A clinical study of viper bite poisoning.

Thirty-one consecutive cases of snake bite were studied in the Benue Valley, Nigeria where carpet viper (Echis carinatus) bite constitutes a serious health problem. E. carinatus was responsible for 26 cases, the night adder (Causus maculatus) for three and the puff adder (Bitis arietans) for two. There were two fatalities, both late admissions following E. carinatus poisoning. One patient died after a subarachnoid haemorrhage and the other after tissue necrosis and the complications of a tightly applied tourniquet. Another fatality after E. carinatus bite was suspected in a patient who dischar

Fatal Sea Snake Envenomation.

A 23-year-old man working on a prawn trawler off the Northern Australian coast was bitten on the right hand by a black-banded sea snake (Laticauda colubrina), resulting in the rapid onset of ptosis, blurred vision, and respiratory difficulties with convulsions, loss of consciousness, and death. Resuscitation was unsuccessful. No antivenom was available because of the remote location. At autopsy, a small puncture wound was identified on the middle interphalangeal joint of the right little finger. No other significant abnormalities were detected. Elevations in creatine kinase (1558 μ/L) and myog

Infrared thermography to diagnose and manage venomous animal bites and stings.

INTRODUCTION: Infrared imaging (IR) is a noninvasive technique that quantifies body surface temperature, producing a digital color image. IR has been used to study diseases in which skin temperature can reflect the presence of inflammation. METHODS: This was an observational pilot study of eight patients envenomed by snakes, spiders, and scorpions. All patients were examined using a thermal camera. RESULTS: In all cases, we obtained infrared images that corroborated clinical findings indicating localized effects of venom, specifically inflammation. CONCLUSIONS: IR has potential for use as a

Marine envenomations.

BACKGROUND: Marine stings are common but most are minor and do not require medical intervention. Severe and systemic marine envenoming is uncommon, but includes box jellyfish stings, Irukandji syndrome, major stingray trauma and blue-ringed octopus envenoming. Almost all marine injuries are caused by jellyfish stings, and penetrating injuries from spiny fish, stingrays or sea urchins. OBJECTIVE: This article describes the presentation and management of marine envenomations and injuries that may occur in Australia. DISCUSSION: First aid for jellyfish includes tentacle removal, application of

Snakebite in Australia: a practical approach to diagnosis and treatment.

Snakebite is a potential medical emergency and must receive high-priority assessment and treatment, even in patients who initially appear well. Patients should be treated in hospitals with onsite laboratory facilities, appropriate antivenom stocks and a clinician capable of treating complications such as anaphylaxis. All patients with suspected snakebite should be admitted to a suitable clinical unit, such as an emergency short-stay unit, for at least 12 hours after the bite. Serial blood testing (activated partial thromboplastin time, international normalised ratio and creatine kinase level)

Digital gangrene following a green pit viper bite.

In Hong Kong, the white-lipped green pit viper (Trimeresurus albolabris) accounts for the majority of venomous snake bites. In these patients, the cardinal features are local pain and swelling and mild coagulation abnormalities, but digital gangrene is uncommon. A 58-year-old woman was bitten on the left middle finger pulp by a white-lipped green pit viper. She developed local pain and swelling and coagulopathy, complicated by pulp tissue necrosis and digital gangrene of left middle finger. She fully recovered after amputation of the left middle finger at the mid portion.

Spotted black snake (Pseudechis guttatus) envenomation in a maned wolf (Chrysocyon brachyurus).

Envenomation by a spotted black snake (Pseudechis guttatus), following multiple bites on the buccal mucosa of a captive maned wolf (Chrysocyon brachyurus), caused the animal's collapse, hemolysis, rhabdomyolysis, local tissue necrosis, hepatic and renal failure, and subsequent death. The wolf died despite intensive supportive care including antivenom administration, fluid support, and a blood transfusion. Gross necropsy findings included myocardial and intestinal hemorrhage, pulmonary congestion, hepatomegaly, and splenomegaly. Microscopic examination of formalin-fixed tissues demonstrated pul

[Avidity of sera-neutralizing snake venom].

Avidity of antivenom sera used for the treatment of snake bites was studied. Sera against the venom of Vipera libetina obtained from producers immunized with crude venoms were more avid than analogous sera obtained to anavenoms. In studying the avidity of polyvalent serum neutralizing the Vipera libetina, echis and cobra venoms showed the serum obtained in immunization with the mixture of crude venoms to be highly avid to all the venoms composing the antigen; besides, it bound the venoms of Vipera libetina and echis more rapidly and more stably than the corresponding monovalent sera.

Management of ophidiotoxicosis caused by coral snakes.

This case report presents an exceptional instance of envenomation following a physical altercation in which a coral snake was used as a whip. During the incident, the snake bit the patient's cheek, leading to envenomation. Within a few hours, he was hospitalised with sensory disturbances, weakness in the affected and adjacent regions, and nausea and vomiting. The snake was identified as an adult male Micrurus tener, also known locally as the 'coralillo' or 'harlequin coralillo'. Laboratory studies revealed leucocytosis, low haemoglobin concentration and an increase of creatine phosphokinase le

Pathogenic mechanisms underlying adverse reactions induced by intravenous administration of snake antivenoms.

Snake antivenoms are formulations of immunoglobulins, or immunoglobulin fragments, purified from the plasma of animals immunized with snake venoms. Their therapeutic success lies in their ability to mitigate the progress of toxic effects induced by snake venom components, when administered intravenously. However, due to diverse factors, such as deficient manufacturing practices, physicochemical characteristics of formulations, or inherent properties of heterologous immunoglobulins, antivenoms can induce undesirable adverse reactions. Based on the time lapse between antivenom administration and

Experimental studies on curing elapid bite with trypsin.

The local injection of trypsin can be regarded as a new and effective therapy for venomous snakebite. Our experiments indicate that when mice or dogs have been injected with the lethal dose of elapid venom subcutaneously, they can all survive and have no symptoms of systemic poisoning if necessary dose of trypsin is injected locally and promptly. When the lethal dose of cobra venom is injected into mice, all of them can survive if trypsin is given within 15 minutes. And even if trypsin is given 20--50 minutes later, 50--90% of the mice can avoid death. The prompt local injection of trypsin is

Human anti-snake venom IgG antibodies in a previously bitten snake-handler, but no protection against local envenoming.

We report a 60 year old male bitten by snakes from the Acanthophis genus (Death adder) on two occasions who developed high titres of human IgG antibodies to Acanthophis venom detected at the time of the second bite. The patient was bitten by Acanthophis antarcticus (common death adder) on the first occasion, developed non-specific systemic effects and did not receive antivenom. Three months later he was bitten by Acanthophis praelongus (northern death adder) and he developed significant local myotoxicity associated with a moderate rise in the creatine kinase (maximum 4770 U/L). He was given an

High-throughput proteomics and in vitro functional characterization of the 26 medically most important elapids and vipers from sub-Saharan Africa.

Venomous snakes are important parts of the ecosystem, and their behavior and evolution have been shaped by their surrounding environments over the eons. This is reflected in their venoms, which are typically highly adapted for their biological niche, including their diet and defense mechanisms for deterring predators. Sub-Saharan Africa is rich in venomous snake species, of which many are dangerous to humans due to the high toxicity of their venoms and their ability to effectively deliver large amounts of venom into their victims via their bite. In this study, the venoms of 26 of sub-Saharan A

A study of an endothelin antagonist from a Chinese anti-snake venom medicinal herb.

Because it is well known that endothelin (ET) plays an important role in the pathogenesis of cardiovascular diseases, antagonists of ET for clinical use are very important. Because ET and some snake toxins have a homologous structure and similar biologic actions the effect of Chinese anti-snake venom herbal medicines on ET bioactivity was investigated both in vivo and in vitro. Hong Bei Si Chou [Cissus assamica (Laws.) Craib] is a herbal medicine used to treat snake bite in Guangxi province. It was found that all the different fractions of EtOH extraction, the EtOAc part of the EtOH extraction

Morbidity and mortality following envenomation by the common night adder (Causus rhombeatus) in three dogs.

In South Africa dogs are frequently presented to veterinarians following snakebite. The offending snakes are usually puff adders (Bitis arietans), cobras (Naja spp.) and mambas (Dendroaspis spp.). Night adder (Causus rhombeatus) bites in dogs have not yet been reported in South Africa. This article deals with three cases of dogs bitten by night adders in which extensive tissue damage was noted and one fatality occurred. Night adder bites may be indistinguishable from puff adder bites. Non-specific treatment included addressing the hypovolaemia and swelling. Specific treatment involving immunot

A transcriptomic view of the proteome variability of newborn and adult Bothrops jararaca snake venoms.

BACKGROUND: Snake bite is a neglected public health problem in communities in rural areas of several countries. Bothrops jararaca causes many snake bites in Brazil and previous studies have demonstrated that the pharmacological activities displayed by its venom undergo a significant ontogenetic shift. Similarly, the venom proteome of B. jararaca exhibits a considerable variation upon neonate to adult transition, which is associated with changes in diet from ectothermic prey in early life to endothermic prey in adulthood. Moreover, it has been shown that the Brazilian commercial antibothropic a…

First authenticated cases of life-threatening envenoming by the hump-nosed pit viper (Hypnale hypnale) in India.

In Kerala, south-western India, five patients developed systemic envenoming after bites by hump-nosed pit vipers (Hypnale hypnale), proved by identification of the snakes responsible. Two of the dead snakes had been misidentified as saw-scaled vipers (Echis carinatus), while three had remained unidentified. Symptoms of local envenoming were pain, swelling, haemorrhagic blistering, bruising and regional lymphadenopathy. Systemic symptoms included headache, nausea, vomiting and abdominal and chest pain. There was evidence of haemostatic dysfunction (coagulopathy, fibrinolysis, thrombocytopenia o
