The Bankart repair illustrated in cross-section. Some anatomical considerations.

The Bankart repair for chronic anterior shoulder instability effectively addresses the pathologic components responsible for repeated dislocation or subluxation. However, contrary to popular belief, the Bankart repair does not precisely restore the premorbid anatomy. The capsule is reattached to the boney rim of the anterioinferior glenoid deep to and lateral to the torn cartilagenous labrum, thus excluding the labrum from the joint anteriorly. This was demonstrated by cross-sectional cadaver dissections performed to illustrate this complex surgical anatomy to orthopaedic residents in training

[Significance of a transthoracic imaging technic in shoulder arthrography].

Even in this era of sonography, arthrography of the shoulder is an important diagnostic investigation, making it possible to exclude unrecognized connective tissue lesions in patients with chronic shoulder pain resistant to therapy. Sometimes standard X-ray techniques do not yield all the information the surgeon needs about ventral or dorsal localization of a rotator-cuff rupture. An additional approach to the joint is needed: a transthoracic lateral-view X-ray provides the surgeon with adequate information to help in selection of the most appropriate operative approach to the shoulder.

The impact of grip width on kinetics and kinematics in the shoulder press among resistance-trained men.

This study investigated the impact of grip width on kinematics and kinetics in the seated barbell shoulder press among resistance-trained men. Eleven participants (age: 25.9 ± 3.1 years, height: 180.4 ± 5.4 cm, body mass: 87.0 ± 7.9 kg) performed eight repetitions to failure with a narrow, medium and wide grip width. Statistical parametric mapping was used to analyse kinematics and kinetics during the entire concentric phase of the last repetition. The main findings revealed that narrower grips increased both load lifted, and shoulder, and elbow range of motion. Moreover, grip width influenced

Capsule function following anterior dislocation: implications for diagnosis of shoulder instability.

During shoulder dislocation, the glenohumeral capsule undergoes non-recoverable strain, leading to joint instability. Clinicians use physical exams to diagnose injury and direct repair procedures; however, they are subjective and do not provide quantitative information. Our objectives were to: (1) determine the relationship between capsule function following anterior dislocation and non-recoverable strain; and (2) identify joint positions at which physical exams can be used to detect non-recoverable strain in specific capsule regions. Physical exams were simulated at three joint positions incl

[Imaging assessment and treatment of Charcot arthropathy of the shoulder assisted by NGH classification].

Charcot arthropathy of the shoulder is a rare destructive joint disorder that is most commonly secondary to syringomyelia and often leads to osteolysis of the shoulder joint and severe upper limb dysfunction. The diagnostic and therapeutic challenges of this condition lie not merely in identifying joint destruction, but in the nonspecific nature of its early clinical manifestations, which may mimic infection, tumor, or inflammatory arthropathy. In addition, the extent of bone destruction in the glenoid and humeral head may vary considerably, making conventional imaging descriptions insufficien…

Simulated effect of reaction force redirection on the upper extremity mechanical demand imposed during manual wheelchair propulsion.

BACKGROUND: Manual wheelchair propulsion is associated with overuse injuries of the shoulder. Reaction force redirection relative to upper extremity segments was hypothesized as a means to redistribute mechanical load imposed on the upper extremity without decrements in wheelchair propulsion performance. METHODS: Two individuals performed wheelchair propulsion under simulated inclined (graded) conditions using self-selected control strategies. Upper extremity kinematics and reaction forces applied to the wheel were quantified and used as input into an experiment-based multi-link inverse dynam

Reverse anatomy shoulder replacement: comparison of two designs.

The aim of this study was to use a biomechanical model of the shoulder to examine the characteristics of two concepts of reverse anatomy imp … nt arms through almost all of the range of motion in the Delta model were greatest. As a result the shoulder joint contact force was decreased for this implant. Furthermore, although the B-W model predicted similar moment arms to natural anatomy, the contact force decreased owing to change in the joint constraints. Finally, it was concluded that the reverse anat …

Treatment of avulsed clavicle and recurrent subluxations of the ipsilateral shoulder by dynamic fixation.

A new operative approach is presented for treatment of fractured distal end of clavicle associated with recurrent anterior shoulder subluxations. One case is presented in which open reduction of the fractured clavicle and stabilizing the anterior shoulder joint were accomplished by transfer of the coracoid process with its attached muscles under the subcapularis muscle onto the clavicle. The operation was followed by complete reduction of the fracture and a stable shoulder joint.

Thermal capsulorraphy of bilateral glenohumeral joints in a pediatric patient with Ehlers-Danlos syndrome.

Ehlers-Danlos syndrome (EDS) is a heterogeneous collection of inherited connective tissue disorders characterized by hypermobility of the joints and hyperextensibility and fragility of the skin. For many patients, the hypermobile joints become problematic. To date, the mainstay of surgical treatment for EDS-related joint laxity has been open surgical capsulorraphy, which, although usually effective, confers significant morbidity to the patient. We present the case of a 9-year-old girl diagnosed with a variant of EDS and severely disabled from multidirectional instability of her shoulders and r

The influence of glenosphere size and glenoid-sided offset on shoulder stability following reverse total shoulder arthroplasty using the Zimmer Trabecular Metal Reverse Plus shoulder system.

BACKGROUND: Recurrent instability continues to be a common complication following reverse total shoulder arthroplasty (rTSA); however, the influence of implant size and offset on joint compression and resistance to subluxation is poorly understood. The purpose of this study was to investigate the effect of glenosphere size, glenoid lateralization, and inferior offset on joint stability following rTSA using the Zimmer Trabecular Metal Reverse Plus shoulder system. METHODS: rTSA was performed on 8 fresh-frozen human cadaveric scapulae with the subscapularis tendon transected at its humeral inse

Unilateral neuropathic arthropathy of the shoulder secondary to syringomyelia: Diagnostic challenges.

Neuropathic arthropathy of the shoulder is a rare disorder characterized by joint degeneration, and is associated with loss of sensory innervation. Syringomyelia is a disease in which fluid-containing cavities (syrinxes) form within the spinal cord. Here, we report a case of neuropathic arthropathy of the shoulder secondary to syringomyelia in a 40-year-old woman. X-rays of the left shoulder revealed damage to bone and joint architecture. Blood tests indicated vitamin D deficiency and secondary hyperparathyroidism. Magnetic resonance imaging of the cervical spine showed a large syrinx from the

Bilateral shoulder septic arthritis in a fit and well 47-year-old man.

Bilateral septic arthritis of the shoulder is uncommon in the immunocompetent patient with no previous risk factors for joint infection, and is thus easily missed. Septic arthritis is associated with significant rates of morbidity and mortality. Early diagnosis and management is the key to a favourable outcome; septic arthritis should be considered as a differential diagnosis in the unwell patient presenting with shoulder pain and reduced range of joint movement. We present a case of a 47-year-old previously fit and well man with bilateral shoulder septic arthritis. We will also review the cur

The modified sleeper stretch and modified cross-body stretch to increase shoulder internal rotation range of motion in the overhead throwing athlete.

SYNOPSIS: Stretching techniques that focus on increasing posterior shoulder soft tissue flexibility are commonly incorporated into prevention and treatment programs for the overhead athlete. The cross-body and sleeper stretch exercises have been described as stretching techniques to improve posterior shoulder soft tissue flexibility and to increase glenohumeral joint internal rotation and horizontal adduction range of motion in the overhead athlete. But, based on the inability to stabilize the scapula and control glenohumeral joint rotation with the cross-body stretch and the potential for sub

[Hemarthroses complicating anticoagulant therapy (heparin, oral anticoagulants)].

Three patients with hemarthrosis complicating anticoagulant therapy are described. Two had acute hemarthrosis of the knee joint occurring during long term oral anticoagulant therapy for cardiovascular disorders. Joint symptoms disappeared rapidly after arthrocentesis and diminution of the dose of oral anticoagulant medication. The third patient has been on chronic hemodialysis for the last three years. After 2 years of this therapy he developed periarthritis of the right shoulder, followed suddenly by hemarthrosis of the right shoulder which remained symptomatic for 9 months in spite of severa

[Overuse injury syndromes in the shoulder area].

The shoulder is the most movable and the most unstable joint in a human body. The glenohumeral joint is particularly vulnerable because overhead activities put tremendous stress on its static stabilizers (ligamentous labral complex) and the dynamic stabilizers (rotator cuff muscles). Overhead activities may stretch or injure the static stabilizers. Minor aberrations in mechanisms controlling stability have a significant and cumulative effect on the shoulder biomechanics, and increase the risk of injury. Impingement syndrome and glenohumeral instability are predominant injuries affecting the sh
