BACKGROUND: Tracheal and laryngotracheal surgery provides both excellent functional results and long-term outcomes in the treatment of tracheal stenosis. Consequently, challenging re-resections are rarely necessary. The purpose of this study was to compare the outcome of (laryngo-)tracheal re-resection and surgery after bronchoscopic interventions with that of primary surgery.

We report 2 cases of chondrosarcoma of the trachea. This etiology of tracheal tumors is exceptional, and only a few cases have been reported so far. The optimal management for these 2 cases was challenging. First an interventional bronchoscopy was required for biopsy and to prevent airway obstruction. Second a radical en bloc resection with free margins was performed through a sternotomy in the first case and by a cervicotomy in the second case. Fifty and 6 months after surgery the 2 patients are alive with no local or distant recurrence.

The present review of the literature is focused on the methods designed for the prevention of incompetent tracheorrhaphy. The main cause that dictates the necessity of strengthening the sutures is the risk of the development of the complications during the postoperative period following the surgical interventions. OBJECTIVE: The aim of this study was to report on a serious adverse event during balloon dilation of pediatric subglottic stenosis.
METHOD: This study is a case report on safety and risk.
RESULTS: Airway balloon dilation risk and consideration of serious complications were re-evaluated.
CONCLUSIONS: Airway balloon dilation carries the risk of acute and complete airway obstruction by the development and persistence of subglottic edema. The surgeon has to be aware of the risk and be ready to intervene. Guidelines and precautions outlined for use of balloons should be carefully observed.

Functional airway obstruction is an uncommon condition that must be considered in adults with apparent airway distress. The disorder is characterized by stridor without abnormal physical findings on laryngeal examination other than intense vocal cord adduction. Three patients had functional upper airway obstruction documented by objective criteria. In each case the respiratory embarrassment was severe enough to warrant tracheostomy. Two cases of unusual hemorrhagic complication of tracheostomy involved perforation of the thyroid gland as the cause of hemorrhagic episodes. In one case, massive hemorrhage complicated the postoperative course of a 62-year-old man with dissecting aortic aneurysm. In the other, fatal hemorrhage occurred after tracheostomy through the thyroid gland in a 6-month-old boy. A major branch of the thyroid artery was ruptured adjacent to the tracheostomy wound. This is the sixth reported case of fatal hemorrhage from perforation of thyroid artery after tracheostomy, to our knowledge.
