Necessity of radiographs in the emergency department management of shoulder dislocations.

STUDY OBJECTIVE: To determine the necessity of radiographs in the emergency department management of patients with suspected shoulder dislocation. METHODS: A prospective, observational study was conducted at a university-affiliated, Level I trauma center ED with an emergency medicine residency program. Physicians entered all patients with a suspected shoulder dislocation and reported whether they were certain that the patient's shoulder was dislocated or reduced, before obtaining radiographs. Outcome measures were the assessments of joint position and the radiology reports of prereduction and

Tension band suture technique for the fixation of a displaced posterolateral acromion process fracture with subacromial impingement.

An adult old cyclist presented to our hospital and was referred to the orthopaedic department with a left shoulder posterolateral acromion avulsion fracture and subacromial impingement demonstrated on X-ray and CT. This highly unusual fracture pattern was treated by open reduction and internal fixation with cannulated screws and a tension band suture technique. This fracture went onto successful union with full range of motion and good patient-reported outcome measures by the Oxford Shoulder Score at 3 months. Informed consent was taken for the following case report.

Anterior Bridge Plate Osteosynthesis in Comminuted Fracture Shaft of Humerus in Manual Workers- is it Optimum Choice?

BACKGROUND: Bridge plate osteosynthesis of fractures by minimal invasion and near acceptable reduction is becoming popular and acceptable entity. Management of humeral shaft fracture has evolved a lot with their pros and cons. Anterior bridge plate osteosynthesis (ABPO) for humeral shaft fracture is pertinent to a minimal invasive procedure, and it has evolved as a new entrant in the surgical techniques. This study was designed to carry out the results and efficacy of ABPO in the comminuted fracture shaft of the humerus in the manual workers. METHODS: Study included the closed comminuted frac

A rat traumatized shoulder model for the study of post-surgical adhesions.

A third of patients after open reduction and plating of proximal humerus fractures require subsequent plate removal principally because of adhesions which occur between the deep surface of the deltoid and fracture fixation zone and limit shoulder motion. A rat model of post-surgical shoulder adhesions was developed using a deltoid split approach commonly used in proximal humerus fracture surgery, with trauma induced by a straight diamond nasal rasp to the undersurface of the deltoid and supraspinatus tendon and adjoining proximal humerus. Using the model, the traumatized limb of 12 animals tre

Short-term clinical and radiographic outcomes in patients treated with a modern intramedullary nailing system for 2-part proximal humerus fractures.

OBJECTIVES: The optimal surgical treatment of displaced proximal humerus fractures (PHFs) remains controversial. There are advocates for both open reduction and internal fixation with plate and screws (ORIF) and intramedullary nailing (IMN). The purpose this study was to evaluate the early-term clinical and radiographic outcomes of IMN for isolated, displaced 2-part surgical neck PHFs using a modern, straight nail system and to determine the effect of preoperative patient and fracture characteristics on outcome. METHODS: This was a case series of 23 patients with displaced 2-part surgical nec

Reduction strategies through the anterolateral exposure for fixation of type B and C pilon fractures.

The surgical management of pilon fractures has evolved over the last several years with treatment shifting from acute definitive fixation to delayed fixation. One of the driving forces behind this change was the high incidence of soft tissue complications in those patients with high-energy pilon fractures (Orthopaedic Trauma Association 43B and 43C) managed with acute stabilization. Meticulous soft tissue handling along with delayed definitive fixation based on the soft tissue envelope has decreased the short-term complications associated with treatment of these injuries. Anterolateral exposur

Anatomic reconstruction of unstable lateral clavicular fractures.

BACKGROUND: Fractures of the lateral clavicle result in a high rate of nonunion compared with midshaft fractures. Many operative treatment methods of unstable lateral clavicle fractures have been described, but no single method has become generally accepted. This study reports our experience treating unstable lateral clavicle fractures with coracoclavicular ligament rupture. METHODS: Within 7 years, 19 patients with acute unstable lateral clavicle fractures were treated using a 2.4-mm T-plate and a coracoclavicular polydioxanone cord. The follow-up averaged 5.3 years (range, 2.0-8.8). Shoulde

Osteoporotic fracture treatment.

Orthopaedic surgeons frequently have to deal with osteoporotic fractures of the distal radius, hip and proximal humerus. Low bone mineral density is not only associated with an increased fracture risk but also with more fracture displacement and reduction loss. The specific problems and main treatment options for these fragility fractures are reviewed.

Surgical treatment of bipolar segmental clavicle fracture: A case report.

BACKGROUND: Shoulder injuries caused by trauma are common, including clavicle fractures. Even so, bipolar segmental fracture of the clavicle is extremely rare and seldom reported. Therefore, there is still a controversial issue about how to treat these bipolar segmental clavicle fractures. CASE SUMMARY: A 56-year-old security guard arrived at our emergency room after falling on his electric bicycle. There was no loss of consciousness or other pain. He had no numbness in his fingers. X-rays and 3D computed tomography revealed that the patient's right shoulder had a bipolar segmental clavicle f

Comparison of the effectiveness of oblique and transverse incisions in the treatment of fractures of the middle and outer third of the clavicle.

BACKGROUND: Iatrogenic supraclavicular nerve injury is frequent during surgical repair of clavicle fractures through a transverse incision. The use of an oblique incision may be a potential approach to avoiding this complication. This study compared the clinical effectiveness of oblique and transverse incisions in the treatment of fractures in the middle and outer thirds of the clavicle. METHODS: This prospective observational study included patients with fracture of the mid-to-outer third of the clavicle between August 2011 and August 2016. We allocated the patients into 2 groups based on th

Rotational alignment of humerus after closed locked nailing.

OBJECTIVE: Rotational malalignment that may happen during closed nailing of humeral fractures is, to date, an unexplored area of investigation. The purpose of this study was to examine the effect of arm position during surgery on humeral rotational alignment and the effect of this alignment on shoulder motion and function. METHODS: Thirty patients with eventual fracture healing after closed humeral locked nailing were retrospectively studied: 15 had retrograde nailing; 15, antegrade. Retrograde nailing was performed with the patient in a decubitus position and with the upper arm anteriorly fl

Operative versus nonoperative treatment in complex proximal humeral fractures.

This updated meta-analysis investigated whether operative treatment is superior to nonoperative treatment in complex proximal humeral fractures. The authors searched the Cochrane Central Register of Controlled Trials, PubMed, and EMBASE. Randomized controlled trials that evaluated operative vs nonoperative treatment for exclusively 3- or 4-part proximal humeral fractures were considered. Six studies with a total of 287 patients who had proximal humeral fractures were included. According to the meta-analysis, no statistically significant differences were found between operative and nonoperative

[Fractures of the proximal humerus].

Proximal humeral fractures are common particularly in the elderly. The decision of the optimal treatment is dependent on many factors. On the one hand the biological age of the patient and the bone structure plays a key-role, on the other hand the living conditions and individual needs are of importance. Most fractures with minimal displacement respond satisfactorily to simple conservative treatment including short sling immobilisation and functional aftertreatment under supervision of the physiotherapist. Most recently there is a trend towards more aggressive surgical intervention with percut
