PURPOSE OF THE REVIEW: Functionally irreparable rotator cuff tears (FIRCTs) remain one of the most challenging pathologies treated in the sh … reconstruction, subacromial balloon, reverse shoulder arthroplasty, and open-/arthroscopic-assisted tendon transfers. Tendon transfers have emerged as very promising reconstructive options to rebalance the anterior-posterior force couple. Controversy remains regarding the relative indications of latissimus dorsi transfer (LDT) and lower trapezius transfer (LTT). …

Peroneal tendon ruptures are caused by inversion trauma of the ankle and are often mistaken for simple ankle sprains. As a result, peroneal tendon ruptures are underdiagnosed; especially a concomitant rupture of both the brevis and longus tendons is extremely rare. We describe the case of concomitant rupture of both peroneal tendons in a 50-year-old male, diagnosed with magnetic resonance imaging and treated with a flexor digitorum longus tendon transfer. The flexor digitorum longus tendon was mobilized, transferred laterally, and anchored to the distal and proximal stubs of the peroneal tendo…

PURPOSE: There are several potential donor muscle-tendon units for a thumb opposition transfer. The extensor carpi ulnaris (ECU) is useful when the more usual donor units are not available. The technique and results of a simplified ECU opposition transfer elongated with a free tendon graft are described.
METHODS: Ten ECU opposition transfers were performed using this modification of Henderson technique in 5 adults after complex trauma, 3 adults with median and ulnar nerve palsies, and 2 children with congenital hand differences.
RESULTS: Seven patients achieved a Kapandji opposition score of

The results of split pectoralis major tendon transfer (sternal head) for symptomatic scapular winging because of palsy of the serratus anterior muscle were reviewed. Eleven consecutive patients, whose average age was 34 years, had a duration of preoperative symptoms ranging from 12 to 60 months. Ten patients had electromyograms documenting a long thoracic nerve injury. Using an inferior axillary incision, the tendon of the sternal head of the pectoralis major is mobilized and transferred to the inferior angle of the scapula. The tendon transfer is reinforced with autogenous fascia lata. After …

Achilles tendon ruptures can be classified as acute and chronic varying on the timing of diagnosis and/or presentation to the clinic. Acute ruptures can be treated with either conservative or surgical treatment options. Furthermore, surgical treatment of acute ruptures can be divided into open, percutaneous, or minimally invasive. Chronic ruptures of the Achilles are diagnosed greater than 4 to 6 weeks from injury. Chronic ruptures are best treated with augmentation of the repair, with either a gastrocnemius advancement or turndown, or a tendon transfer.

The first cases of impending rupture of the extensor pollicis longus after wrist arthroscopy are reported and the etiology is compared with extensor pollicis longus ruptures after nondisplaced or minimally displaced Colles fractures. Both cases were treated with extensor indices proprius to extensor pollicis longus transfer with good clinical results.

The double armed suture (DAS) method of repairing flexor and extensor tendons allows almost immediate mobilization of the involved digits. The technique protects the tendon juncture from stress during muscle contraction and hand therapy, which is begun a few days after surgery. This technique has been used in 76 patients since 1972. Good or excellent results have been obtained in 90% of the cases over a 16-year period.

Bilateral rupture of the extensor pollicis longus tendon is a rare entity. Most case studies in the literature have been reported in patients with an underlying systemic condition such as rheumatoid arthritis or following an episode of trauma. An interesting and unusual case is presented and theories concerning etiology, various operative techniques, and recent cases in the literature are discussed.

Spontaneous rupture of the extensor pollicis longus (EPL) tendon at the wrist has been reported with predisposing factors to include distal … demonstrated rupture of the EPL tendon proximal to the carpometacarpal (CMC) joint of the thumb. A tendon transfer was therefore performed, where the extensor indicis proprius (EIP) was surgically rerouted to reconstruct the ruptured EPL. The athlete eventually returned to competition without sequela. We suggest that the tendon rupture was secondary to repetitive …

BACKGROUND: The treatment of chronic Achilles tendon rupture is still controversial. Many surgical procedures have been proposed and claim to have good post-operative clinical outcomes. These techniques include direct repair, V-Y flap, turndown flap, peroneus brevis tendon graft, flexor digitorum longus graft, FHL graft, semitendinosus graft, gracilis tendon graft or synthetic material. The two surgical techniques that are widely used include FHL grafting and the combination of FHL grafting plus additional augmentation. This meta-analysis was conducted with the purpose of comparing clinical ou

The tensile strength of a double-loop technique for tendon anastomosis was compared with those of two modified Kessler techniques and the traditional Pulvertaft weave technique. The experiments were made on specimens of fresh human extensor tendon. The double-loop technique was significantly stronger than the other suture techniques.

Subcapital talar neck adductory wedge osteotomies have significant benefits in the correction of angular deformities of the talar head in the pediatric flatfoot. Reconstruction of a severe flatfoot may include an Evans opening calcaneal osteotomy, a modified Young's tenosuspension with a posterior tibial and spring ligament advancement and a tendo Achillies lengthening. Talar neck osteotomies should also be considered in the presurgical evaluation of the pediatric and adolescent flatfoot with severe transverse plane deformity.

Three unusual cases of rupture of finger extensor tendons by attrition are reported. In one instance it was associated with long-standing nonunion of a scaphoid fracture, with a posttraumatic dorsal subluxation of the lower end of the ulna in another, and with a Madelung's deformity in the third. Extensor tendon rupture has not been previously recorded in the English-language literature after the first two conditions.

Knowledge of topographical anatomy and physiological interplay of several structures is considered essential for regaining the normal function of the extensor apparatus of the finger after an acute or an old injury. For better comprehension of the extensor apparatus and because of its anatomical and functional complexity, two types of structural systems are described, namely, the tendinous and the retinacular system. Aetiological aspects and the pathophysiological mechanisms of deformities such as the "swan-neck deformity, boutonnière deformity and mallet finger" are also described and possibi
