Mapping typical and hypokinetic dysarthric speech production network using a connected speech paradigm in functional MRI.

We developed a task paradigm whereby subjects spoke aloud while minimizing head motion during functional MRI (fMRI) in order to better understand the neural circuitry involved in motor speech disorders due to dysfunction of the central nervous system. To validate our overt continuous speech paradigm, we mapped the speech production network (SPN) in typical speakers (n = 19, 10 females) and speakers with hypokinetic dysarthria as a manifestation of Parkinson disease (HKD; n = 21, 8 females) in fMRI. We then compared it with the SPN derived during overt speech production by 15O-water PET in the …

Speech and language therapy intervention in schizophrenia: a case study.

BACKGROUND: There is a significant body of evidence documenting the speech and language abnormalities found in adult psychiatric disorders. These speech and language impairments can create additional social barriers for the individual and may hinder effective communication in psychiatric treatment and management. However, the role of speech and language therapy in this patient population has not been extensively studied. AIMS: Speech and language therapy is reported in a 53-year-old adult male patient who presented with severe poverty of speech as part of his diagnosis of schizophrenia. The a

Speech Motor Deficits in Developmental Stuttering and Parkinson's Disease: From Cellular and Circuit to Network-Level Disruptions.

Speech dysfluency occurs across multiple motor disorders to varying degrees, yet the underlying mechanisms remain incompletely understood. Like locomotion, speech production is relies on basal ganglia circuits, and disruptions in these pathways can lead to varied symptomatic presentations. In this perspective, we discuss evidence from cellular, circuit and network-level studies to highlight shared and disorder-specific mechanisms of speech dysfluency in Parkinson's disease and developmental stuttering. Basal ganglia function and dopaminergic modulation are central to both disorders, while cort

Initial evidence of vowel space reduction in a subset of individuals with schizophrenia.

OBJECTIVE: Acoustic phonetic measures have been found to correlate with negative symptoms of schizophrenia, thus offering a path toward quantitative measurement of such symptoms. These acoustic properties include F1 and F2 measurements (affected by tongue height and tongue forward/back position, respectively), which determine a general "vowel space." Among patients and controls, we consider two phonetic measures of vowel space: average Euclidean distance from a participant's mean F1 and mean F2, and density of vowels around one standard deviation of mean F1 and of F2. METHODS: Structured and

Design and Evaluation of Korean Electropalatography (K-EPG).

Recently, the development of medical rehabilitation technology has resulted in an increased interest in speech therapy equipment. In particular, research on articulation therapy for communication disorders is being actively conducted. The existing methods for the diagnosis and treatment of speech disorders, such as traditional tactile perception tests and methods based on the empirical judgment of speech therapists, have many limitations. Moreover, the position and contact force of the tongue are key factors in speech disorders with regards to articulation. This is a very important factor in t…

Management of velopharyngeal disorders. A case series.

Patients with acquired defects or congenital malformations of the palate exhibit disturbances in speech, including hypernasality, nasal emission, and decreased intelligibility of speech. Maxillofacial prosthetic treatment can reestablish the palatopharyngeal integrity to provide the potential for acceptable speech. This article describes a case series of patients with palatopharyngeal disorders and their treatment approaches.

A highly penetrant form of childhood apraxia of speech due to deletion of 16p11.2.

Individuals with heterozygous 16p11.2 deletions reportedly suffer from a variety of difficulties with speech and language. Indeed, recent copy-number variant screens of children with childhood apraxia of speech (CAS), a specific and rare motor speech disorder, have identified three unrelated individuals with 16p11.2 deletions. However, the nature and prevalence of speech and language disorders in general, and CAS in particular, is unknown for individuals with 16p11.2 deletions. Here we took a genotype-first approach, conducting detailed and systematic characterization of speech abilities in a

Diagnostic Assessment of Childhood Apraxia of Speech Using Automatic Speech Recognition (ASR) Methods.

We report findings from two feasibility studies using automatic speech recognition (ASR) methods in childhood speech sound disorders. The studies evaluated and implemented the automation of two recently proposed diagnostic markers for suspected Apraxia of Speech (AOS) termed the Lexical Stress Ratio (LSR) and the Coefficient of Variation Ratio (CVR). The LSR is a weighted composite of amplitude area, frequency area , and duration in the stressed compared to the unstressed vowel as obtained from a speaker's productions of eight trochaic word forms. Composite weightings for the three stress para

Factors associated with the development of psychiatric illness in children with early speech/language problems.

This paper reports on the correlates of psychiatric illness in children with speech and language disorders. Of 600 children selected from a community speech clinic, 50% were found to have diagnosable psychiatric disorders according to DSM-III criteria. These "psychiatrically ill" children were compared to the "psychiatrically well" children in the sample on a variety of developmental, socioeconomic, medical, and psychosocial factors. Although some differences were found between the well and ill children in other factors, the majority of the differences, and the most highly significant differen

The application of naturalistic conversation training to speech production in children with speech disabilities.

The purpose of this experiment was to test the effectiveness of including speech production into naturalistic conversation training for 2 children with speech production disabilities. A multiple baseline design across behaviors (target phonemes) and across subjects (for the same phoneme) indicated that naturalistic conversation training resulted in improved spontaneous speech production. The implications of these findings are discussed relative to existing models of speech production training and other aspects of communication disorders.

Increasing Educator Resources for Mild Cognitive Impairment and Dementia Using a Knowledge-to-Action Framework: The Development of DementiaBank Grand Rounds.

PURPOSE: An educational gap exists in content knowledge and clinical education for speech-language pathologists concerning the treatment and assessment of cognitive-communication disorders from mild cognitive impairment (MCI) or dementia due to Alzheimer's disease (AD). This gap may result in less effective speech-language pathology services for this population. The overarching goal of this project was to enhance graduate speech-language pathology education about MCI and dementia due to AD to improve the quality of future service providers. METHOD: To achieve this goal, this project had two o

Anatomical and neuromuscular maturation of the speech mechanism: evidence from acoustic studies.

This paper surveys acoustic studies of speech development and discusses the data with respect to the anatomical and neuromuscular maturation of the speech mechanism. The acoustic data on various aspects of speech production indicate that the accuracy of motor control improves with age until adult-like performance is achieved at about 11 or 12 years, somewhat after the age at which speech sound acquisition usually is judged to be complete. Other topics of discussion are (1) problems in the spectrographic analysis of children's speech, (2) formant scale factors that relate children's and adults'

Speech results following late palatal surgery in previously unoperated Sri Lankan adolescents with cleft palate.

The speech of 18 Sinhala speaking patients over 11 years of age with previously unrepaired palatal clefts is presented preoperatively, 8 months postoperatively, and after two brief courses of speech therapy (8 and 12 months postoperatively). Speech was usually severely disordered in the unoperated patient. The postoperative results show that speech often remains disordered following surgery, but there is the potential for change in some patients when supported by intensive goal directed therapy.

Is there a schizophasia? A study applying the single case approach to formal thought disorder in schizophrenia.

It has been suggested that formal thought disorder, the incoherent speech of schizophrenia, may involve a language disturbance among other abnormalities, or even be a form of dysphasia. Six patients with and seven without formal thought disorder were evaluated on an aphasia test battery. Spontaneous speech was also analysed using Brief Syntactic Analysis. Poor performance on the aphasia test battery was found to be associated with general intellectual impairment but not with formal thought disorder. Naming was preserved in both groups. Patients with formal thought disorder, but not those witho
