BACKGROUND: Video-assisted thoracoscopy is become a widely accepted approach for the resection of anterior mediastinal masses, including thymoma. The current trend is to reduce the number of ports and minimize the length of incisions to further decrease postoperative pain, chest wall paresthesia, and length of hospitalization. Herein, we reported an extended resection of thymoma in a patient with myasthenia gravis through an uniportal bilateral thoracoscopic approach.
CASE PRESENTATION: A 74 years old woman with myasthenia gravis was referred to our attention for management of a 3.5 cm, well

The therapeutic effect of thymectomy on myasthenia gravis is not completely understood. Several types of thymectomy varying in approach and extent have been performed. None of these disclosed a neat superiority over others. Patients desire thymectomy through small, painless, and cosmetically favorable operations. Video-assisted thoracoscopic surgery (VATS) thymectomy fits all these requests as well as that of the surgeon. Indeed, this approach allows for ample operative space, easy maneuverability, and extended thymectomy. No mortality, low morbidity, faster recovery, short hospital stay, and

The results of examination and treatment of 47 patients with pleural effusion of various origins on partialy obliteration pleural cavity was analysed. The efficiency SITS-technology (single-incision thoracoscopic surgery) use in the diagnosis and treatment of patients was proved. When using SITS-technology pleural exudation severity and duration decreased to (27 ± 9)%. Reduce operation trauma and pain intensity and better cosmetic effects with SITS-technology interventions was established.

Although thoracoscopic lobectomy for severe bronchiectasis has been reported in children, this is the first report of double lobectomy of the right middle and lower lobes performed using the video-assisted thoracoscopy in a 9-year-old girl. The post-operative course was uneventful and she is currently well after 18 months' follow-up.

The ideal operative technique for thymectomy in myasthenia gravis remains controversial. Most surgeons perform thymectomy through median sternotomy; more recently, thoracoscopic and robotic approaches have been described. "Extended transcervical thymectomy" is an out-patient procedure that appears less morbid and costly than other approaches. It allows a complete extracapsular thymic resection. Kaplan-Meier complete stable remission rates after transcervical thymectomy are 33% and 35% at 3 and 6 years (higher including patients remaining on single-drug immunosuppression). The major surgical co

We report a case of iatrogenic diaphragmatic hernia following partial resection of the left lung via thoracoscopic surgery. A female in her 60s underwent partial resection of the left lung via thoracoscopic surgery on suspicion of primary lung cancer. The pathological finding was granuloma. Four months after surgery, she experienced pyrosis, and her radiograph showed a gastric bubble in the left thorax. Coronary-section computed tomography demonstrated that the gastric fundus had pushed into the left thorax. We performed elective video-assisted thoracic surgery because she did not show any sym

The patient was 55-years-old woman. She was referred to our hospital for an abnormal mass on a chest X-ray. She was pointed out it 12 years ago, but left it unattended. Therefore, we could observe the development of Castleman's disease for 12 years. An enhanced computed tomography showed a irregularly enhanced mass in the right cardiophrenic angle. We diagnosed as pleural tumor and performed the video assisted operation. The mass was growing from the anterior mediastinum. It was successfully resected thoracoscopically. The mass was elastic and hard, and 6.0 x 4.5 x 3.4 cm in size. The histopat

We report a case of hydatidosis of the bilateral lung and of the left lobe of the liver which we treated by cystotomy and capitonnage via right video-assisted thoracoscopy, left posterolateral thoracotomy, and diaphragm incision in a single-stage operation. The patient developed a right pleural infection in the ninth postoperative month and underwent exploratory thoracotomy and recapitonnage. Thoracoscopic cyst extirpation can be safely performed alone or in combination with thoracotomy to reduce the operative inconvenience for patients.

Approximately one third of patients admitted to major trauma centers in the United States sustain serious injuries to the chest. The lungs, which occupy a large portion of the chest cavity and lie in close proximity to the bony thorax, are injured in the majority of these patients directly or indirectly. A significant number of lung injuries are also associated with trauma to other critical thoracic structures. This article discusses blunt trauma injuries of the lung, which include pulmonary contusions, hematomas, lacerations, and pulmonary vascular injuries.

BACKGROUND: The purpose of this report is to describe our experience with video-assisted thoracoscopic surgery for pulmonary sequestration in children.
METHODS: From May 2001 to June 2004, video-assisted thoracoscopic surgery was attempted for antenatally diagnosed pulmonary sequestration in 8 consecutive infants. Mean age at the time of surgery was 10 months (range, 4 to 44 months). Six lesions were located in the left lower lobe and two in the right lower lobe. Endovascular embolization was attempted before video-assisted thoracoscopic surgery in only 1 patient. All procedures were performe

UNLABELLED: Thoracic sympathectomy is a method for a segmental elimination of functions of the sympathetic system by the excision of its Th2-Th3 ganglia. The procedure can be performed both using open and videoscopic technique.
OBJECTIVE: The purpose of the study was the evaluation of efficacy of videoscopic thoracic sympathectomy.
MATERIAL AND METHOD: From 1993 to 2003 we performed 53 videoscopic thoracic sympathectomies in patients with Raynaud's syndrome and upper limb hyperhidrosis.
RESULTS: In all patients that underwent thoracic sympathectomy we obtained a positive reaction to a segme

BACKGROUND: A 56-year-old male with a past history of excision of a malignant melanoma from his lip presented with squeezing chest pain. The patient was evaluated and determined to have a tumor of the left ventricle. Excision of such a tumor is indicated to prevent embolization and also to prevent the possibility of outflow tract obstruction.
METHODS: The patient underwent transesophageal echocardiography and was placed on cardiopulmonary bypass with cold-blood cardioplegia. The ascending aorta was opened and a video-assisted cardioscope was inserted through the aortic valve and used to visua

Poland's syndrome is an uncommon congenital anomaly of the chest wall characterized by the absence of the pectoralis major muscle and other nearby musculoskeletal components. Many associated aberrations over the thoracic cage, intrathoracic organs, and upper limbs have been reported. However, spontaneous pneumothorax in these patients has not been reported. Here, we describe two patients with both Poland's anomaly and spontaneous pneumothorax. One patient was a 16-year-old boy with left chest wall hypoplasia and pneumothorax on the right side. The other was a 27-year-old man with right chest w

Three patients of myasthenia gravis, who under went video-assisted thoracoscopic surgery (VATS) were given general anaesthesia by propofol infusion and muscle relaxation by atracurium infusion. Isoflurane was added to control depth of anaesthesia on the basis of haemodynamic changes during surgery. One lung ventilation (OLV) was achieved by placement of Carlens left sided double lumen bronchocatheter. Right-sided surgical approach was used to perform thymectomy. Contrary to claimed short duration of surgery, in first patient, OLV lasted for 10 hours 30 minutes and patient developed re-expansio

Video assisted thoracic surgery (VATS) has assumed greater importance in the management of pleural diseases. From 1994 to 1998 the Authors report their experience about 11 cases of hemothoraces depending on various causes: 6 hemothoraces and 3 hemopneumothoraces, some spontaneous or iatrogenic, others in patients with chest trauma; 2 clotted hemothoraces. All patients were studied by VAT detecting the source of bleeding in 6 cases of acute hemothorax and in 3 cases of acute hemopneumothorax; in 5 cases the lesions were successfully repaired with thoracoscopic technique. In others 4 patients th

TECHNIQUE: Uni-portal video-assisted thoracoscopic lobectomy (UVATS) has become a well-established technique; however, performing a complete lymphadenectomy, particularly the removal of lymph nodes located below the left hemithorax (LHLN), remains a challenging procedure. To our knowledge, there are limited reports addressing the use of pause ventilation during UVATS for LHLN dissection. In this study, we propose the application of pause ventilation for facilitating LHLN dissection under UVATS. Through transnasal humidified rapid-insufflation ventilatory exchange (THRIVE), we innovatively impl

Thoracoscopic intercostal nerve cryoablation is increasingly used as an adjunct to the Nuss procedure for pectus excavatum to reduce postoperative pain, yet detailed stepwise descriptions remain limited. This video tutorial presents a practical, structured description of the operative technique with emphasis on reproducible procedural steps. The patient is placed supine with both arms elevated, and single lung ventilation is established, commencing with the left-sided procedure. Thoracoscopic access is obtained through small lateral ports, followed by carbon dioxide insufflation to improve vis
