Between 1982 and 1985, idiopathic scoliosis was treated by instrumentation using Armstrong's technique, segmental spinal instrumentation (Luque's method) being reserved for neuromuscular scoliosis. Of the 29 patients treated by the technique 22 have been followed up for more than 18 months. Results were compared with those in a similar group of 30 patients operated upon using Harrington's original procedure between 1977 and 1982. This retrospective study complements that of Michel et al., and provided the following results: Although the instrumental correction gain was comparable in the two gr

Recent work has highlighted functional correlations between direct measures of head and neck posture and primate cervical bony morphology. Primates with more horizontal necks exhibit middle and lower cervical vertebral features that indicate increased mechanical advantage for deep nuchal musculature and mechanisms for column curvature formation and maintenance. How features of the C1 and C2 reflect quantified measures of posture have yet to be examined. This study incorporates bony morphology from the upper cervical levels from 20 extant primate species in order to investigate further how post

OBJECT: In the surgical management of adolescent idiopathic scoliosis (AIS), patients are often preoperatively informed that they will gain height as a result of their surgery. However, current estimations conflict significantly and do not have any clinical correlation. The authors developed a formula that would predict postoperative gains in height after deformity correction in AIS.
METHODS: A large, multicenter, prospective database was retrospectively queried for AIS patients with Lenke Type 1, 2, or 3 curves having undergone posterior spinal fusion alone. A univariate and multivariate ana

Thoracic volume was calculated in 50 adolescent patients operated on for severe idiopathic thoracic scoliosis. In 25, anterior instrumentation was used (group 1), and posterior instrumentation in the other 25 patients (group 2). Calculation of thoracic volume was made from measurements of pre-operative and post-operative radiographs. The mean spinal curvature in group 1 was 73+/-12.4 degrees before the operation, and 19+/-15 degrees after the operation, and in group 2 the curvature was 75+/-13 degrees before the operation and 37+/-10 degrees after the operation. The calculated thoracic volume

CASE DESCRIPTION: A 2-year-old alpaca was evaluated because of acute onset of cervical scoliosis.
CLINICAL FINDINGS: Physical examination revealed severe scoliosis of the caudal portion of the cervical vertebral column with a C-shaped curvature to the right side. No gait deficits were observed. Cervical radiography confirmed severe curvature of C4 to C6 but did not reveal any bony changes. Cerebrospinal fluid had high total protein concentration and extremely high nucleated cell count with a high proportion of eosinophils, suggesting parasitic infection.
TREATMENT AND OUTCOME: The alpaca was

BACKGROUND: Prioritizing patients using empirically derived access targets can help to ensure high-quality care. Adolescent scoliosis can worsen while patients wait for treatment, increasing the risk of adverse events. Our objective was to determine an empirically derived access target for scoliosis surgery and to compare this with consensus-based targets.
METHODS: Two-hundred sixteen sequential patients receiving surgery for adolescent idiopathic scoliosis were included in the study. The main outcome was need for additional surgery. Logistic regression modeling was used to evaluate the relat

PURPOSE: To propose a robust and accurate method for straightening magnetic resonance (MR) images of the spinal cord, based on spinal cord segmentation, that preserves spinal cord topology and that works for any MRI contrast, in a context of spinal cord template-based analysis.
MATERIALS AND METHODS: The spinal cord curvature was computed using an iterative Non-Uniform Rational B-Spline (NURBS) approximation. Forward and inverse deformation fields for straightening were computed by solving analytically the straightening equations for each image voxel. Computational speed-up was accomplished b

BACKGROUND CONTEXT: Current research on mental health in adult spinal deformity (ASD) patients remains relatively scarce, particularly regarding specific psychological factors including anxiety, kinesiophobia and pain catastrophizing. These negative psychological factors have been associated with worse pain and function in studies of patients with common lumbar diseases (CLD). It is currently unknown whether these psychological factors play a similar role in ASD patients. A comprehensive understanding of adverse psychological conditions within the ASD population and their associations with phy

BACKGROUND CONTEXT: High mechanical stress at the lumbosacral junction (LSJ) contributes to elevated failure rates in long-segment adult spinal deformity (ASD) fusions. To enhance fusion across the LSJ and preserve correction after ASD surgery, pelvic fixation has become a critical component of surgical constructs. However, conventional techniques that use a single point of fixation per side remain prone to implant failure. Biomechanical studies indicate that multiple-point pelvic fixation (MPF) may improve load distribution and construct stability, yet supporting clinical evidence remains lim

BACKGROUND: The etiology of the disabling symptom camptocormia/dropped head in Parkinson's disease (PD) is still debated. Many PD patients develop involuntary flexion of the spine in an upright position that does not respond to dopaminergic treatment.
OBJECTIVES: To study the histopathology of paraspinal muscles in PD-associated camptocormia with potential consequences for therapy.
METHODS: We report a cohort of 66 patients with PD (mean age 68.4 ± 8.1 years) and associated camptocormia according to consensus criteria. Paravertebral muscle biopsy was performed in all patients with consecutiv

STUDY DESIGN: Retrospective study.
OBJECTIVES: To investigate the incidence and risk factors of mechanical failure (MF) following anterior column realignment (ACR) in patients with severe degenerative sagittal imbalance (DSI).
SUMMARY OF BACKGROUND DATA: Considering the biomechanical properties of the procedure, ACR may increase the risk of MF, including proximal junctional kyphosis (PJK) and rod fracture (RF). However, this issue has been poorly documented in the literature.
METHODS: We included patients aged ≥60 years with severe DSI radiographically defined by pelvic incidence (PI)-lumba

BACKGROUND: The incidence of rod fracture after corrective surgery for adult spinal deformity (ASD) is high. Although many reports have investigated the effects of rod bending considering postoperative body motion, and countermeasures, there are no reports investigating the effects during intraoperative correction. The purpose of this study was to investigate the effect of ASD correction on rods by using finite element analysis (FEA) based on the rod shape changes before and after spinal corrective fusion.
METHODS: Five ASD patients (mean age 73 years, all female) who underwent thoracic to pe

Deep brain stimulation (DBS) has become a well-established treatment modality for Parkinson's disease (PD), especially regarding motor fluctuations, dyskinesias, and tremor. Although postural abnormalities (i.e., Camptocormia [CC] and Pisa syndrome [Pisa]) are known to be a major symptom of PD as well, the influence of DBS on postural abnormalities is unclear. The objective of this study is to analyze the existing literature regarding DBS for PD-associated postural abnormalities in a systematic review and meta-analysis.
