Practices in clearance of the pediatric cervical spine following blunt trauma: A Western Pediatric Cervical Spine Study analysis.

BACKGROUND: Clearance of the pediatric cervical spine (CS) may pose a challenge following blunt trauma. Prior studies suggest that less than half of pediatric trauma centers (PTCs) have a written protocol for CS clearance. This study aimed to evaluate national practices for pediatric CS clearance in adult trauma centers (ATCs) and PTCs. METHODS: Western Pediatric Cervical Spine Study members were queried for hospital demographics and current CS clearance practices. Data included trauma center verification level and the number of yearly trauma activations. Study members provided information ab…

The forgotten spine score: study protocol for a novel patient-centered outcome measure in lumbar spine fusion surgery.

Lumbar spine fusion (LSF) surgeries have increased due to the rising prevalence of degenerative spine disorders and advancements in surgical techniques. However, the success of these procedures should be evaluated from the patient's perspective, emphasizing the need for effective patient-reported outcome measures (PROMs). Existing PROMs often focus on symptoms and lack specific validation for surgical patients undergoing LSF. Inspired by the Forgotten Joint Score used in joint replacement surgery, here we introduce the Forgotten Spine Score (FSS), a new patient-centered PROM designed to assess…

Compensatory mechanism of the spine after corrective surgery without lumbar-sacral fixation for traumatic thoracolumbar kyphotic spine deformity.

BACKGROUND: It remains unclear whether long fusion including lumbar-sacral fixation is needed in corrective surgery to obtain good global sagittal balance (GSB) for the treatment of traumatic thoracolumbar kyphotic spine deformity. The purposes of this study were to evaluate compensatory mechanism of the spine after corrective surgery without lumbar-sacral fixation and to evaluate the parameters affecting the achievement of good GSB post-operatively. METHODS: Twenty (20) subjects requiring corrective surgery (distal end of fixation was L3) were included in this study. The radiographic paramet…

Types and prevalence of coexisting spine lesions on whole spine sagittal MR images in surgical degenerative spinal diseases.

PURPOSE: We investigated types and prevalence of coexisting lesions found on whole spine sagittal T2-weighted images (WSST2I) acquired from magnetic resonance imaging (MRI) and evaluated their clinical significance in surgical degenerative spinal diseases. MATERIALS AND METHODS: Coexisting spinal lesions were investigated using WSST2I from 306 consecutive patients with surgical degenerative spinal diseases. Severity of coexisting lesions was classified into four grades (0-3). Lesions of grade 2 and 3 were defined as "meaningful coexisting spine lesions" (MCSL). Degenerative spinal diseases we…

Redefining the applications of navigation in spine surgery.

Intraoperative navigation in spine surgery has evolved into an increasingly versatile platform that enhances precision, safety, and operative workflow across a variety of surgical procedures. In the setting of burst fractures, it improves spatial awareness during vertebrectomy, ensuring complete disc removal, accurate endplate preparation, and safe anterior column reconstruction. In sacral and metastatic tumors, navigated osteotomies facilitate precise margin definition and tumor resection. Navigated deployment of interbody and expandable corpectomy cages optimizes positioning and reduces fluo…

Role of MR Imaging in Spine Infection.

This article brings to light the importance of MR imaging not only as a tool for the diagnosis of spine and spinal cord infections but also for identifying potential complications that can increase morbidity and mortality. The authors explore the typical imaging characteristics of common and infrequent spine infections and discuss their relevant pathophysiology and clinical features. Infections are categorized by the affected anatomic structure and by the pathogenic microorganisms, such as bacteria, fungi, parasites, and viruses. It also highlights the role of MR imaging in detecting complicat…

Spine and Pelvic Issues in the Baseball Player.

Spine and pelvis issues in Major League Baseball do not often get the publicity that other, more common injuries do. However, despite low incidence rates involving significant time loss, there is a very high percentage of players who report having some lumbopelvic discomfort at some point in their careers. Baseball athlete's bodies have unique clinical baseline presentations throughout the kinetic chain representing the unique demands of the game. The authors aim to outline a plan that can identify potential problematic presentations, implement an efficient and effective plan of care that will…

An unusual case of gunshot injury to the spine.

An eighteen-year-old male was brought with a gunshot to the right thorax. There was reduced air entry on the right chest, and the patient had AIS A neurology. However, plain radiographs of the spine were normal. CT of the chest revealed the trajectory of the bullet, which entered through the T7 right neural foramen, transected the cord, and exited the left foramen with minimal osteoligamentous disruption. The patient was managed conservatively, and once his respiratory function recovered, he underwent rehabilitation. This is the first report describing a low-velocity gunshot injury, causing co…

Updates in the Management of Metastatic Spine Disease.

The number of cancer diagnoses continues to increase each year in the United States, and given the propensity for bone metastases from solid organ malignancies, orthopaedic spine surgeons will inevitably encounter patients with metastatic spine disease and need to have a framework for approaching the evaluation and treatment of these complex patients. Many patients seeking care for spinal metastases already have a history of disseminated malignancy, but metastatic spine disease itself will be the presenting symptom of cancer in approximately 20% of patients. Because the first presentation of c…

Osteoporosis Evaluation and Management in Spine Surgery.

Osteoporosis is a global health problem affecting over 200 million people worldwide and 54 million adults in the United States. Approximately half of all postmenopausal women will have an osteoporosis-related fracture during their lifetime. In the United States, the direct medical cost related to osteoporosis is expected to exceed $25 billion by 2025. Management of osteoporosis in vertebral fractures and elective spine surgery is of the utmost concern, given the high prevalence of osteoporosis in the general population and the increased risk of complication in this population. New pharmacologi…

Contiguous Burst Fractures of the Lumbar Spine.

Burst fractures of vertebrae are usually caused by high-energy axial compression force, mostly caused by fall from height or road traffic ac … ne-year follow-up, the patient was pain-free with complete neurological recovery. Contiguous lumbar spine burst fractures are very rare in occurrence. Though burst fractures are managed surgically to provide stability, the surgical approaches depend on the individual fracture pattern, degree of spinal canal occlusion, and neurological status.
