Uveal Effusion Syndrome as the Initial Manifestation of Primary Sjögren's Syndrome and Review of the Literature.

Purpose: The authors report a38-year-old woman with primary Sjögren's syndrome who initially showed recurrent blurred vision caused by uveal effusion syndrome and later developed dry mouth, dry eyes, and arthralgia. During the 5-year-course of disease, the patient's 3-time-onset was all manifested as blurred vision after decreased immunity. Despite the initial absence of sufficient immunological evidence, the final presence of positive serum anti-SS-A, rheumatoid factors, ANA, and inflammatory findings in minor salivary gland biopsy indicated primary Sjögren's syndrome. Methods: Retrospective r

Finger-Prick Autologous Blood in the Treatment of Persistent Corneal Epithelial Defects.

PURPOSE: Autologous hemoderivative eye drops have a role in the management of persistent epithelial defects (PEDs), but their use may be limited by cost and availability. Finger-prick autologous blood (FAB) treatment uses whole capillary blood, obtained from a sterilized fingertip, as an alternative form of hemoderivative eye drop therapy. To date, 1 report has described the safe and effective use of FAB for dry eye and PEDs. We report the results of 10 eyes (10 patients) treated with FAB for PEDs. METHODS: Ten patients with PEDs in 1 eye for a mean of 259 ± 201 days due to diabetic neurotrop

Refractive surprise after routine cataract surgery with multifocal IOLs attributable to corneal epithelial basement membrane dystrophy.

We describe two patients in whom postsurgical refractive error occurred after routine refractive lens exchange cataract surgery with multifocal intraocular lens insertion most likely attributable to the presence of underlying corneal epithelial basement membrane dystrophy (EBMD). In Case 1, there was an unexpected hyperopic postoperative spherical equivalent refractive error of +1.50 diopters and in Case 2, a cylindrical refractive error of 2.75 diopter cylinder. We examine the possible causes of error and discuss potential management strategies to prevent and address these unpredictable posto

Quantification of Ocular Biomechanics In Ocular Manifestations of Systemic Autoimmune Diseases.

Purpose: To quantify biomechanical change associated with autoimmune diseases using Corvis ST deformation data. Methods: Cross-sectional, observational, case control study of 76 patients with systemic autoimmune disease and 21 control subjects. All patients underwent detailed ophthalmic examination with Corvis-ST (Oculus Optikgerate Gmbh, Germany) measurements for biomechanical properties of the eye. Corneal deformation and also deformation caused by the extracorneal tissue were recorded. Using a mathematical formula, the three parameters, namely, corneal stiffness (kc), extracorneal tissue st

[Fungal keratitis in Germany : Diagnostics, conservative and microsurgical treatment].

Fungal keratitis is still a relatively rare but very serious ophthalmological condition with a potentially vision-threatening course, possibly including even loss of the eye. Similar to acanthamoeba keratitis, it is often initially misdiagnosed with the slit lamp. For more than 20 years an increase in the incidence of fungal keratitis has been observed, also in Germany possibly due to the increased use of soft contact lenses and climate change. This article provides an overview of the typical clinical signs and symptoms of keratomycosis and presents the latest findings in the field of diagnost…

Eyelid Retraction Following Combined Frontalis Flap and Levator Advancement Surgery.

An 84-year-old female with a history of bilateral congenital ptosis presented with pain in the OD. The patient had undergone bilateral frontalis flap and levator advancement for decompensated congenital ptosis 18 months prior. On examination, the patient demonstrated significant retraction and contour abnormalities of the right upper eyelid. Slit lamp exam revealed superficial punctate keratitis of the OD. The patient underwent ptosis revision of the right upper eyelid, which revealed scarring of the orbital septum to the advanced levator muscle and a severely fibrotic levator muscle. Followin…

[Early diagnosis of keratoconus].

Keratoconus is morphologically associated with increasing deformation, thinning and scarring of the cornea. This functionally leads to refractive changes and visual deterioration. In the early stages there are often no clear clinical signs in the slit-lamp examination; however, confirming the diagnosis as early as possible is important in order to provide patients with an appropriate treatment. For the early diagnosis of keratoconus, various diagnostic devices have been introduced in recent years and decades. These include keratometry with reflection-based or elevation-based systems and optica

A retained lens fragment induced anterior uveitis and corneal edema 15 years after cataract surgery.

A 60-year-old male was referred to the ophthalmologic clinic with aggravated anterior uveitis and corneal edema despite the use of topical and systemic steroids. He had undergone cataract surgery in both eyes 15 years previous. Slit lamp examinations revealed a retained lens fragment in the inferior angle of the anterior chamber, with severe corneal edema and mild anterior uveitis. The corneal edema and uveitis subsided following surgical extraction of the lens fragment. That a retained lens fragment caused symptomatic anterior uveitis with corneal edema 15 years after an uneventful cataract s…
