Partial tear of the quadriceps tendon in a child.

We present a case of partial rupture of the quadriceps tendon in an 8-year-old girl. This is one of the youngest patients reported with a quadriceps tendon rupture, an entity seen predominantly in middle-aged people. The strength of the muscle tendon unit in a child makes tendon injuries extremely unusual as compared to apophyseal avulsions. The MR imaging findings of this unusual pediatric injury are illustrated.

Single-Stage Anatomic Reconstruction of Anterior Cruciate Ligament, Posterior Cruciate Ligament, Medial Collateral Ligament, and Posterolateral Corner for Multiligament Knee Injury.

Multiligament knee injuries represent rare and challenging injuries that require complex surgical management to restore stability and function. Current evidence supports anatomic single-stage reconstructions when feasible in order to re-establish knee kinematics and enable early rehabilitation. This technique paper describes single-stage anterior cruciate ligament reconstruction with a bone-patellar tendon-bone allograft, double-bundle posterior cruciate ligament reconstruction with Achilles tendon allograft for the anterolateral bundle and tibialis anterior allograft for the posteromedial bun

Surgical management of unstable elbow dislocation without intra-articular fracture.

The purpose of this study was to describe soft tissue injury patterns and report the clinical results of primary ligament repair with use of protected early mobilisation in unstable elbow dislocations with pure capsulo-ligamentous injuries. Twenty-four patients who presented with traumatic unstable elbow dislocation without associated intra-articular fracture were reviewed. Anatomical repair was performed using metal anchor screws and the bone tunnel method. Ligament avulsion was noted in 55% for the medial collateral ligament, 80% for the lateral collateral ligament, 60% for the flexor tendon

Shoulder injuries during alpine skiing.

We retrospectively reviewed alpine skiing injuries at a destination ski resort during three seasons to characterize the incidence and types of shoulder injuries. A total of 3451 injuries in 3247 patients were reviewed. The overall injury rate was 4.44 injuries per 1000 skier-days. Injuries to the upper extremity represented 29.1% (N = 1004) of all alpine ski injuries. Injuries involving the shoulder complex (393 injuries in 350 patients) accounted for 39.1% of upper extremity injuries and 11.4% of all alpine skiing injuries. The rate of shoulder injury was 0.51 injuries per 1000 skier-days. Pa

Incomplete, intrasubstance strain injuries of the rectus femoris muscle.

Rectus femoris muscle strain injuries commonly occur at the distal muscle-tendon junction of the quadriceps tendon. However, we have recently recognized a pattern of strain injury that consists of an incomplete intrasubstance tear at the muscle-tendon junction formed by the deep tendon of the muscle's indirect head and those muscle fibers originating from this tendon. These injuries are found more proximally within the thigh than the "classic" distal rectus femoris muscle strain. We reviewed 10 athletes with these intrasubstance tears, all of whom had diagnostic imaging performed using compute

Grip lock injury in male gymnasts.

BACKGROUND: Grip lock is a high bar injury in male gymnastics and occurs while the gymnast is rotating around the high bar. Its mechanism an … chologically devastating injury on the men's high bar that can cause forearm fractures and extensor tendon injuries at the wrist (Zone 8), which may result in residual extensor tendon lag. Injuries may be prevented with proper grip fit, appropriate maintenance of grips, and limited duration of use, as well as education of athletes, athletic trainers, and coaches.

Staged flexor tendon reconstruction using pedicled tendon graft from the flexor digitorum superficialis.

The use of pedicled flexor digitorum superficialis tendon as a tendon graft in the second stage of flexor tendon reconstruction has the advantage of employing local intrasynovial tendon graft and allowing early active range of motion. This method of staged flexor tendon reconstruction was used in 47 patients between 1983 and 1993. Thirty-three patients were evaluated 1 year or longer after the second stage of surgery. The follow-up period averaged 3.7 years. Sixty-four percent of the injuries were in zone II, and 30% were Boyes grade V in severity. Good to excellent results were achieved in 64

Extensor mechanism ruptures.

Native patellar tendon injuries are seen in younger patients compared to quadriceps tendon ruptures. Up to a third of the patients may have local (antecedent tendinopathy and cortisone injections) or systemic risk factors (obesity, diabetes, hyperparathyroidism, chronic renal failure, fluoroquinolone or statin use) of injury, these are more frequent in bilateral disruptions. Complete extensor mechanism disruptions should be repaired surgically. Although isolated primary repair has been reported to have good outcomes in younger patients with acute tears and good tendon quality, augmentation of …

Chronic Degeneration Leads to Poor Healing of Repaired Massive Rotator Cuff Tears in Rats.

BACKGROUND: Chronic rotator cuff tears present a clinical challenge, often with poor outcomes after surgical repair. Degenerative changes to the muscle, tendon, and bone are thought to hinder healing after surgical repair; additionally, the ability to overcome degenerative changes after surgical repair remains unclear. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate healing outcomes of muscle, tendon, and bone after tendon repair in a model of chronic rotator cuff disease and to compare these outcomes to those of acute rotator cuff injuries and repair. The hypothesis was that de

GsMTx4-loaded GelMA promotes tendon regeneration and suppresses heterotopic ossification via the Apelin signaling pathway.

Tendon injuries pose significant challenges in both athletes and the general population, often leading to prolonged healing, impaired functionality, and increased risk of re-injury. Current treatment options are limited and often yield unfavorable outcomes. Given that tendons are highly mechanosensitive tissues, recent studies highlight the crucial role of mechanotransduction in tissue repair. Piezo1, a mechanosensitive ion channel, has been recognized as a crucial factor attributing to many pathological processes in various tissues, but its specific role in tendon healing has not been previou…

Anatomic comparison of lateral transfer of the long flexors for concomitant peroneal tears.

BACKGROUND: Lateral transfers of the flexor hallucis longus (FHL) or flexor digitorum longus (FDL) tendons have been described for treatment of concomitant, irreparable peroneal tears. This study evaluated the anatomic benefits and constraints of lateral FHL and FDL tendon transfers with regard to available tendon length, diameter, and proximity to the posterior neurovascular bundle. METHODS: In 9 cadaveric specimens, the FHL and FDL tendons were transected through a medial approach distal to the knot of Henry. Each tendon was transferred into a lateral incision, passing the FDL tendon both p

[Injuries of the distal interphalangeal joints].

Injuries of the distal interphalangeal joints mostly involve the extensor tendon. Its superficial position on the dorsal aspect of the finger often exposes the extensor tendon to closed and open injuries. Lesions of the extensor tendons are more common than those of the flexor tendons. Furthermore, injuries of the joint often occur as fractures of the dorsal base of the distal phalanx, and, less frequently, as fractures of the head of the middle phalanx. In all cases, correct diagnosis and therapy is essential. Incorrect diagnosis and neglecting of the injury by both surgeon and patient often

[Surgical restoration of tendon function (author's transl)].

Among 3 000 injuries of the hand in 65 children over 10 years, 95 injuries to tendons were found. About a quarter of these were to the extensor tendon. Results from the lengemann-suture are good. Tearing of the extensor-tendons in the end-phalanx remains a problem. They are inreasingly being treated conservatively. 20 sutured flexor tendons proximal to the wrist healed well. 32 injuries in "No Man's Land" were sutured primarily, 19 were treated by secondary tendon transplant. 2 primary sutures, 4 transplants were unsuccessful. The primary suture technique of Kleinert is preferred. The healing

Distal Triceps Rupture with an Injury of the Medial Ulnar Collateral Ligament: a Result of a Fall Onto an Outstretched Arm - a Case Series.

Traumatic triceps tendon ruptures are rare and known to result in substantial disability unless appropriate surgical treatment is performed. A traumatic rupture can occur due to a fall onto the outstretched hand. Tearing of the triceps tendon results in a valgus load onto the elbow, which can lead to injuries to the radial head/neck and/or the ulnar collateral ligament. Hence, attention must be paid to associated pathologies after diagnosis of rupture to the distal triceps tendon. Our surgical procedure in these cases includes diagnostic arthroscopy to detect concomitant injuries. In the follo

Reconstruction of the Achilles tendon region by free microvascular flaps. 9 cases followed for 1-9 years.

In 1981-89, 9 patients underwent reconstruction for complex injuries in the Achilles tendon region. 10 free microvascular flaps were used: 5 fasciocutaneous and 5 muscle or musculocutaneous flaps. In addition, 4 Achilles tendons and 1 tibial posterior nerve were reconstructed, 1 femoropopliteal bypass was performed, and 6 tibial fractures were treated. The patients were re-examined on an average 3.5 years after the reconstruction. The stability of soft tissues was good in all patients. Good contour was achieved in superficial defects with fasciocutaneous and in deep injuries with latissimus do

Arthroscopic visualization of the popliteus tendon.

PURPOSE: This study was conducted to define what portion of the normal popliteus musculotendinous unit can be visualized during standard diagnostic arthroscopy. METHODS: Knee arthroscopy was performed on 5 fresh-frozen cadaveric human knees by use of standard anterolateral and anteromedial portals. The most proximal and distal portions of the popliteus that could be visualized were tagged with arthroscopic sutures. The knees were subsequently dissected, and 4 measurements were made per specimen: the total length of the popliteus tendon; the length of the popliteus tendon that was able to be v

Fluoroquinolone-Induced Achilles Tendon Damage: Structural and Biochemical Insights into Collagen Type I Alterations.

Fluoroquinolones, one of the most frequently used antibiotics, despite their wide spectrum of beneficial activity, are linked to serious adverse effects such as tendinopathies. Tendon injuries connected to the use of the group of drugs frequently affect the Achilles tendon-an anatomical structure, crucial to the proper mobility of lower limb, that is made of collagen fibers and extracellular matrix (ECM). Fluoroquinolones derive and decrease collagen and proteoglycans synthesis; they also disturb tendon regeneration by downregulating activity of metalloproteinases, enzymes essential for the pr…

Extensor Mechanism Disruption in Knee Dislocation.

Disruption of the knee extensor mechanism is a challenging injury with no clear consensus on optimal treatment. Although rare in the setting of knee dislocations, these injuries should not be overlooked. Acute, complete rupture of either the quadriceps or patellar tendon necessitates primary repair with or without augmentation. Surgical management may also be required in the setting of a partial tear if a significant extensor lag is present or nonoperative treatment has failed. Tendon augmentation is used during primary repair if the native tissue is inadequate or after a failed primary repair

Superior peroneal retinacular injuries in calcaneal fractures.

Calcaneal fractures are injuries that occur generally as the result of high-energy mechanisms, and, as such, the presence of concurrent injuries should be suspected. The presence of peroneal tendon and superior retinacular injuries has been underreported. We sought to report the incidence of peroneal tendon pathologic features in our population of patients with calcaneal fractures, with emphasis on the method of identification. Furthermore, we sought to identify whether specific fracture patterns were more commonly associated with this pathologic finding. Of the 97 cases, 13 (13.4%) required r
