Ruptura esplénica después de terapia anticoagulante en un paciente con antecedente de COVID-19: reporte de caso.

We present the case of a 75-year-old man with a history of COVID-19 and splenic infarct treated with enoxaparin, who presented with intense abdominal pain and tomographic findings of free peri-splenic fluid and a hyperdense image in the spleen. An emergency laparotomy was performed, with findings of a splenic rupture at the vascular hilum. Spontaneous splenic rupture is a rare and fatal entity that should be suspected in a patient with history of COVID-19 who presents with acute abdominal pain after the administration of heparin.

Recurrent ruptured spleen.

Regrowth of splenic tissue after splenectomy for trauma and splenectomy for idiopathic thrombocytopenia purpura have been reported. However, rupture of splenic tissue, either spontaneous or traumatic, that requires a second surgical intervention for hemoperitoneum caused by a ruptured splenic nodule or splenosis has rarely been reported. We report the case of a 43-year-old man in whom hemoperitoneum developed 25 years after he underwent an open splenectomy, after a motor vehicle accident, that required removal of a recurrent ruptured splenotic nodule.

Spontaneous rupture of the spleen secondary to amyloidosis.

BACKGROUND: Rupture of the spleen has been classically associated with trauma. It sometimes is a spontaneous event or may be secondary to a pathological condition of the spleen or even in a healthy spleen. Systemic amyloidosis is characterized by the extracellular deposition of amyloid proteins in one or more organs. The spleen can be affected in 41% of patients. We undertook this study to present a case of spontaneous splenic rupture secondary to amyloid infiltration of the spleen, an uncommon condition in the literature. CLINICAL CASE: We present the case of a 46-year-old male with abdomina…

[The "splenic emergency syndrome" during pregnancy (a report of 2 cases)].

The syndrome "urgency of lienal origin" during a pregnancy consists of two syndromes--violent spontaneous pain in the left hypochondrium or … cases: the first one--a case of spontaneously ruptured splenic artery aneurysm, the second one--of splenic rupture. The first of two cases came of successful for both mother and baby. This is the sixth similar case described in literature till 1978. The second case presented--a spontaneous lienal rupture, is very rare. The favourable prognosis is entirely determ …

Medico-legal difficulties in a death due to delayed traumatic rupture of massive sarcoid spleen with rivaroxaban therapy.

Splenic rupture is a potentially fatal event, most frequently associated with blunt abdominal trauma. We hereby present an unusual post-mortem case of delayed splenic rupture occurring 5.5 days after a single punch to the chest. The case was further complicated by massive sarcoid splenomegaly and rivaroxaban therapy prescribed for prior deep vein thrombosis. Detailed histopathological examination of an overlying rib fracture and an organizing splenic hematoma revealed healing changes entirely consistent with the reported injury timeframe. The combined effects of trauma, underlying granulomatou…

Traumatic splenic cyst.

The authors report two cases of traumatic rupture of splenic cyst. In each case a previous abdominal trauma was found responsible for the presence of the cyst. A new minor abdominal trauma provoked a major haemoperitoneum. In both cases laparotomy revealed the rupture of a large cyst of the upper pole of the spleen, with extension of the lesion into healthy splenic parenchyma. Conservative splenic surgery with cystectomy was possible in both cases; microscopic examination confirmed a true pseudocyst in case 2. In presence of haemoperitoneum and ultrasonographic view of a splenic cyst, the trea

[Endovascular treatment of incidental and emergency splenic aneurysm].

True splenic aneurysms are saccular dilations of all the layers of the splenic artery, more common in women, pregnancy and portal hypertension. They are usually asymptomatic and diagnosed incidentally during the study of other abdominal diseases. Up to 10% may present with rupture, which implies a high morbidity and mortality. Treatment of splenic aneurysms is still a subject of controversy and there is a great variety of therapeutic modalities. We present two cases of patients with splenic aneurysms: one who presented with rupture and the other one incidentally diagnosed. Both were treated wi

The development and clinical features of splenic aneurysm associated with liver cirrhosis.

OBJECTIVES: Splenic artery aneurysm (SAA) is usually asymptomatic, but can be fatal if it ruptures. Portal hypertensive patients with varix or splenomegaly are sometimes complicated by SAA. However, there have been no large-scale clinical studies regarding whether liver cirrhosis itself is associated with splenic aneurysm regardless of varix or splenomegaly. METHODS: In the present study, we retrospectively analyzed 303 cirrhotic patients examined with arteriography. The diagnosis and characteristics of SAAs were determined, and the relation with splenic artery diameter was evaluated. RESULT

[Secondary rupture of the spleen after external cardiac massage].

A case of delayed splenic rupture occurring 18 days after external cardiac massage is reported. A 30 year old woman underwent mitral valve replacement. Two cardiac arrests in the immediate postoperative period were successfully treated by external cardiac massage. Recovery was uneventful until the 18th day, when the patient began to suffer from dizziness. Haemodynamic and biological data suggested hypovolaemia by dehydration. The day after, her abdomen became painful and anaemia was discovered. Splenic rupture was diagnosed by abdominal ultrasonography and splenectomy was carried out; no other…

Delayed diagnosis of angiosarcoma of the spleen: clinically presenting as recurrent haemoperitoneum following embolisation.

Angiosarcoma is a highly aggressive and extremely rare neoplasm of the spleen, with a limited number of reported cases worldwide. We report a case of a 69-year-old man who presented with progressive symptoms of fatigue, abdominal and back pain, anaemia and haemoperitoneum 1 year following embolisation of the spleen for splenic rupture. Additional imaging and colonoscopy with biopsy revealed intra-abdominal fluid and a colonic adenocarcinoma. Diagnostic laparoscopy demonstrated a haemoperitoneum and colon carcinoma, after which splenectomy via laparotomy was performed. Pathology report revealed…

[A case of primary splenic angiosarcoma with intraperitoneal hemorrhage treated by transcatheter arterial embolization].

Splenic tumors are very rare. In Japan only 42 cases of splenic angiosarcoma have been reported. We encountered a case of spontaneous rupture of a splenic angiosarcoma and liver metastasis. A 60-year-old woman who suddenly went into hemorrhagic shock presented at our hospital. Then acute spontaneously ruptured spleen and hepatic tumors were diagnosed by abdominal CT. After emergency TAE, the patient was hemodynamically stable, but died of liver failure 13 days after admission. The pathological diagnosis was primary splenic angiosarcoma with multiple organ metastasis on autopsy. Splenic angiosa

A rare case of sequential visceral artery aneurysm rupture associated with multifocal vascular abnormalities.

Visceral artery aneurysms (VAAs) are rare but potentially fatal, with rupture representing their most serious complication. We report the case of a previously healthy 35-year-old woman who presented to the emergency department with acute abdominal pain caused by rupture of a splenic artery aneurysm, resulting in massive hemoperitoneum. She underwent emergency laparotomy with splenectomy, but approximately one hour postoperatively developed recurrent hemorrhagic shock, necessitating a second surgical intervention with proximal splenic artery ligation. Postoperative CT revealed multiple visceral

Non-invasive assessment of esophageal and fundic varices in patients with primary biliary cholangitis.

OBJECTIVES: The Baveno VII consensus recommends endoscopic screening for varicose veins in cases of liver stiffness measurement (LSM) ≥ 20 kPa or platelet count ≤ 150 × 109/L. Whether this approach was appropriate for patients with primary biliary cholangitis (PBC) remains uncertain. This study expanded the observed risk factors by adding analysis of ultrasound images as a non-invasive tool to predict the risk of esophageal or fundic varices. METHODS: We enrolled 111 patients with PBC whose complete ultrasound images, measurement data, and LSM data were available. The value of the periportal

Spontaneous splenic haematoma in a teenager with infectious mononucleosis.

Spontaneous subcapsular splenic hematoma formation without rupture in infectious mononucleosis is a very unusual occurrence. Splenic rupture in infectious mononucleosis (Pfeiffer disease, or glandular fever) is a rare but life-threatening complication. A conservative management is successful in those patients in stable condition. We describe the presentation and the clinical progress of a case in whom a sudden enlargement in hematoma's diameter needed a splenectomy to avoid the risk of blood effusion in the abdominal cavity. The contribution of the sonographic examination and follow-up in the …
