Pharmacokinetics and dose estimates following intrathecal administration of 131I-monoclonal antibodies for the treatment of central nervous system malignancies.

PURPOSE: Treatment of malignant disease in the central nervous system (CNS) with systemic radiolabeled monoclonal antibodies (MoAbs) is comp … d biphasic, with more rapid clearance occurring in the first 24 h. Radioimmunoconjugate entered the subarachnoid space and subsequently the vascular compartment. From this information, the areas under the effective activity curves for ventricular CSF, blood, and subarachnoid CSF were calculated to permit dosimetry. Critical organ doses were calculated using conventi …

Formation and resolution of brain edema associated with brain tumors. A comprehensive theoretical model and clinical analysis.

The purpose of this study is to quantify the relative contribution of the mechanisms in the absorption of edema fluid. The convection/diffusion and the comprehensive bulk flow model were applied for the finite element analysis of peritumoral brain edema. For clinical analysis, 90 meningiomas studied by MRI were selected. Serial CT scan and MRI were performed at 0, 2, 4, 6 hours after injection of Iopamidol or Gadpenteic acid respectively. Then the tracer distributions in the edematous brain was analyzed. The tracer movement in the brain is well represented by the convection/diffusion equation.

Apurulent bacterial meningitis (compartmental leucopenia in purulent meningitis).

Meningococci and Haemophilus influenzae may invade the subarachnoid space during the bacteriaemic phase without impairment of the blood-CSF barrier and in the absence of any leucocyte reaction. In pneumococcal meningitis the CSF may also contain less than 100 cells/microliter despite the presence of "pure bacterial cultures", but the barrier is completely broken when the serum/CSF concentration ratio is below 10. A clinical analysis of eight patients with fewer than 100 cells/microliter revealed that the first symptoms of meningitis appeared at least 3 days prior to the diagnostic lumbar punct…

The development of interependymal pores in the rhombencephalic posterior tela in late embryonic, larval and metamorphosing stages of Rana pipiens.

The structure of the roof of the fourth cerebral ventricle has been studied by scanning and transmission electron microscopy in Rana pipiens from the late embryonic stages, through the larval stages to metamorphosis. Stage 20 and 21 post-hatching embryos have a rhombencephalic roof consisting of undifferentiated embryonic cells. From embryonic stage 22-25 there is progressive differentiation into choroid plexus and posterior tela ependymal cells. Between 1 and 3 interependymal pores occur in the posterior tela and there is a small incipient subarachnoid space dorsal to the posterior tela. Duri…

Cerebrospinal fluid outflow resistance in rabbits with experimental meningitis. Alterations with penicillin and methylprednisolone.

Acute bacterial meningitis may be associated with increased intracranial pressure, neurological sequelae such as communicating hydrocephalus, and a slow response to antibiotic therapy. Alterations in cerebrospinal hydrodynamics are at least partially responsible for these complications. Constant, low-flow short-duration manometric infusion studies through a hollow-bore pressure monitoring device in direct continuity with the supracortical subarachnoid space were performed in rabbits with experimental meningitis. Maximal resistance to cerebrospinal fluid (CSF) outflow from the subarachnoid to v…

Different Glymphatic-Lymphatic Coupling in the Nasal Mucosa and Parasagittal Dura.

BACKGROUND: Glymphatic-lymphatic coupling is difficult to visualize in humans. PURPOSE: To evaluate the transport from the basal subarachnoid space to lymphatic vessels in the nasal mucosa and in the parasagittal dura, respectively. METHODS: A highly resolved 3D compressed sensing black blood sequence with almost isotropic resolution (0.5 ×0.5 ×0.6 mm 3 ) was acquired in 26 patients before and 2 to 4, 6 to 8, 24 to 48, and 72 to 96 hours after intrathecal injection of 0.5 mL gadobutrol. T1 signal intensities were measured in CSF spaces (perisylvian, above cribriform plate, midsylvian, and pa…

[Modern principles of neurocritical care].

Neurological diseases frequently demanding admittance to a dedicated neurological intensive care unit (neurocritical care) comprise space-occupying ischemic stroke, intracerebral hemorrhage, subarachnoid hemorrhage, traumatic brain injury, status epilepticus, bacterial meningitis, myasthenic crisis and Guillain-Barré syndrome. Due to often necessary analgesia, sedation and mechanical ventilation, neuromonitoring should ideally be employed. This consists of bedside invasive and non-invasive methods for monitoring cerebral perfusion, oxygenation, metabolism and neurophysiology. Modern treatment

Intracranial peripheral primitive neuroectodermal tumor manifesting as abducens nerve palsy.

INTRODUCTION: Peripheral primitive neuroectodermal tumor (PNET) occurring as an extra-axial lesion within the intracranial space and extending to the subarachnoid space is extremely rare. CASE REPORT: An 18-month-old girl presented with an intracranial peripheral PNET manifesting as abducens nerve palsy. Magnetic resonance imaging on admission revealed a lesion affecting the trigeminal and abducens nerves. The tumor was partially removed via the subtemporal approach. Histological examination showed a high-grade, undifferentiated neoplasm of small cell type with positive immunostaining for MIC…

Spontaneous spinal cerebrospinal fluid leak and intracranial hypotension.

A case of intracranial hypotension with spontaneous cerebrospinal fluid (CSF) leak was reported. A Tc-99m diethyltriaminepentacetic acid radionuclide cisternography (RNC) showed the accumulation of radioactivity in the area of the subarachnoid space, the poor migration of the isotope over the convexities, and the early appearance of kidney and bladder activity. To localize the site of CSF leak, RNC will be the choice, and when the time comes, RNC will work well in the location of the leak.

Cerebrovascular lesions in acquired immune deficiency syndrome (AIDS).

Cerebrovascular lesions were seen in 28 of 83 cases (34%) of acquired immune deficiency syndrome (AIDS). Cerebral hemorrhage was noted in 4 cases, cerebral infarct in 23 cases and both in 1 case. Cerebral hemorrhage was in various locations such as intraparenchymal, subarachnoid space, subdural space and epidural space. Large, clinically evident hemorrhage was noted in 2 of 5 cases and bleeding tendency was noted in 2 cases. Most of the 24 cases with cerebral infarcts were not clinically evident; they were multiple, small and mainly involved the striatum, cerebral cortex and brain stem. Mural …

Normal pressure hydrocephalus as an unusual presentation of supratentorial extraventricular space-occupying processes: report on two cases.

Normal pressure hydrocephalus (NPH) is a clinical and radiographic syndrome characterized by ventriculomegaly, abnormal gait, urinary incontinence, and dementia. The condition may occur due to a variety of secondary causes but may be idiopathic in approximately 50% of patients. Secondary causes may include head injury, subarachnoid hemorrhage, meningitis, and central nervous system tumor. Here, we describe two extremely rare cases of supratentorial extraventricular space-occupying processes: meningioma and glioblastoma multiforme, which initially presented with NPH.

Cerebrospinal fluid leak associated with nasal continuous positive airway pressure treatment for obstructive sleep apnoea.

Clear rhinorrhoea is a common symptom in patients with obstructive sleep apnoea (OSA), which may worsen with nasal continuous positive airway pressure treatment (nCPAP). However, rhinorrhoea can also be the presenting symptom of cerebrospinal fluid (CSF) leak, which is due to a communication between the subarachnoid space and the nasal cavity or sinuses. We report another case of a patient with OSA in whom CSF leak developed following the initiation of nCPAP treatment.

Posterior fossa subdural hematoma due to ruptured arteriovenous malformation. Case report.

Patients with cerebellar arteriovenous malformations (AVM) commonly present to the neurosurgical department after having suffered hemorrhages. The subarachnoid space is the usual location for these often repeating episodes of bleedings. In addition, these patients can present with parenchymal hemorrhage. Acute subdural hematoma caused by a ruptured cerebellar AVM is a rare entity and is not generally recognized. The authors present a case of acute posterior fossa subdural hematoma resulting from a ruptured cerebellar AVM.

Congenital malformation of the inner ear and recurrent meningitis. A case report.

Congenital deformities of the inner ear can be associated with meningitis and varying degrees of hearing loss or deafness. Recurrent periods of meningitis, sometimes lethal, are due to communication between the middle ear cavity and the subarachnoid space. Different types of congenital dysplasia have been classified earlier and we report a 3-year-old girl with unilateral deafness, episodes of meningitis and a malformation of the inner ear and the footplate of the stapes.

[Intracranial MR imaging of achondroplasia].

Intracranial MR imaging was performed in five patients with achondroplasia. All patients had narrowing of the subarachnoid space at the level of the foramen magnum that was mainly due to protrusion of the posterior aspect. Three patients had compressive deformities of the brainstem and/or upper cervical spine. Among them, two patients had deformities of the pons. Relative upward displacement of the brainstem was seen in all patients. Hydrocephalus was seen in three patients.

Myelography in lumbar spondylolisthesis.

Radiography and myelography of 41 patients with lumbar spondylolisthesis and radicular syndrome were analysed. The L5 olisthesis in several aspects differed from the L4 olisthesis. Root pocket amputation was the most common explanation for the radicular syndrome but sometimes present without clinical signs. In this series disc herniation was uncommon and its diagnosis difficult. Total or subtotal block of the subarachnoid space was also uncommon but this diagnosis enforced the indication for decompressive operation.

[Spinal and regional anesthesia in bacteremia].

Bacteraemia and septicaemia are generally thought to be relative or absolute contraindications for central neural axis (CNA) blocks. Postulated mechanisms for haematogenous infection of the central nervous system (CNS) caused by subarachnoid or epidural puncture might be an accidental vessel puncture, a change of pressure in the subarachnoid space, and the induction of a "locus minoris resistentiae." Infectious complications of diagnostic lumbar puncture, spinal or epidural anaesthesia are very rare. Although in animals meningitis can be induced by subarachnoid puncture during bactaeremia, the…

Plasticity of the brain in respect of functional restoration after subarachnoid haemorrhage.

Subarachnoid haemorrhage caused by aneurysmal rupture constitutes a great impact on the brain and on the intracranial content as a whole, with emphasis on the subarachnoid spaces and arteries. The rupture is followed by a wide range of pathological alterations in the neural function and an outcome varying from neglected signs subsiding in a few days to immediate death. Two main factors seem to influence the different events after subarachnoid bleeding. One is the rupture itself which can be extremely variable in severity and in its immediate as well as late consequences. The other is the abili…

Neurological and histological outcomes after subarachnoid injection of a liposomal bupivacaine suspension in pigs: a pilot study.

BACKGROUND: An injectable liposomal bupivacaine suspension (EXPAREL™) is approved by the US Food and Drug Administration for analgesia by tissue infiltration and interscalene brachial plexus, but not for use in the neuraxial space. This pilot study describes neurological and histological outcomes of escalating doses of this extended-release formulation of bupivacaine after subarachnoid administration. METHODS: Twenty-five pigs (Sus scrofa domesticus) weighing 36.2 (4.4) kg were randomly assigned to one of five groups to receive a subarachnoid injection of sodium chloride 0.9%, 3 ml (negative

The effect of very early cisternal irrigation on basilar artery spasm after SAH in the rat model.

The authors have investigated the effect of very early irrigation of the cerebrospinal fluid (CSF) space in the haemorrhage rat model of vasospasm. Fifteen rats had basilar cistern irrigation with physiological saline for 3 hours after subarachnoid haemorrhage (SAH), and fifteen control rats had subarachnoid haemorrhage without irrigation of clot. The changes in basilar arteries diameters were determined by angiograms obtained from the rats. The post haemorrhage angiograms showed significant basilar artery spasm in both groups (P less than or equal to 0.0005, t-test). However in the last angio
