Sex- and Age-Specific Analysis of Mountain Biking Injuries: A 10-Year Review of National Injury Data.

BACKGROUND: Mountain biking (MTB) is a quickly growing sport, with many athletes sustaining high-impact injuries. Current literature has not analyzed the most recent MTB-related national emergency department (ED) injury data. HYPOTHESIS: It was hypothesized that (1) the total number of injuries presenting to US EDs would significantly increase over the study period, (2) male patients would experience higher rates of shoulder injuries and airborne injury mechanisms than female patients, and (3) youths would present more frequently with injuries of the head and face than adults. STUDY DESIGN: …

The effects of shoulder morphology on the distribution of shoulder pressure during load carriage.

To enhance the prevention of shoulder pressure injuries in various load-bearing populations, the effects of shoulder morphology on pressure distribution were investigated. In this study, 69 participants underwent three-dimensional scanning, and based on shoulder morphological characteristic indicators, they were classified into four shoulder types. From these, 28 participants were selected to have the pressure within shoulder regions measured using a pressure-sensing vest while carrying a backpack load equivalent to 15% of their body weight. The results indicated that variations in shoulder mo

Prevalence and mechanisms of injuries in water polo: a systematic review.

OBJECTIVE: To summarise the information available in the literature on the prevalence of injuries in water polo and injury risk factors. ME … injuries seem to affect mainly the shoulder area. Low level evidence suggests a correlation between shoulder injuries and lack of strength or flexibility as well as large volumes of overhead throwing. Further prospective research is needed to investigate risk factors for other body areas.

Outcomes of glenohumeral dysplasia after brachial plexus birth injury using the Sup-ER orthosis.

We investigated the efficacy of the supination-external rotation ('Sup-ER') orthosis, designed as a non-operative treatment to maintain normal anatomical growth of the shoulder, on the progression of glenohumeral dysplasia in patients with brachial plexus birth injuries. The Sup-ER orthosis was fabricated for 20 infants diagnosed with glenohumeral dysplasia after brachial plexus birth injuries and its success in correcting glenohumeral dysplasia was confirmed by objective calculations of the alpha angle on serial ultrasound findings and improvement in Active Movement Scale scores. Of the 20 pa

[Long head of the biceps pathologies : diagnosis and treatments].

The long head of the biceps (LHB) tendon is a common source of shoulder pain. Often associated with other injuries, the tendon can alone be responsible of the symptoms. Spontaneous rupture has been observed to allow for pain relief. The spectrum of LHB lesions is broad. Repetitive motion, carrying heavy loads, have been associated with diagnosis. Specific tests have been described to diagnose biceps injuries, their implementation can remain difficult in the presence of associated pathologies. The combination of a history and clinical examination with the use of targeted imaging is required to

Subscapularis tendon tears: identifying mid to distal footprint disruptions.

We present an arthroscopic technique used to identify mid to distal subscapularis tendon disruptions. These tears can be easy to miss and require a thorough arthroscopic evaluation of the medial biceps sling and the medial side wall of the bicipital groove to detect. The arthroscopic procedure is performed with the patient in the lateral decubitus position, and the same portals used for standard subscapularis repair are used for this technique. If a high degree of clinical suspicion exists for subscapularis pathology and no tendon disruption is initially identified, an inspection of the medial

Early Sport Specialization Linked to Throwing Arm Function and Upper Extremity Injury History in College Baseball Players.

BACKGROUND: Evidence suggests that shoulder and elbow injuries account for 31% to 37% of all National Collegiate Athletic Association (NCAA) baseball injuries, and up to 69% of NCAA baseball injuries are the result of noncontact and overuse mechanisms. Early sport specialization may contribute to the high rates of upper extremity injuries in college baseball players. HYPOTHESIS: Higher specialization by age 13 years would be associated with worse subjective throwing arm function and a greater history of shoulder and elbow injury. STUDY DESIGN: Cohort study. LEVEL OF EVIDENCE: Level 2. METH

Dorsal approach in transfer of the distal spinal accessory nerve into the suprascapular nerve: histomorphometric analysis and clinical results in 14 cases of upper brachial plexus injuries.

PURPOSE: The spinal accessory nerve (SAN) is conventionally transferred to the suprascapular nerve (SSN) through an incision in the supraclavicular region (the anterior approach) to improve shoulder function in brachial plexus injuries. This approach carries a risk of partial denervation of upper trapezius muscle. Here we describe how dorsal nerve transfer through an incision placed directly over the scapular spine preserves the proximal branches to the upper trapezius muscle and allows nerve transfer close to target muscles. METHODS: We report our experience with the dorsal approach in 14 ca

Scapulothoracic dissociation: evaluation and management.

Scapulothoracic dissociation is an infrequent injury with potentially devastating outcomes. Knowledge of this injury is based on small patient series and case reports. The aim of this article is to review the evaluation, management and functional outcomes following scapulothoracic dissociation. Often caused by high traction forces applied to the shoulder girdle, there is a complete loss of the scapulothoracic articulation with lateral scapular displacement and intact skin. This is frequently associated with muscular, ligamentous and osseous injuries to the shoulder girdle, vascular injuries to

Axillary nerve injury.

Axillary nerve injury remains the most common peripheral nerve injury to affect the shoulder. It most often is seen after glenohumeral joint dislocation, proximal humerus fracture, or a direct blow to the deltoid muscle. Compression neuropathy has been reported to occur in the quadrilateral space syndrome, although the true pathophysiology of this disorder remains unclear. The axillary nerve is vulnerable during any operative procedure involving the inferior aspect of the shoulder and iatrogenic injury remains a serious complication of shoulder surgery. During the acute phase of injury, the sh

Survey of neuromuscular injuries to the patient and surgeon during urologic laparoscopic surgery.

OBJECTIVES: Laparoscopy may be complicated by neuromuscular injuries, both to the patient and to the surgeon. We used a survey to estimate the incidence of these injuries during urologic laparoscopic surgery, to assess risk factors for these injuries, and to determine preventive measures. METHODS: A survey of neuromuscular injuries associated with laparoscopy submitted to 18 institutions in the United States was completed by 18 attending urologists from 15 institutions. RESULTS: From among a total of 1651 procedures, there were 46 neuromuscular injuries in 45 patients (2.7%), including abdom

Pediatric shoulder trauma.

Pediatric shoulder trauma is relatively uncommon. Injuries requiring surgical intervention are even rarer. However, it is important for the practicing orthopaedic surgeon to differentiate nonoperative injuries from the urgent and potentially operative injuries. Missing such an injury in the pediatric population could be potentially life threatening or lead to long-term disability. Posterior sternoclavicular dislocations should be differentiated from medial clavicular physeal injuries and promptly reduced. Open fractures or neurovascular threatening fractures should be attended to immediately.

Common injuries in volleyball. Mechanisms of injury, prevention and rehabilitation.

Volleyball has become an extremely popular participation sport worldwide. Fortunately, the incidence of serious injury is relatively low. The sport-specific activity most commonly associated with injury is blocking. Ankle sprains are the most common acute injury. Recurrent sprains may be less likely to occur if an ankle orthosis is worn. Patellar tendinitis represents the most common overuse injury, although shoulder tendinitis secondary to the overhead activities of spiking and serving is also commonly seen. An unusual shoulder injury involving the distal branch of the suprascapular nerve whi

Function and form of the shoulder in congenital and untreated growth hormone deficiency.

OBJECTIVES: The shoulder is the most mobile joint in the entire human body. During arm elevation, it requires the integrity of a set of muscles, bones, and tendons. Individuals with short stature often need to raise their arms above the shoulder girdle and may have functional restriction or shoulder injuries. The impact of isolated GH deficiency (IGHD) on joints remains not well defined. The purpose of this work is to evaluate the function and structure of the shoulder in short-statured adult individuals with untreated IGHD due to the same homozygous mutation in the GHRH receptor gene. METHOD…
