupture.

INTRODUCTION AND IMPORTANCE: Traumatic diaphragmatic ruptures following blast injury or penetrating trauma rarely present themselves with chronic symptoms warranting elective surgery. CASE PRESENTATION: We present the case of a 49-year-old man who survived a grenade explosion and experienced chronic chest pain. Considering the previous trauma, computed tomography imaging was performed and showed a left-sided traumatic diaphragmatic rupture ventral to the spleen, resulting in herniation of the transverse colon and omentum in the thoracic cavity. Metal shrapnel was located between the stomach a…
