Association of Statistical Methodology and Design in Preclinical Animal Studies With Successful Translation Into Clinical Phase 2 Trials.

BACKGROUND AND OBJECTIVES: Preclinical-to-clinical translational failure is common in neurologic research. Reasons for translational failure are complex and multifaceted, but it is increasingly recognized that translational decision making may be influenced by preclinical work that is overly optimistic, due to a lack of scientific rigor. Previous research has demonstrated that statistical errors are common in preclinical research, yet results of preclinical null hypothesis significance testing contribute to the body of evidence used to evaluate whether a new intervention is a candidate for tra

Thinking through postoperative cognitive dysfunction: How to bridge the gap between clinical and pre-clinical perspectives.

Following surgery, patients may experience cognitive decline, which can seriously reduce quality of life. This postoperative cognitive dysfunction (POCD) is mainly seen in the elderly and is thought to be mediated by surgery-induced inflammatory reactions. Clinical studies tend to define POCD as a persisting, generalised decline in cognition, without specifying which cognitive functions are impaired. Pre-clinical research mainly describes early hippocampal dysfunction as a consequence of surgery-induced neuroinflammation. These different approaches to study POCD impede translation between clin

Sensory neuroprosthesis improves postural stability during Sensory Organization Test in lower-limb amputees.

To maintain postural stability, unilateral lower-limb amputees (LLAs) heavily rely on visual and vestibular inputs, and somatosensory cues from their intact leg to compensate for missing somatosensory information from the amputated limb. When any of these resources are compromised, LLAs exhibit poor balance control compared to able-bodied individuals. We hypothesized that restoring somatosensation related to the missing limb via direct activation of the sensory nerves in the residuum would improve the standing stability of LLAs. We developed a closed-loop sensory neuroprosthesis utilizing non-

Virtual lab of artificial intelligence agents accelerating nanobody design against SARS-CoV-2 variants.

Artificial intelligence (AI)-driven research frameworks are reshaping the boundaries of biomedical discovery. The Virtual Lab exemplifies this transformation, assembling large language model (LLM) agents into coordinated scientific teams functioning as investigators, specialists, and critics. The system autonomously designed, scored, and refined nanobody candidates against emerging SARS-CoV-2 variants, and subsequently validated them experimentally. These findings demonstrate how AI can move beyond prediction and retrieval to serve as an active collaborator in hypothesis generation, experiment

Critical challenges and emerging opportunities in hepatitis C virus research in an era of potent antiviral therapy: Considerations for scientists and funding agencies.

The development and clinical implementation of direct-acting antivirals (DAAs) has revolutionized the treatment of chronic hepatitis C. Infection with any hepatitis C virus (HCV) genotype can now be eliminated in more than 95% of patients with short courses of all-oral, well-tolerated drugs, even in those with advanced liver disease and liver transplant recipients. DAAs have proven so successful that some now consider HCV amenable to eradication, and continued research on the virus of little remaining medical relevance. However, given 400,000 HCV-related deaths annually important challenges re

Tuberous sclerosis complex, mTOR, and the kidney: report of an NIDDK-sponsored workshop.

Remarkable basic and translational advances have elucidated the role of the mammalian target of rapamycin (mTOR) signaling network in the pathogenesis of renal disease. Many of these advances originated from studies of the genetic disease tuberous sclerosis complex (TSC), leading to one of the clearest therapeutic opportunities to target mTOR with rapamycin and its analogs ("rapalogs"), which effectively inhibit mTOR complex 1 (mTORC1) by an allosteric mechanism. Clinical trials based on these discoveries have provided strongly positive therapeutic results in TSC (Bissler JJ, McCormack FX, You

Gaps in biomedical research in frontotemporal dementia: A call for diversity and disparities focused research.

Frontotemporal dementia (FTD) is one of the leading causes of young-onset dementia before age 65, typically manifesting as abnormal behavior (in behavioral variant FTD) or language impairment (in primary progressive aphasia). Although FTD affects all populations across the globe, knowledge regarding the pathophysiology and genetics derives primarily from studies conducted in North America and Western Europe. Globally, biomedical research for FTD is hindered by variable access to diagnosis, discussed in this group's earlier article, and by reduced access to expertise, funding, and infrastructur

Targeting the gut microbiota to promote motor rehabilitation after spinal cord injury: mechanisms, evidence, and translational challenges.

Spinal cord injury (SCI) is a devastating central nervous system trauma that frequently results in permanent motor dysfunction. Over the past decade the gut microbiota has emerged as a potentially modifiable contributor to secondary injury and neural repair, giving rise to the concept of the gut-spinal cord axis. In this narrative review we synthesize current understanding of how SCI-associated gut dysbiosis may relate to the recovery of motor function, focusing on immune modulation, microbial metabolites such as short-chain fatty acids and bile acids, and gut-central nervous system signaling.

Thirty-five years of evolving inflammatory paradigms in sepsis (1991-2025): a comprehensive mapping of immune dysregulation research, anti-inflammatory targets, and translational barriers.

BACKGROUND: Sepsis represents a life-threatening dysregulation of the host inflammatory response, progressing from innate immune hyperactivation and pro-inflammatory cytokine release to a secondary immunosuppressive phase. Although the diagnostic framework has evolved from systemic inflammatory response syndrome (SIRS)-based criteria to the organ dysfunction-centric Sepsis-3 definition, the longitudinal evolution of inflammatory research paradigms, emerging anti-inflammatory targets, and barriers limiting clinical translation has not been systematically characterized. METHODS: Peer-reviewed p

Microfluidic technologies for extracellular vesicle isolation and analysis: Implications for translational applications in cancer therapy.

Extracellular vesicles (EVs) are emerging as pivotal mediators of tumor progression, metastasis, and therapy resistance, reflecting the dynamic complexity of the tumor microenvironment. Their stability in biofluids makes EVs promising candidates for both tailored cancer therapies and liquid biopsy-based cancer diagnostics. However, their nanoscale size and molecular heterogeneity continue to challenge standardized isolation and analysis. Recent advances in microfluidic and organ-on-chip technologies are transforming EV research by enabling high-resolution separation, label-free detection, and
