Pneumatization of the skull base due to valsalva maneuver.

Objectives : We herewith report the case of a 43-year-old patient who experienced a skull base fracture associated with minimal trauma. The computed tomography scan examination showed unusual extensive pneumatization of the skull base. This patient used to perform repeated Valsalva maneuvers on a daily basis since childhood. Methods : A literature search and analysis of pneumatization mechanisms was performed. Results : Extensive skull base pneumatization has already been reported in patients performing daily Valsalva maneuver. Two pathogenic hypotheses are discussed: the bone cavity expansion…

Skull base tumor in a patient with phacomatosis pigmentovascularis.

A 58-year-old man with clinical diagnosis of phacomatosis pigmentovascularis (PPV) experienced tinnitus and progressive hearing loss due to a jugular foramen tumor.Attached to the tumor capsule, were several pigmented spots. Pathological examination revealed a tumor composed by two different tissues, namely a Schwannoma grade I associated with a leptomeningeal blue nevus. The neuropathological aspects of this unusual association are discussed. The association of PPV with a pigmented skull base tumor has not been described to date and illustrates the importance of systemic examination in PPV.

Prevention and management of positional skull deformities in infants.

Positional skull deformities may be present at birth or may develop during the first few months of life. Since the early 1990s, US pediatricians have seen an increase in the number of children with cranial asymmetry, particularly unilateral flattening of the occiput, likely attributable to parents following the American Academy of Pediatrics "Back to Sleep" positioning recommendations aimed at decreasing the risk of sudden infant death syndrome. Positional skull deformities are generally benign, reversible head-shape anomalies that do not require surgical intervention, as opposed to craniosyno…

The thickness of skull at the halo pin insertion site.

STUDY DESIGN: The thickness of 415 skulls of Thai population was measured at the areas where halo pins are placed. OBJECTIVE: Analysis of skull thickness at the area of halo pin insertion site. SUMMARY OF BACKGROUND DATA: Halo orthosis is a device used for providing stable fixation of instability of cervical spine injury. Although this device can be applied quickly and safely, complications do occur. While clinical experiences have informed us that cranial bone thickness varies with age. METHODS: Coronal computerized tomography scans of 415 heads of patients who had no head injury, skull fr…

[Diagnosis and treatment of injuries to the frontal skull base].

The role of ENT surgery in the management of anterior skull base defects has become increasingly important in recent years. Transnasal endoscopic surgical techniques and intraoperative navigation enable a minimally invasive approach in a large proportion of patients, thus helping to avoid morbidity typically associated with neurosurgical subfrontal approaches. Whereas traffic accidents and sport injuries are the main causes of anterior skull base trauma in the civilian setting, penetrating injuries caused by gunshots and improvised explosive devices (IEDs) play an increasing role in the milita…

[Application of neuroendoscope in the treatment of skull base chordoma].

OBJECTIVE: To further explore the application, approach, indication and prognosis of neuroendoscope treatment for skull base chordoma. METHODS: A total of 101 patients of skull base chordoma were admitted at our hospital from May 2000 to April 2010. There were 59 males and 42 females. Their major clinical manifestations included headache, cranial nerve damage and dyspnea. They were classified according to the patterns of tumor growth: Type I (n = 13): tumor location at a single component of skull base, i. e. clivus or sphenoid sinus with intact cranial dura; Type II (n = 56): tumor involving …

The sociopolitical history and physiological underpinnings of skull deformation.

In this report, the evidence, mechanisms, and rationale for the practice of artificial cranial deformation (ACD) in ancient Peru and during Akhenaten's reign in the 18th dynasty in Egypt (1375-1358 BCE) are reviewed. The authors argue that insufficient attention has been given to the sociopolitical implications of the practice in both regions. While evidence from ancient Peru is widespread and complex, there are comparatively fewer examples of deformed crania from the period of Akhenaten's rule. Nevertheless, Akhenaten's own deformity, the skull of the so-called "Younger Lady" mummy, and Tutan…

A triage system for referrals of pediatric skull deformities.

The clinical pathways for craniosynostosis and nonsynostotic skull deformity are entirely different. At the Dutch Craniofacial Center (DCFC), all patients were assessed in the same multidisciplinary craniofacial clinic, a common practice in countries with developed health care. However, the high volume of referrals of nonsynostotic cases frequently resulted in the capacity of these clinics being exceeded, with some patients being assessed in the general pediatric plastic surgery clinic instead. In these general clinics, not all the multidisciplinary team members are routinely present, so patie…

Cloverleaf skulls: etiologic heterogeneity and pathogenetic variability.

Cloverleaf skulls (Kleeblattschädel-Syndromen, trilobular skulls) have different modes of development (pathogenetic variability). Synostosis may involve various sutures in different types of cloverleaf skulls. Their facial features, radiographic findings, and CNS anomalies are also reviewed. Different causes of cloverleaf skulls have been identified (etiologic heterogeneity). One table lists 26 different conditions with trilobular skulls identified under various etiologic headings: monogenic, chromosomal, disruptive, iatrogenic, and distinctive conditions of unknown cause. Discussed in detail …

[Surgery at the skull base with transnasal endoscopic control].

INTRODUCTION: Skull base surgery is targeted to diseases in the posterior, lateral or anterior skull base. Development of endoscopic paranasal sinus surgery has contributed to expand the indications for transnasal surgery at the anterior skull base. Patients do not have visible scars and have less postoperative pain after endoscopic surgery and a decreased postoperative morbidity in general results in a shorter hospital stay. MATERIAL AND METHODS: An overview is given of transnasal endoscopic skull base surgery, based on literature retrieved from PubMed and illustrated by three patients opera…

The fluctuating asymmetry of medieval and modern human skulls.

The analysis of fluctuating asymmetry (FA) allows for estimating the influence of stress factors on human development and allows to evaluate resistance to stress. FA is often used as a marker of prenatal stress. The aim of this work is to estimate the symmetry of skulls from selected historic human populations and to analyse changes in their morphology which have taken place over centuries. The studied material consisted of two skull samples - a modern sample containing 82 skulls and a medieval sample of 77 skulls from Gródek on the Bug River. Radiographs were taken in postero-anterior (P-A) a…

Diagnostic and surgical challenges in the pediatric skull base.

Advances in management of adult skull base pathologies are increasingly being applied in children. Pediatric patients present special challenges because of their smaller anatomy, but potential gains in reduced morbidity make improvements in skull base approaches well worth pursuing.

Bone strain gradients and optimization in vertebrate skulls.

It is often stated that the skull is optimally designed for resisting feeding forces, where optimality is defined as maximum strength with minimum material. Running counter to this hypothesis are bone strain gradients--variation in bone strain magnitudes across the skull--which in the primate skull have been hypothesized to suggest that different parts of the skull are optimized for different functions. In this paper strain gradients in the skulls of four genera of primates, Sus, and Alligator were documented and compared. Strain gradients were pervasive in all taxa sampled. Patterns of strain…

Evaluation of guidelines for skull radiography in head injury.

The benefits of utilisation of guidelines for radiographic skull examination of head injured patients were studied in one hundred and eleven patients. The causes of head injury were road traffic accidents (RTA) 87.4%, fall and assault 4.5% each, collapsed wall 1.8%, gunshot injury and industrial accident 0.9% each. RTA resulted predominantly from burst tyre 34%, vehicle pedestrian accidents 23.7% and vehicle collision 18.6%. Compliance with the use of the guidelines was 48%. Radiographic skull examination was carried out in 22 patients (20% of study population) and the positive yield for skull…

[Dedifferentiated chordoma of the skull base. A case report].

Sarcomatous transformation in chordoma is a very rare condition and has been emphasized as a distinct entity because of its more aggressive clinical course. Here we describe a case of dedifferentiated chordoma arising from the skull base region of an 11-year-old boy, with tumor recurrence within one year. This tumor showed features of pleomorphic cell sarcoma with areas more typical of chordoma. Most of tumor cells expressed cytokeratin, epithelial membrane antigen, vimentin and S-100 protein, thus confirming the diagnosis of dedifferentiated chordoma.

Cavernomas of the skull: review of the literature 1975-2000.

We describe four patients with intraosseous cavernous hemangiomas of the skull which were localized supraorbitally, parietally (two cases), and occipitally. The diameter ranged from 15 mm to 25 mm. They presented with slowly growing mass, tender to pressure, with spontaneous pain, and with freely mobile skin above the cavernoma sites. Magnetic resonance imaging (hyperintensity on T2 and isointensity with brain on T1) and CT (osteolytic lesion with erosion of the tabula externa) confirmed the plain skull films showing the honeycomb or sunburst appearance pattern. Resections and postoperative co…
