PURPOSE: Rapidly progressive destructive arthrosis (RPDA) of the glenohumeral joint is a rare but distinct clinical entity characterized by rapid humeral head collapse of unclear etiology. This study aimed to characterize the clinical, radiologic, and intraoperative features of RPDA, to propose a three-stage radiologic classification, and to evaluate short-term outcomes following shoulder arthroplasty (reverse or total) according to disease stage. METHODS: From March 2015 to December 2023, 23 patients were diagnosed with RPDA of the glenohumeral joint based on clinical and radiologic findings.

BACKGROUND: Fractures of the proximal humerus are associated with a profound temporary and sometimes permanent, impairment of function and quality of life. The treatment of comminuted fractures of the proximal humerus like selected three-or four-part fractures and split fractures of the humeral head is a demanding and unresolved problem, especially in the elderly. Locking plates appear to offer improved fixation; however, screw cut-out rates ranges due to fracture collapse are high. As this may lead to higher rates of revision surgery, it may be preferable to treat comminuted fractures in the

BACKGROUND: Proximal humerus nonunion is a challenging complication of fractures that can be treated surgically with either open reduction internal fixation (ORIF) or reverse total shoulder arthroplasty (RTSA). The few studies published on this subject have shown high rates of complications and revision surgery when RTSA has been performed for proximal humerus nonunion. The purpose of this study was to determine the rates of complications and revision of this procedure at our institution, as well as to identify any variables that may impact risks of complications and reoperations.

BACKGROUND: Superior inclination of the base plate in reverse shoulder arthroplasty (RSA) is underestimated and may lead to major setbacks in terms of functional outcomes due to the altered biomechanics. Joint instability, scapular notching, and loosening of the glenoid component are considered the most serious sequelae. Therefore, a thorough preoperative radiological assessment of the affected shoulder joint and customized design of the prosthesis according to the glenoid morphology are decisive and directly correlated to the outcome. In this article, we propose a simple radiographic techniqu

BACKGROUND: Stemmed humeral components have been used since the 1950s; canal-sparing (also known as stemless) humeral components became commercially available in Europe in 2004. The Simpliciti total shoulder system (Wright Medical, formerly Tornier) is a press-fit, porous-coated, canal-sparing humeral implant that relies on metaphyseal fixation only. This prospective, single-arm, multicenter study was performed to evaluate the two-year clinical and radiographic results of the Simpliciti prosthesis in the U.S.
METHODS: One hundred and fifty-seven patients with glenohumeral arthritis were enrol

This paper describes a clinical randomized single-blinded study of the effects of Functional Electrical Therapy (FET) on the paretic arms of subjects with acute hemiplegia caused by strokes. FET is an exercise program that comprises voluntary arm movements and opening, closing, holding, and releasing of objects that are assisted by a neural prosthesis (electrical stimulation). FET consisted of a 30 min everyday exercise for 3 consecutive weeks in addition to conventional therapy. Twenty-eight acute hemiplegic subjects participated in a 6 mo study. The subjects were divided into lower functioni

BACKGROUND: Following shoulder arthroplasty, any well-planned rehabilitation program should include muscle strengthening. However, it is not always clear how different external loads influence shoulder kinematics in patients with shoulder prostheses. The objective of this study was to describe shoulder kinematics and determine the contribution of the scapulothoracic joint to total shoulder motion of patients with total and reverse shoulder arthroplasties and of healthy individuals during rehabilitation exercises (anteflexion and elevation in the scapular plane) using different loading conditio

Giant bursitis with rice bodies is an important clinical entity recognized in rheumatoid arthritis. Usually the bursitis is connected to a joint space. In this unusual case of a giant bursitis of the shoulder/neck region, no connection to a joint could be found. The bursitis lays directly on a rib. The clinical and radiological findings are presented and this special case is discussed in comparison to the literature.

Breast damage is likely to result in compensation according to the principle of full compensation. However, breast damage, little covered in the scales usually used, requires a specific methodology. The quantification of functional prejudice must be done with regard to the age of the victim and must take into account the possible repercussions on mobility (shoulder and/or spine) as well as the psychological repercussions. Whether or not the nipple and areola are preserved should be described. Temporary aesthetic damage (before breast reconstruction) must be assessed on the basis of objective e

Myoelectric prostheses assist users to live their daily lives. However, the majority of users are primarily confined to forearm amputees because the surface electromyography (sEMG) that understands the motion intents should be acquired from a residual limb for control of the myoelectric prosthesis. This study proposes a novel fabric vest socket that includes embroidered electrodes suitable for a high-level upper amputee, especially for shoulder disarticulation. The fabric vest socket consists of rigid support and a fabric vest with embroidered electrodes. Several experiments were conducted to

The wear of ultra-high molecular weight polyethylene, UHMWPE, components has been associated with the failure of joint prostheses in the hip, knee, and shoulder. Considering that in vitro experiments are generally too expensive and time-consuming, computational models are an attractive alternative to study the wear behavior of UHMWPE components. The objective of the present study was to develop a computational wear model to evaluate the wear resistance of anatomical and reverse shoulder prostheses. The effects of the wear law and the updating of the UHMWPE surface on the prediction of wear wer

BACKGROUND: Proximal humeral fractures that are treated with locked plate constructs remain susceptible to collapse into a varus position. The objectives of the present study were to examine how medial comminution affects fracture stability and to determine the effect of calcar fixation on osteosynthesis stability.
METHODS: Eleven matched pairs of cadaveric humeri were osteotomized to create standard three-part fractures involving the surgical neck and the greater tuberosity. Five matched pairs were randomly assigned to have the medial calcar region remain intact. Six matched pairs had remova

BACKGROUND: Loosening of the glenoid component continues to be the foremost cause of medium and long-term failure of shoulder replacements. The purpose of this study was to evaluate the clinical and radiographic results of a minimally cemented all-polyethylene pegged glenoid component designed for biologic fixation.
METHODS: Forty-four shoulders in forty-one patients with a mean age of sixty-six years underwent total shoulder arthroplasty with a pegged bone-ingrowth glenoid component. Outcome data included the American Shoulder and Elbow Surgeons questionnaire, the Simple Shoulder Test, and v

Rotator cuff tear arthropathy represents a spectrum of shoulder pathology characterized by rotator cuff insufficiency, diminished acromiohumeral distance with impingement syndromes, and arthritic changes of the glenohumeral joint. Additional features may include subdeltoid effusion, humeral head erosion, and acetabularization of the acromion. Although the progression of rotator cuff tears seems to play a role in the development of cuff tear arthropathy, information is lacking regarding the natural progression of rotator cuff tears to cuff tear arthropathy. Controversy remains about the role of

In a prospective, randomized study between 2000 and 2004, 20 patients with primary osteoarthritis of the shoulder had a total shoulder arthroplasty with radiostereometric analysis, 10 with keeled and 10 with pegged glenoid components. The relative movement of the glenoid component with respect to the scapula was measured over a 24-month period. Three keeled and five pegged glenoids needed reaming for erosion. The largest translations occurred along the longitudinal axis (mean of 1.35 mm for keeled eroded components) (P = .017 for keeled vs pegged components and P = .013 for eroded vs non-erode

Constrained total shoulder arthroplasty is a salvage procedure for patients with disabling pain and a nonfunctioning rotator cuff mechanism. Considering the advanced pathologic process and severe symptoms of these patients, the results of pain relief are gratifying. Many of the patients have weak or absent deltoid muscles, and active overhead motion is not possible. A method of attaching the metal glenoid to the scapula has been designed to reduce the risk of loosening. However, constrained total shoulder arthroplasty does produce considerable stress on bony attachments. Careful patient select
