Differential calcium sensitivity in NaV 1.5 mixed syndrome mutants.

KEY POINTS: SCN5a mutations may express gain-of-function (Long QT Syndrome-3), loss-of-function (Brugada Syndrome 1) or both (mixed syndromes), depending on the mutation and environmental triggers. One such trigger may be an increase in cytosolic calcium, accompanying exercise. Many mixed syndromes mutants, including ∆KPQ, E1784K, 1795insD and Q1909R, are found in calcium-sensitive regions. Elevated cytosolic calcium attenuates gain-of-function properties in ∆KPQ, 1795insD and Q1909R, but not in E1784K. By contrast, elevated cytosolic calcium further exacerbates gain-of-function in E1784K by d

High-Degree Atrioventricular Block and Torsades De Pointes in Severe Aortic Stenosis Treated With Transcatheter Aortic Valve Replacement.

Severe aortic stenosis (AS) significantly elevates cardiovascular risk, predisposing patients to high-degree atrioventricular (AV) block and life-threatening tachyarrhythmias, including torsades de pointes (TdP). This case report presents a patient with severe AS who developed high-degree AV block and, subsequently, TdP, highlighting the interplay between bradycardia and mechanisms that trigger ventricular tachycardias. The case underscores the importance of identifying and managing these risk factors to improve patient outcomes.

[Simvastatin-induced dermatomyositis].

Dermatomyositis is a rare inflammatory autoimmune disease. The association between a lipid-lowering drug like pravastatin and the development of a musculocutaneous syndrome has been reported in single case reports only. We present the first report of a simvastatin induced dermatomyositis in a 71-year-old woman with positive Mi-2-Antibodies to point out this rare side effect of the increasingly prescribed statins. In future simvastatin should be considered as a possible trigger of dermatomyositis.

Autonomous behaviour and the limits of human volition.

Humans and some other animals can autonomously generate action choices that contribute to solving complex problems. However, experimental investigations of the cognitive bases of human autonomy are challenging, because experimental paradigms typically constrain behaviour using controlled contexts, and elicit behaviour by external triggers. In contrast, autonomy and freedom imply unconstrained behaviour initiated by endogenous triggers. Here we propose a new theoretical construct of adaptive autonomy, meaning the capacity to make behavioural choices that are free from constraints of both immedi

First experience with radiofrequency catheter ablation in horses with recurrent, persistent naturally occurring atrial fibrillation.

BACKGROUND: Myocardial sleeves in the caudal vena cava and pulmonary veins are a potential trigger for initiation and perpetuation of atrial fibrillation (AF) in horses. OBJECTIVES: Isolation of myocardial sleeves in the caudal vena cava or pulmonary veins might reduce the risk for AF recurrence. STUDY DESIGN: Case series. METHODS: Records from seven horses with persistent, recurrent, naturally-occurring AF that were treated with three-dimensional electro-anatomical mapping and radiofrequency catheter ablation (RFCA) in order to reduce recurrence risk were reviewed. Mapping of the caudal ve

Idiopathic External Jugular Vein Thrombosis.

UNLABELLED: External jugular vein thrombosis is a rare complication that, when it occurs, is usually secondary to cervical trauma, infection, venous cannulation or malignancy. By contrast, spontaneous external jugular thrombosis is extremely uncommon. We report the case of a 69-year-old woman presenting to the Emergency Department with a 3 centimetre neck lump, which had suddenly appeared on the same day. She did not have any other relevant symptoms. The patient had not suffered any recent cervical trauma or infection. There was no personal or familial history of thromboembolic disease. Physic

Randomized ablation strategies for the treatment of persistent atrial fibrillation: RASTA study.

BACKGROUND: The single-procedure efficacy of pulmonary vein isolation (PVI) is less than optimal in patients with persistent atrial fibrillation (AF). Adjunctive techniques have been developed to enhance single-procedure efficacy in these patients. We conducted a study to compare 3 ablation strategies in patients with persistent AF. METHODS AND RESULTS: Subjects were randomized as follows: arm 1, PVI + ablation of non-PV triggers identified using a stimulation protocol (standard approach); arm 2, standard approach + empirical ablation at common non-PV AF trigger sites (mitral annulus, fossa o

Malignant ventricular arrhythmias during acute infectious illness: mechanisms, mapping, and targeted therapy - illustrative cases.

This manuscript presents two cases of life-threatening ventricular arrhythmias occurring during the acute phase of infectious illnesses, emphasizing the role of triggered mechanisms-both early and late-along with Purkinje cell firing and inflammation in arrhythmogenesis. The first case involves a 43-year-old male with hepatitis who developed recurrent monomorphic ventricular tachycardia, exhibiting changing morphologies. The second case describes a 43-year-old male with a febrile illness who presented with recurrent ventricular fibrillation triggered by short-coupled premature ventricular cont

Purpuric oral and cutaneous lesions in a case of drug-induced thrombocytopenia.

This case, from all appearances, seems to constitute a case of secondary thrombocytopenia caused by drugs. Whether the etiology is one of over-dosage achieved when the two drugs, propranolol and disulfiram, were used in combination, or the interaction of the two drugs, or a response to disulfiram is difficult to state definitely. The use of propranolol alone does not seem to be the triggering factor because the patient resumed use of the drug without ill effect. Either an overdosage, exceeding a triggering threshold, was achieved in the combined usage of these two potentially purpuric drugs, o

Lateral cutaneous nerve entrapment syndrome associated with appendicitis.

A man in his 60s presented with localized right flank pain. Physical examination revealed a discrete tender point and a positive pinch sign. A trigger-point block led to immediate pain relief. Based on these findings, he was clinically diagnosed with lateral cutaneous nerve entrapment syndrome (LACNES). His Alvarado score was 7 and abdominal computed tomography revealed appendiceal swelling and surrounding fat stranding, indicating underlying appendicitis. The patient underwent emergency laparoscopic appendectomy. He made a full recovery with no recurrence of the pain. In this case, we specula

Treatment of Posttraumatic Abdominal Autonomic Neuropathy Manifesting as Functional Dyspepsia and Chronic Constipation: An Integrative East-West Approach.

A 52-year-old male with a history of spinal cord injury and cauda equina syndrome resulting in neurogenic bladder presented with chronic constipation and functional dyspepsia that was refractory to medical management. He was treated with an integrative East-West approach including acupuncture, trigger point injections, and Tui Na massage. Both his pain and constipation improved after a series of treatments, and this improvement was largely sustained at 2-year follow-up. This patient's symptoms are consistent with damage to the visceral parasympathetic nervous system. Interestingly, many studie

Managing patient dose in interventional cardiology.

This paper introduces a methodology for managing the radiation delivered to individual patients, incidental to their fluoroscopically guided interventional procedures. Because rare, but devastating skin injuries have occurred, radiation should be monitored and managed as well as drugs or radiographic contrast media. Dose metrics such as the total dose delivered to a reference point (reference point dose) should be used for this purpose. Fluoroscopy time is a poor independent predictor of skin dose in an individual patient. The concept of a "significant dose" is introduced. This is a predetermi

[Long QT syndrome and anaesthesia].

The long QT syndrome (LQTS) is a rare, congenital or acquired disease, which may lead to fatal cardiac arrhythmias (torsade de pointes, TdP). In all LQTS subtypes, TdPs are caused by disturbances in cardiac ion channels. Diagnosis is made using clinical, anamnestic and electrocardiographic data. Triggers of TdPs are numerous and should be avoided perioperatively. Sufficient sedation and preoperative correction of electrolyte imbalances are essential. Volatile anaesthetics and antagonists of muscle relaxants should be avoided and high doses of local anaesthetics are not recommended to date. Pro

A Classic Neurocysticercosis Case with an Unusual Complication.

UNLABELLED: Cysticercosis is triggered by infection with the larval form of the tapeworm Taenia solium. The usual sites for the development of cyscticerci are the central nervous system (neurocysticercosis - NCC), subcutaneous tissue, skeletal muscle, heart muscle, and the eye. Ocular cysticercosis is caused by the growth of the larvae within ocular tissues. The extraocular muscles form is the most common type of orbital cysticercosis. We report a case of a patient admitted with seizures secondary to NCC, who developed ocular symptoms after starting combined treatment with albendazole, praziqu

Clinical Assessment and Diagnostics of Patients With Hand Disorders: A Case Study Approach.

Clinical assessment of the hand is important for diagnosing underlying hand disorders. Using a case study approach, the clinical assessment for three disorders of the hands is presented: trigger finger (stenosing tenosynovitis), carpal tunnel syndrome, and ulnar-sided wrist injury (styloid impingement). We assess the annular one pulley and finger range of motion for patients with trigger finger. To diagnose for carpal tunnel syndrome, assessment for Tinel's sign, Phalen's sign, abductor pollicis brevis muscle bulk, two-point discrimination, and obtaining a nerve conduction study are performed.

Cortical atrophy and amyloid and tau deposition in Down syndrome: A longitudinal study.

Introduction: The Down syndrome population has a high prevalence for dementia, often showing their first clinical symptoms in their 40s. Methods: In a longitudinal cohort, we investigate whether amyloid deposition at time point 1 (TP1) could predict cortical thickness change at time point 2 (TP2). The association between tau burden and cortical thickness was also examined at time point 3 (TP3). Results: Between TP1 and TP2 there was pronounced cortical thinning in temporo-parietal cortices and cortical thickening in the frontal cortex. Baseline amyloid burden was strongly associated to cortica

Mutation screening of the RYR1 gene and identification of two novel mutations in Italian malignant hyperthermia families.

Point mutations in the ryanodine receptor (RYR1) gene are associated with malignant hyperthermia, an autosomal dominant disorder triggered in susceptible people (MHS) by volatile anaesthetics and depolarising skeletal muscle relaxants. To date, 17 missense point mutations have been identified in the human RYR1 gene by screening of the cDNA obtained from muscle biopsies. Here we report single strand conformation polymorphism (SSCP) screening for nine of the most frequent RYR1 mutations using genomic DNA isolated from MHS patients. In addition, the Argl63Cys mutation was analysed by restriction

Abdominal Wall Pain: A Common Clinical Problem.

Abdominal wall pain (AWP) is a common and underrecognized cause of chronic abdominal pain. The etiology of AWP varies. History and physical examination are critical to an accurate diagnosis of AWP. Trigger point injection using either a corticosteroid, a local anesthetic, or a combination of both often gives relief of pain and is of diagnostic and therapeutic value. Increased awareness of AWP as a cause of chronic, nonvisceral abdominal pain can prevent fruitless searches for intra-abdominal pathology and reduce medical costs.
