Alzheimer Disease: The proposed role of tanycytes in the formation of tau tangles and amyloid beta plaques in human brain.

Currently, the causes for Alzheimer Disease (AD) are thought to lie in the formation of abnormal protein deposits including tau tangles and Amyloid ß (Aβ) plaques in the human cortex. These proteins are believed to accumulate in the brain due to impaired waste removal resulting in neurodegeneration. In an alternative hypothesis we have recently proposed the existence of an aquaporin4 aqua channel (AQP4)-expressing tanycyte-derived canal network that likely internalizes waste for removal from the brain. We propose that both Aβ and tau protein may play important structural roles in this canal sy

Single-Atom Synergy of Luminescence and Catalytic Stability in Silver-Encapsulated Gold Nanoclusters.

The classic "silver effect" in gold(I) catalysis has long been confined to its role as a halide abstractor or transient stabilizer of catalytic intermediates. Here, we fundamentally redefine this concept by demonstrating that trace residual Ag(I) ions from halide abstraction of Ph3PAuCl act as a dynamic template to drive the hierarchical assembly of organogold(I) precursors into a high-nuclearity bimetallic nanocluster [Au11Ag1]. Alternatively, an independently synthesized hollow [Au11] nanocage can selectively and strongly encapsulate a single silver(I) ion (Ka ∼ 105 M-1) to reproduce [Au11Ag

Structural Evolution and Resilience of Digital Economy Ecosystems: A Joint Temporal Exponential Random Graph Model and Motter-Lai Framework Approach.

Existing static analysis methods neglect the structural evolution of complex network topologies and the cascading failures induced by local load redistribution, leading to assessment biases in the analysis of the resilience of digital economy ecosystems. To accurately quantify the system's resilience threshold, this paper proposes a physical computation framework combining a joint Temporal Exponential Random Graph Model (TERGM) and an improved Motter-Lai algorithm (TERGM-ML). This framework utilizes Markov Chain Monte Carlo Maximum Likelihood Estimation (MCMC-MLE) to model endogenous structura

Brain Endothelial Glycocalyx as a Blood-Facing Translational Interface in Alzheimer's Disease: Beyond "Leaky" Barriers Toward Repair-First Stratification.

Alzheimer's disease (AD) pathogenesis is increasingly recognized as involving blood-brain barrier (BBB) and neurovascular unit (NVU) destabilization. The brain endothelial glycocalyx-a blood-facing glycan-rich interface-represents a critical but under-characterized determinant of BBB dysfunction in AD. In this review, we systematically reappraised glycocalyx abnormality mechanisms, distinguishing robust causal evidence from murine models from limited human observational data. We propose a four-dimensional interface-state framework (structural, glycosylation, transport, inflammatory) to stratif

Surgical management of macular telangiectasia type 2-associated macular hole with internal limiting membrane deficiency and imaging assessment of müller cell function.

This study presents a rare case of macular telangiectasia type 2 (Mac Tel 2) with a secondary full-thickness macular hole, in which an unexpected internal limiting membrane (ILM) deficiency was discovered intraoperatively. Swept-source OCT angiography (SS-OCTA) volumetric data were utilized to construct 3D models for the assessment of Müller cell function. Although ILM flap surgery achieved initial anatomical closure and visual improvement, the hole reopened after six years, with corresponding vision decline. Intraoperative ICG staining revealed patchy ILM absence, indicating severe Müller cel

Left ventricular pseudoaneurysm as a rare catastrophic complication to surgical repair of mitral valve endocarditis.

INTRODUCTION: Left ventricular pseudoaneurysm (LVPA) is rarely described in the medical literature, as a complication of mitral valve repair after infective endocarditis (IE) and only a few cases have been published until now. CASE PRESENTATION: We reported a case of a woman in her forties without past medical history who underwent a successful surgical repair of mycotic pseudoaneurysm at the apex of the left ventricular after two months of mitral mechanical prosthetic valve replacement due to infective endocarditis. DISCUSSION: Infective endocarditis (IE) remains a rare condition with high

Editorial Commentary: Olecranon Bone Autografting Combined With an Arthroscopic Approach for the Treatment Scaphoid Nonunion-An Important Technique for Wrist Surgeons.

Effective treatment of scaphoid pseudoarthrosis is critical to reduce the risk of progression to the potentially debilitating scaphoid nonunion advanced collapse, including complications of persistent wrist joint instability, degenerative arthritis, decreased range of motion, chronic pain, and functional impairment. Both anatomic and fracture-related pathophysiology predispose patients to scaphoid nonunion, including limited retrograde blood flow, fracture location, and delay of appropriate treatment. Recent studies have demonstrated successful outcomes in treatment of scaphoid nonunions, with

Early and Late Pathomechanisms in Alzheimer's Disease: From Zinc to Amyloid-β Neurotoxicity.

There are several systemic and intracerebral pathologic conditions, which limit provision and utilization of energy precursor metabolites in neuronal cells. Energy deficits cause excessive depolarization of neuronal cells triggering glutamate-zinc evoked excitotoxic cascade. The intracellular zinc excess hits several intraneuronal targets yielding collapse of energy balance and impairment functional and structural impairments cholinergic neurons. Spreader Grafts in Functional Rhinoplasty.

Management of the middle vault is paramount to achieving optimal aesthetic and functional outcomes in rhinoplasty. The ideal treatment for middle vault complications, such as internal nasal valve collapse, inverted-V deformity, and middle vault dorsal asymmetry, is prevention. Risk factors for middle vault problems in rhinoplasty that may be identified in preoperative consultation include short nasal bones, long and weak upper lateral cartilages, thin skin, previous trauma or surgery, preoperative positive Cottle maneuver, tension nose deformity, and anteriorly positioned inferior turbinates.

Blast Injuries.

BACKGROUND: Blast injuries in the United States and worldwide are not uncommon. Partially due to the increasing frequency of both domestic and international terrorist bombing attacks, it is prudent for all emergency physicians to be knowledgeable about blasts and the spectrum of associated injuries. OBJECTIVE: Our aim was to describe blast physiology, types of blast injuries associated with each body system, and manifestations and management of each injury. DISCUSSION: Blast injuries are generally categorized as primary to quaternary injuries. Primary injuries result from the effect of trans

Cardiotoxicity of antimalarial drugs.

There are consistent differences in cardiovascular state between acute illness in malaria and recovery that prolong the electrocardiographic QT interval and have been misinterpreted as resulting from antimalarial cardiotoxicity. Of the different classes of antimalarial drugs, only the quinolines, and structurally related antimalarial drugs, have clinically significant cardiovascular effects. Drugs in this class can exacerbate malaria-associated orthostatic hypotension and several have been shown to delay ventricular depolarisation slightly (class 1c effect), resulting in widening of the QRS co

Vacuole formation in the endothelium of rat extremity vessels depends on fixation techniques and vessel type.

Applying immersion fixation for electron microscopy, huge clear endothelial membrane-bound vacuoles of 0.1-3 microm diameter were noted in the extremity veins of Sprague-Dawley rats. Histological and electron microscopic histochemical methods were applied to determine whether they were the product of programmed cell death or any other kind of cell damage. Image analyzer was used to measure the total area of the vacuoles in the endothelium cells. Neither lipid content nor acidic phosphatase activity could be identified in the vacuoles. In saphenous and brachial veins, the vacuoles occupied 20.6

Lumbar vacuum disc, vertical instability, standalone endoscopic interbody fusion, and other treatments: an opinion based survey among minimally invasive spinal surgeons.

BACKGROUND: A diseased lumbar intervertebral vacuum disc void of any structurally intact tissue may be vertically unstable. A primary standalone endoscopic decompression and interbody fusion surgery in the treatment of vertical instability in patients with a vacuum disc may be a more reliable treatment than decompression alone. METHODS: The authors solicited responses to an online survey sent to spine surgeons by email, and chat groups on social media networks, including Facebook, WeChat, WhatsApp, and Linkedin. Descriptive and correlative statistics were employed to count the responses and c

Pediatric Sleep Respiratory Disorders: A Narrative Review of Epidemiology and Risk Factors.

Sleep is a fundamental biological necessity, the lack of which has severe repercussions on the mental and physical well-being in individuals of all ages. The phrase "sleep-disordered breathing (SDB)" indicates a wide array of conditions characterized by snoring and/or respiratory distress due to increased upper airway resistance and pharyngeal collapsibility; these range from primary snoring to obstructive sleep apnea (OSA) and occur in all age groups. In the general pediatric population, the prevalence of OSA varies between 2% and 5%, but in some particular clinical conditions, it can be much

The Role of the Size and Location of the Tumors and of the Vertebral Anatomy in Determining the Structural Stability of the Metastatically Involved Spine: a Finite Element Study.

Vertebral fractures associated with the loss of structural integrity of neoplastic vertebrae are common, and determined to the deterioration of the bone quality in the lesion area. The prediction of the fracture risk in metastatically involved spines can guide in deciding if preventive solutions, such as medical prophylaxis, bracing, or surgery are indicated for the patient. In this study, finite element models of 22 thoracolumbar vertebrae were built based on CT scans of three spines, covering a wide spectrum of possible clinical scenarios in terms of age, bone quality and degenerative featur

Genomic Characteristics of Gender Dysphoria Patients and Identification of Rare Mutations in RYR3 Gene.

Gender dysphoria (GD) is characterized by an incongruence between the gender assigned at birth and the gender with which one identifies. The biological mechanisms of GD are unclear. While common genetic variants are associated with GD, positive findings have not always been replicated. To explore the role of rare variants in GD susceptibility within the Han Chinese population, whole-genome sequencing of 9 Han female-to-male transsexuals (FtMs) and whole-exome sequencing of 4 Han male-to-female transsexuals (MtFs) were analyzed using a pathway burden analysis in which variants are first collaps

Immunohistochemical and ultrastructural study of hepatic sinusoidal linings during dichloropropanol-induced acute hepatic necrosis.

An acute phase of severe hepatic necrosis induced by dichloropropanol was examined immunohistochemically and ultrastructurally, in order to study chronological changes of sinusoidal morphology during acute hepatic injury. Male Wistar rats were injected with 1,3-dichloro-2-propanol (DC2P) and sacrificed at various intervals after the injections. DC2P-injected rats showed zonal necrosis of the centrilobular space with a peak from 24 to 48 h after the injection. Destruction of sinusoidal linings appeared at 4 h, and was gradually aggravated along the advancing hepatocytic necrosis. Monocytic infl
