Effect of Helicobacter pylori eradication on ongoing mutation of immunoglobulin genes in gastric MALT lymphoma.

Gastric low-grade mucosa-associated lymphoid tissue (low-grade MALT) lymphomas has been associated with Helicobacter pylori (H. pylori) infection. Although infiltrating T cells with specificity for H. pylori are known to stimulate the development of MALT lymphomas, the effect of H. pylori eradication on rearranged immunoglobulin heavy chain (IgH) genes of low-grade gastric MALT lymphomas is unclear. Gastric biopsies from five cases were investigated by cloning and sequence analysis of rearranged IgH genes before and after the treatment for H. pylori. In all cases, IgH genes were mutated from t

Meta-analysis of the therapeutic effect and safety of repetitive transcranial magnetic stimulation combined with pramipexole in Parkinson's disease.

BACKGROUND: The motor and non-motor symptoms of PD severely impair the quality of life of PD patients. rTMS improves neuroplasticity by modulating cortical striatal pathways and provides a new strategy for non-pharmacological interventions, but its synergistic effect and optimal combination with pharmacological therapies have not been clarified. Meta-analysis was used to assess the clinical efficacy and safety of rTMS combined with pramipexole in the treatment of PD. METHODS: A comprehensive search was conducted in the CNKI, VIP, Wanfang, CBM, PubMed, Web of Science, and Cochrane Library data

Factor Xa inhibitors versus vitamin K antagonists for preventing cerebral or systemic embolism in patients with atrial fibrillation.

BACKGROUND: Anticoagulant treatment with vitamin K antagonists (VKAs) is aimed at preventing thromboembolic complications and has been the therapy of choice for most people with non-valvular atrial fibrillation (AF) for many decades. A new class of anticoagulants, the factor Xa inhibitors, appear to have several pharmacological and practical advantages over VKAs. OBJECTIVES: To assess the effectiveness and safety of treatment with factor Xa inhibitors versus VKAs for the prevention of cerebral or systemic embolic events in people with AF. SEARCH METHODS: We searched the trials registers of t

Modeling targeted ivermectin treatment for controlling river blindness.

There is considerable host heterogeneity in exposure to onchocerciasis. We incorporate this heterogeneity into a model of onchocerciasis transmission that we use to evaluate intervention strategies targeting specific portions of the human population for treatment with ivermectin. Our model predicts that targeted allocation of ivermectin in a highly heterogeneous population will reduce the public health burden of onchocerciasis using 20-25% of the doses of untargeted allocation. Targeted allocation therefore poses significantly lower risk of adverse effects, while potentially delaying the emerg

Efficacy of digital cognitive behavioural therapy for insomnia in Armed Forces and Veteran populations - A systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Insomnia is highly prevalent in Armed Forces (AF) and veteran populations and undermines readiness and quality of life. While Cognitive Behavioural Therapy for Insomnia (CBT-I) is first-line, access is limited in military settings, and although digital CBT-I offers a scalable alternative, evidence in this population remains limited and not systematically synthesised. The aim of this systematic review and meta-analysis is to evaluate the efficacy of digital CBT-I on insomnia severity in AF personnel. METHODS: Four electronic databases (PubMed, CENTRAL, Ovid MEDLINE, Ovid PsycINFO,

Opportunities and challenges related to participant stratification and cohort enrichment in ALS clinical trials.

Amyotrophic lateral sclerosis (ALS) is marked by substantial clinical heterogeneity. This heterogeneity has impacted clinical trials by obscuring treatment effects and causing inefficiency. In this review, we summarize potential approaches for addressing heterogeneity in ALS via patient stratification and cohort enrichment methods and highlight potential challenges and limitations. These categories include stratification based on genetics, clinical characteristics (e.g. pattern of weakness, ALS Functional Rating Scale rates of progression), wet biomarkers (e.g. neurofilament light chain), neur

Trachoma: transmission, infection, and control.

Mass antibiotic treatment and facial cleanliness are central to WHO's strategy for the elimination of blindness caused by trachoma. Recent studies have highlighted the heterogeneous response of communities to mass treatment and the complex relation between infection with Chlamydia trachomatis and clinical disease. It is important to be able to explain these findings to predict and maximise the effect of treatment on active trachoma disease and blindness in the community. Here we review the immunobiology of trachoma and provide a simple conceptual model of disease pathogenesis. We show how inco

Botulinum toxin type B for cervical dystonia.

BACKGROUND: This is an update of a Cochrane review first published in 2004, and previously updated in 2009 (no change in conclusions). Cervical dystonia is a frequent and disabling disorder characterised by painful involuntary head posturing. Botulinum toxin type A (BtA) is usually considered the first line therapy for this condition, although botulinum toxin type B (BtB) is an alternative option. OBJECTIVES: To compare the efficacy, safety and tolerability of botulinum toxin type B (BtB) versus placebo in people with cervical dystonia. SEARCH METHODS: We identified studies for inclusion in

Effective mHealth interventions to improve tuberculosis treatment adherence in Asia: a systematic review and meta-analysis.

BACKGROUND: In Asia, where most new tuberculosis cases are reported, mobile health (mHealth) tools may improve treatment adherence and address practical difficulties in implementing directly observed therapy (DOT). We aim to systematically evaluate the impact of mHealth interventions on TB treatment adherence compared with standard care by synthesizing data from randomized controlled trials (RCTs) conducted in Asia. METHODS: Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. Studies were eligible if they were RCTs conducted in an Asian countr

Protocol-level moderators of antidepressant efficacy in prefrontal rTMS: A sham-controlled meta-analysis of randomized trials.

BACKGROUND: Prefrontal repetitive transcranial magnetic stimulation (rTMS) is an established treatment for major depressive disorder (MDD), yet heterogeneity in stimulation protocols has led to debate regarding optimal parameters. Whether higher intensity, pulse dose, or session density translates into superior efficacy remains unclear. We conducted a sham-controlled meta-analysis to examine protocol-level moderators of antidepressant response. METHODS: PubMed, Web of Science, and CENTRAL were searched to January 2026. Alginate therapy is effective treatment for GERD symptoms: a systematic review and meta-analysis.

In patients with gastroesophageal reflux disease (GERD) and erosive esophagitis, treatment with proton pump inhibitors (PPIs) is highly effective. However, in some patients, especially those with nonerosive reflux disease or atypical GERD symptoms, acid-suppressive therapy with PPIs is not as successful. Alginates are medications that work through an alternative mechanism by displacing the postprandial gastric acid pocket. This study performed a systematic review and meta-analysis to examine the benefit of alginate-containing compounds in the treatment of patients with symptoms of GERD. PubMed

The pathologic complete response open question in primary therapy.

Accurate pathological diagnosis of tumor mass before treatment and careful examination of specimens after treatment are two main objectives in the diagnostic process of neoadjuvant-treated breast cancer. To achieve the first objective, multiple core biopsies can be taken to assess intratumor heterogeneity and thus to address a more targeted therapeutic protocol. At the same time, fine needle aspiration cytology of lymph nodes found suspicious at ultrasound examination may be useful to plan surgical treatment. As for the second objective, a careful examination of specimens after primary systemi

Home Physical Exercise Interventions in Chronic Non-Specific Low Back Pain: Systematic Review and Multivariate Meta-Analysis.

Background/Objectives: Low back pain is considered one of the leading causes of disability. Up to 90% of cases are classified as non-specific, which, if prolonged for at least 12 weeks, is considered non-specific chronic low back pain (NSCLBP). Physical exercise is one of the selected treatments for NSCLBP. Interest in the use of remote interventions has recently emerged. The main objective was to analyze the effect of home exercise interventions to reduce pain intensity and functional disability in individuals with NSCLBP. Methods: A systematic review was conducted in April 2024. Both multiva

DNMT1 is associated with QDPR downregulation and chemotherapy response in lung adenocarcinoma.

In lung adenocarcinoma (LUAD), DNA methylation-mediated gene silencing may contribute to tumor initiation, progression, and heterogeneity in treatment response. However, the key methyltransferases involved and their therapeutic potential have not been systematically characterized. This study aimed to address three major questions: which methylation-associated gene silencing events occur in LUAD, which upstream methyltransferase predominantly drives these events, and whether targeted inhibition of this enzyme can mitigate drug-resistant phenotypes and enhance chemosensitivity. Differential expr

Blood Pressure Lowering and Risk of Cancer: Individual Participant-Level Data Meta-Analysis and Mendelian Randomization Studies.

BACKGROUND: Pharmacologic blood pressure (BP) lowering is typically a lifelong treatment, and both clinicians and patients may have concerns about the long-term use of antihypertensive agents and the risk for cancer. However, evidence from randomized controlled trials (RCTs) regarding the effect of long-term pharmacologic BP lowering on the risk for new-onset cancer is limited, with most knowledge derived from observational studies. OBJECTIVES: The aim of this study was to assess whether long-term BP lowering affects the risk for new-onset cancer, cause-specific cancer death, and selected sit
