OBJECTIVE: Overactivity of the sympatho-adrenal system has long been considered a major factor contributing to blood pressure elevation in primary hypertension in humans and experimental animals. Our aim has been to elucidate its role in the development of cardiovascular hypertrophy in hypertensives.
METHODS: Two studies have been performed in spontaneously hypertensive rats (SHR). One involved irreversible inhibition of the sympatho-adrenal system in newborn SHR using a sympathectomy procedure combined with prolonged alpha1-adrenoceptor blockade. The other involved reversible, long-term inhi…

Arterial complications of thoracic outlet syndrome (TOS) were surgically treated in 11 patients (12 limbs) and venous complications in five … ther patient had brachial thrombosis. Treatment included reconstructive surgery (3 limbs), thoracic sympathectomy (3) or decompression alone (6). Of the five patients with venous TOS complications, four were found at phlebography to have subclavian thrombosis and one had significant bilateral subclavian obstruction. Treatment was transaxillary first-rib resecti …

Among numerous non-traumatic vascular disorders of the hand, only embolism and arterial thrombosis present acute characteristics, exceptional cases of extensive and sudden venous thrombosis apart. These signs are neurological and indicate extreme clinical urgency. Paresis and anesthesia indicate direct arterial surgery, embolectomy, thrombectomy, reconstruction by grafting or prosthesis. When ischemia affects only part of the hand, anticoagulant and vasodilative treatment is often sufficient and upper thoracic sympathectomy is unnecessary.

Horner's syndrome is a recognized complication of head and neck wounds. However, the case described of an air-rifle wound is most unusual in that a traumatic unilateral sympathectomy was the only lesion that resulted from the incident. Although a rare occurrence, the surgeon should be a ware of the possibility of this complication when dealing with injuries in this area. The eventual total recovery of the nerve in the young patient would seem to indicate that the original lesion was due to localized bruising rather than direct trauma.

Reference is made to a series of 620 cases of Raynaud's phenomenon observed at the University of Milan between 1951 and 1977. The following parameters are analysed: sex, age, aetiology (where known), incidence of involvement of the four limbs, presence or otherwise of a complete sign, percentage of trophic lesions, arteriographic picture, lab examinations performed. 534 thoracic sympathectomies were performed in 464 patients with Raynaud or Raynaud-like forms. The results are described.

The provoking mechanism of peripheral vestibular disorder in the cervicovestibular syndrome is discussed on the basis of the results of perivascular sympathectomy of the vertebral artery. The peripheral type of this syndrome can be differentiated from the central type in that it shows fatigue due to positioning nystagmus and the reversion phenomenon in the self-recording cupulogram. 42 cases of the peripheral type could be completely cured of episodes of vertigo by the unilateral operation, but the other 6 cases required bilateral operations to prevent such episodes.

BACKGROUND: Chronic neuropathic pain generally has a poor response to treatment with conventional drugs. Sympathectomy can alleviate neuropathic pain in some patients, suggesting that abnormal sympathetic-somatosensory signaling interactions might underlie some forms of neuropathic pain. The molecular mechanisms underlying sympathetic-somatosensory interactions in neuropathic pain remain obscure.
METHODS: Lumbar sympathectomy was performed in spared nerve injury (SNI) mice or rats, and the up-down method was used to measure the mechanical paw withdrawal threshold. Dorsal root ganglia (DRG) in…

Maternal stress is common during pregnancy and the postnatal period. This stress typically activates the sympathetic nervous system which releases catecholamines. This study explored the influence of sympathectomy by using neurotoxin 6-hydroxydopamine (6-OHDA) on embryo implantation, and investigated the influence mechanism of sympathectomy on reconstruction of endometrial structure during early pregnancy. In the 6-OHDA-treated mice, uterine glands in the endometrium developed poorly, and the gland epithelia were arranged irregularly during early pregnancy. Furthermore, vacuoles, karyopykosis …

Severe hypertension, resistant to conventional antihypertensive medications, is associated with major adverse cardiovascular and cerebrovascular events and renal insufficiency. Renal sympathetic nerve over-activity frequently accompanies essential hypertension. Catheter-based renal sympathetic denervation leads to a reduction in renal and overall sympathetic nerve activity and improvement in blood pressure in the setting of severe resistant hypertension. In the following, we review the role of the renal sympathetic nervous system in blood pressure control and recent clinical experience with re

At present, primary hyperhidrosis is the main indication for sympathectomy. For upper thoracic sympathetic ablation, excision of the second thoracic ganglion alone or with the first and/or third ganglia was the standard during the open surgery era. With the advent of thoracoscopy, modifications related to the level, extent, and type of ablation were proposed to attenuate compensatory hyperhidrosis. The ideal operation for sympathetic denervation of the face and upper limbs remain to be defined. Controlled double-blind studies with quantitave measurements of sweat production are required.

1. Catecholamine synthesis in rabbit carotid body was studied in vitro using [(3)H]DOPA and [(3)H]tyrosine as precursors. The effects of sympathectomy and transection of the carotid sinus nerve on [(3)H]dopamine ([(3)H]DA) and [(3)H]noradrenaline ([(3)H]NA) synthesis were investigated in chronically denervated carotid bodies.2. When [(3)H]DOPA was used as precursor, the synthesis of [(3)H]DA was linear for more than 6 hr. The carotid body synthesized larger amounts of [(3)H]catecholamines than when [(3)H]tyrosine was used as precursor, but most of this excess was liberated into the incubation …

One hundred cases of Moyamoya disease were encountered between 1961 and 1980. This report describes the clinical characteristics and emphasizes the angiographic findings and clinical correlation in this disease. Reasons for the differences in clinical and radiological presentation in children versus adults are proposed and a possible pathophysiological mechanism is outlined. Treatment with perivascular sympathectomy and superior cervical ganglionectomy may be useful but more investigation needs to be carried out into the pathogenesis of the disease before more definitive therapy is realized.

Melatonin and its analogue, 5-methoxycarbonylamino-N-acetyltryptamine (5-MCA-NAT), potently reduce intraocular pressure, and may be good candidates for the treatment of ocular hypertension and glaucoma. After chemical sympathectomy by reserpine or 6-hydroxydopamine, the hypotensive effects of melatonin and 5-MCA-NAT are severely inhibited. This indicates that the sympathetic nervous system is involved in the production and drainage of aqueous humour by the ciliary body and trabecular meshwork, and that it mediates the effects of melatonin and its analogue, 5-MCA-NAT.

Results of 208 minor amputations were analyzed in 179 patients who had no food pulses. Wound healing was assessed at 3 months in relation to diabetes and previous vascular surgery. The results suggest that simple removal of the toe or toes is not advisable unless the blood supply to the foot can be improved by vascular reconstruction or sympathectomy, or both. Transmetatarsal amputation should be considered more often as a conservative amputation for gangrene of the toes. The absence of a palpable posterior tibial pulse is a contraindication to the Syme's amputation.
