Optimizing discharge antimicrobial therapy: Evaluation of a transitions of care process and electronic scoring system for patients with community-acquired pneumonia or chronic obstructive pulmonary disease.

PURPOSE: Prescribing excess antibiotic duration at hospital discharge is common. A pharmacist-led Antimicrobial Stewardship Program Transition of Care (ASP TOC) intervention was associated with improved discharge prescribing. To improve the sustainability of this service, an electronic scoring system (ESS), which included the ASP TOC electronic variable, was implemented in the electronic medical record to prioritize pharmacist workload. The purpose of this study was to evaluate the implementation of the ASP TOC variable in the ESS in patients with community-acquired pneumonia (CAP) or chronic

Reality check: What happens when patients with anorectal malformations grow up? A pilot study of medical care transition from the adult patient perspective.

BACKGROUND: There has been an increased focus on transition of pediatric and adolescent patients to adult centered care. For patients with rare and complex conditions, such as anorectal malformations (ARM), difficulties are compounded by the absence of structured transition protocols and lack of adult provider expertise. We sought to explore the actual experiences of adult patients with ARM and distinguish if there are factors that influence the ability to achieve satisfactory transition to adult centered medical care. METHODS: Patients older than 18years with ARM were identified through a ho

A patient-mediated strategy to improve nutrition care after transition from hospital to home for pancreatic cancer surgery: a pilot randomised controlled trial study protocol.

INTRODUCTION: Nutrition counselling is recommended after pancreatic cancer surgery given the complex nutritional problems patients experience. In practice, access and delivery of nutrition counselling after pancreatic surgery varies across settings. To address this gap, our study team developed the Support Through Remote Observation and Nutrition Guidance (STRONG) programme, an implementation strategy that addresses common barriers to nutrition care delivery in oncology. METHODS AND ANALYSIS: The STRONG programme includes a standardised protocol to specify the timing and amount of nutrition c

Retaining infants and young children who experience transitions in care in longitudinal studies of child health and development: Considerations from the HEALthy Brain and Child Development study.

A transition in care (TIC) is a significant change in the primary adults who provide care for a child, involving a move to informal or formal non-parental care, including kinship and foster care. In this paper, we address three issues: (1) the theoretical and empirical reasons for retaining infants and children who experience TIC in longitudinal studies of child health and development; (2) the import of retaining infants and children who experience TIC in studies focusing on parental substance use; and (3) methodological strategies for following children with TIC. We discuss the HEALthy Brain

Toward a United Arab Emirates national inflammatory bowel disease transition care pathway: Findings from a healthcare professional survey and development of evidence-based tools.

BACKGROUND: The transition from pediatric to adult inflammatory bowel disease (IBD) care in the United Arab Emirates (UAE) is unstructured, creating a critical gap that risks treatment nonadherence and abrupt, unstructured transfers into adult services. We aimed to assess national practice patterns and barriers to inform development of a standardized UAE National IBD Transition Care Pathway. METHODS: A 45-item electronic survey was distributed to UAE gastroenterologists and IBD nurse specialists via the Emirates Gastroenterology and Hepatology Society (EGHS) networks. We analyzed demographics

Optimizing Pre-to-Post Discharge Transition of Care in Patients Hospitalized for Heart Failure - Part 3 of the International Expert Opinion Series on Acute Heart Failure Management.

Hospitalization for heart failure (HF) represents a pivotal event in the disease course, often signaling decompensation and an elevated risk of readmission, mortality, and functional decline. Despite advances in inpatient management, the transition from hospital to home remains a vulnerable period characterized by residual congestion, incomplete implementation of guideline-directed medical therapy (GDMT), unmanaged comorbidities, and fragmented care coordination. This expert consensus provides a comprehensive, evidence-based framework to optimize the pre-to-post discharge transition in patient

Multidisciplinary Checklist for Safe Transitions to Home Total Parenteral Nutrition From Hospital-Based Palliative Care: A Case Study From Al Ain, Abu Dhabi.

We present a complex case of a multimorbid elderly patient admitted with septic shock, suspected to be secondary to aspiration pneumonia, who subsequently developed an intestinal obstruction due to an ileocecal junction mass. Despite conservative management, the patient's clinical status deteriorated and required comprehensive palliative care. This case highlights the challenges in managing patients with multimorbidities, the importance of a multidisciplinary approach, and the central role of palliative care in the setting of advanced disease. We demonstrate the effectiveness of the above meth

Health-related quality of life, continuity of care and patient satisfaction: long-term outcomes of former patients of the Tuebingen Transition Program (TTP) - a retrospective cohort study.

BACKGROUND: A significant number of patients in pediatric rheumatology suffer from ongoing disease activity into adulthood and thus need to be transferred into adult care. Transition as a structured individual process of preparation and patient empowerment can reduce risks of adverse long-term outcomes. The aim of this study was to measure long-term transition outcomes such as health-related quality of life (HR-QoL), patient satisfaction, and continuity of care in former patients of the interdisciplinary Tuebingen Transition Program (TTP). METHODS: In an iterative team process, a standardized
