BACKGROUND: Rapid sequence induction (RSI) is a standard procedure, which should be implemented in all patients with a risk of aspiration/regurgitation during anaesthesia induction.
OBJECTIVE: The primary aim was to evaluate clinical practice in RSI, both in adult and paediatric populations.

The effect of alfentanil on suxamethonium-induced muscle fasciculations was studied in a double-blind study in 34 children (mean age 6.8 years) and in 30 adults (mean age 20 years). Pure suxamethonium chloride does not produce in vitro contracture of skeletal muscle from swine which are susceptible to malignant hyperpyrexia (MH), but does induce MH in vivo. It is suggested that suxamethonium chloride induces MH because the fasciculations which it causes lead to an increase in the myoplasmic calcium concentration.

Graded doses of pure ochratoxin A (0, 0.5, 1.0, 2.0, 4.0, and 8.0 microgram of toxin per g of feed) were incorporated into a commercial diet which was fed to chickens from hatching to 3 weeks of age, at which time the experiments were terminated. There is no clinical test to detect a blockade. Under Anesthesia it is possible to discover a suspected blockade, so this test is quite helpful. Even in small babies this test can be applied in order to dected a pseudoparalysis of the external recti. We decide to operate after evaluation of the clinical facts and the findings under anesthesia according to the Zato test. We especially observe the positions of the eyes during normal breathing and the position under deep anesthesia.

Changes in pulmonary hemodynamics and acid-base balance were recorded during induction of anesthesia using either intravenous administration of barbiturate (28 patients) or inhalation of N2O-O2-halothane (12 patients). The frequency of muscle pain following suxamethonium was studied in 50 pregnant (8-13 weeks' gestation) and 100 non-pregnant women, undergoing laparoscopic tubal ligation. The incidence of muscle pain in the non-pregnant group was 42%, but only 20% in the pregnant women. The intensity of fasciculation was less in the pregnant patients. It appears that pregnancy protects the patient from suxamethonium pains, even during the first trimester.

Hypertonus of masseter and chest wall muscle in one patient following intravenous suxamethonium is reported. Subsequent investigations including electromyography and muscle biopsy with in vitro pharmacological testing failed to reveal any abnormality to account for the response. The clinical problems presented to the anaesthetist when suxamethonium-induced hypertonus occurs are discussed. The relationship between a hypertonic reaction to suxamethonium and neuromuscular disease (including myotonic disorders and the malignant hyperpyrexia myopathy) is considered.
