A Case was demonstrated of perforated primary ulcer of jejunum in a young man aged 19. The ulcer was located on the antimesenterial side of the first jejunal twisting about 8 cm. from the duodeno-jejunal flexure. Besides this perforated ulcer, there was another one on the lower part of jejunum, just close to the mesenterial root, about 20 cm. away from the flexure. It was a covered perforation. Clinically, there existed a picture of acute abdomen; this patient was successfully treated by urgent laparotomy. A resection was made of the altered segment of jejunum in length of 40 cm. along with th…

The patterns of analgesic ingestion in gastric and duodenal ulcer patients were compared with those of matched community controls in order to ascertain differences that may exist between ulcer and nonulcer subjects of comparable age and sex. The differences sought concerned amounts and types of analgesics ingested. The types of analgesics studied were aspirin and acetaminophen, ingested either alone or together. Analgesics such as dextropropoxyphene and codeine were disregarded. It was found that there was a strong positive association between heavy analgesic intake and chronic gastric ulcer w…

The results of an endoscopic study of the incidence of gastritis and gastric cancer in gastric ulcer in a series of 112 patients 1 to 4 years (mean 2.6 years) after the initial examination are presented. Chronic antral gastritis was found in 48 % of cases and was mainly atrophic. In 10 cases both the antral and body mucosa were normal. In 11 cases the antral mucosa was normal before treatment and remained normal in most of them. During the observation period 5 patients died from other than gastrointestinal diseases. Eleven patients were operated on for complications or relapses of ulcer diseas…

Although hospitalizations and deaths attributable to peptic ulcer have decreased notably during the past decade, it is not certain whether this decrease is because of reduced incidence of new cases or changes in other factors, such as the severity of the disease. Several genetic traits associated with peptic ulcer have been recognized. Hyperpepsinogenemia I is the most prevalent. Peptic ulcer is a heterogeneous group of disorders with multiple genetic and environmental causes. One manifestation of the diversity of ulcer disease is the variety of physiologic abnormalities seen in patients. The …

Five cases of Mooren's ulcer in young Nigerians seen over a 2-year period, in Lagos, healed after limbal conjunctivectomy. The maximum follow up in 2 cases was 2 years. Previous trauma or corneal foreign bodies as seen in four of our cases seem to suggest the precipitation of Mooren's ulcer by various ocular insults. We believe that collagenolytic activity of the conjunctiva plays a prominent role in the pathogenesis of the ulcer and is supported by this report in which our five cases healed after excision of the limbal conjunctiva. Helminthosis, found in four of our five patients with Mooren'…

Three of 100 patients who underwent Nissen fundoplication developed gastric ulcers, all high on the lesser curve. Two patients presented one year after, and the third seven years after fundoplication. All healed with medical therapy, but a relapse in one patient necessitated partial gastrectomy. The pathogenesis, is not known, but vascular factors may be relevant. Patients complaining of epigastric pain after fundoplication require careful endoscopic examination for ulcer, especially near the cardia, since this may be a site of predilection.

Over the years, the aeromedical disposition of aircrew members with bleeding duodenal ulcer has been a vexing problem because two essential questions cannot be categorically answered: Is the risk of rebleeding less with medical or with surgical treatment? In either case, is the risk of rebleeding low enough to permit, with reasonable prudence, the resumption of aircrew duties by the aviator? To try to answer these questions, the author has reviewed a portion of the literature and a number of medical records of patients with bleeding duodenal ulcer. With this information, the author draws his c…

The child's gastroduodenal ulcer is not uncommon. Its symptomatology is notably different from the adult's and gives it a rather original aspect. The pain is often atypical, almost permanent and its rythm is not necessarily related to the meals. The psychological context and the family background are almost always disclosed. The digestive hemorrhage is easily revealing. Radiological traps are to be known. The treatment is virtually always medical. Cimetidine has been used only in complications, in which it has appeared very useful. Surgery is not called for but in case of perforation or hemorr…
